3 using collagen meniscus substitute: function of joint regulation 研究開発分担者 ( 日本語 ) 大阪大学大学院医学系研究科運動制御学助教小笠原一生所属役職氏名 : ( 英語 )Osaka University Graduate School of Medicine, Biomechanics and Motor Contral, Assistant Professor, Issei Ogasawara II. 成果の概要 ( 総括研究報告 ) 本研究開発は いままで有効な治療法がなかった欠損のある半月板損傷に対し 我々が開発した新規医療機器 コラーゲン半月板補填材 ( 特許第 号 ) を用いた治療の臨床実用化をめざした PMDA との事前相談 (H25 年 11 月 ) 対面助言(H ) フォローアップ面談(H ) にて合意した 1) 非臨床安全性試験と 2) 大動物安全性 有効性試験に加えて 3) ヒトでの手術に必要な手術機器や手術手技の最適化と 4) 術後の膝関節 運動機能改善の評価方法の開発研究と 5) 本品の製造会社と治験実施予定会社と定期的な打ち合わせを行い PMDA 規制当局との相談を行った その結果 1) GLP 準拠した非臨床生物学的安全性試験では 細胞毒性試験 埋植試験を実施し 安全性が確認された 2) ミニブタを用いた大動物試験を実施し 術後 2,4,8,12 週で コラーゲン半月板補填材は周囲組織より細胞が浸潤し 経時的に周囲組織との連続性と補填材の吸収がみられた 術後 6ヶ月 9ヶ月では コラーゲンは吸収され消失し 半月板の欠損部に補填した本品は線維性組織に置換され 半月板の形状は保たれていた 関節に炎症や関節軟骨の障害は認めず安全性 有効性が確認された 3) ヒトでの手術に必要となる医療機器と手技を検討し 本品を欠損に適合するサイズに切り出しして関節鏡視下に自家静脈血由来フィブリンクロットを用いて欠損部に縫合する方法を開発した 4) 本治療を用いた術前後の膝関節運動機能を 動作筋電図 重心動揺 床反力解析 動作解析の指標と 神経筋機能として動作の保持や滑らかさなどを三次元加速度計と我々が開発した動的バランス解析システムを用いて sub clinical なレベルで検知可能あることがわかった さらに MRI 画像をもちいて術前の半月板損傷のタイプ分類 欠損の有無 サイズなどの解析が可能であった 5) ヒト実用化にむけた治験プロトコールや実施方法などの考え方の相談を PMDA 薬事戦略相談を H に実施し 大動物試験での有効性試験とヒト臨床試験の経過の報告と試験デザインの基本的考え方として 対象疾患が比較対象となる従来治療がないため 比較対象試験ではなく単群試験での試験についての方法 問題点を明らかにした これらの成果により コラーゲン半月板補填材は ヒト実用化において安全性が高く 有効性を示しうる評価方法があり 実用化の可能性が高いことが明らかになった 英文 The purpose of this project was to clarify the possibility of clinical application of newly developed collagen-based meniscus substitute for a meniscus defect which thought to be irrepairable by means of existing treatment procedures. This project included non-clinical in vitro studies to show safety, animal studies to prove safety and efficacy, to develop suitable surgical procedure and surgical equipments, to establish evaluation methods of knee and motion function pre and post surgery, and to prepare and explore the method for the human 3
4 clinical trials by discussing with the manufacturing company, an appointed company which undergoes clinical trial, and PMDA. As results from this project, pre-clinical in vitro and animal study demonstrated safety and efficacy of this medical device for a treatment of meniscus defect and that there is a possible procedure to use this device in human. In addition to these results, new evaluation methods including MRI or motion analysis have possibility to show the efficacy in human clinical trials.
