He was referred to our department because of the enlarged left testis. The diagnosis of Leydig cell tumor of the left testis was suggested by hormonal laboratory data and testicular ultrasonographic investigation. Left orchiectomy was performed.the histological examination revealed the characteristics of Leydig cell tumor. Twenty-four cases of testicular Leydig cell tumor reported in the Japanese literature until 1988 are clinically analyzed. We discuss the usefulness of ultrasonography in detecting and localizing the tumor. Key words: Testicular tumor, Leydig cell tumor, Child (Acta Urol. Jpn. 35: , 1989)

3 甲 Fig.1.Ultrasonogramshowingawellcircumscribedroundmass(largearrows) withinthelefttestis(smallarrows) 難泌尿紀要35巻12号1989年購畷導聴鳶漁 2140 磯纏難欝騨灘 Fig.4.Microscopicappearanceofthetesticular tumor Fig.2.Grossappearanceofthelefttestis 20mmと腫大しており,その性状は弾性硬,表滑で圧痛なく皮膚との癒着は認められなかった.女化乳房,座瘡,変あった.内 ng/dlと FsHが32mlU/mlと正常範囲内で中テストステロンが196,8 中LHは1.OmlU/ml以下. 低下していた.DHEA-sは正常範囲内で,また尿中17-KS,17- 画像検査:睾レベルがやや低下していた(Fig.1).右常所見は認められなかった.頭部X線部CTで根骨X線以1:の撮影では骨年齢は11歳らにその周囲には正常睾丸組織が境界明瞭に認められた(Fig.2). 組織学的所見正常睾丸組織と腫瘍との境界は明瞭であるが,一部に腫瘍辺縁部に精細管が取り込まれている部分も認められた(Fig.3).腫瘍周囲の正常睾倍率では,明瞭な核小体を有し軽度に大小不同のある睾丸には異撮影,MRI も副腎の腫大は認められず, 所見より,Leydigcelltumorに黄褐色類円形の核と,好酸性で小穎粒を持つ豊富な胞体を示後腹膜リンパ節に転移を疑う所見も認められなかった.乎の腫瘍を,さ膜は正常面にて mmの睾丸内に径では下垂体および松果体腫瘍などを疑う所見は認められなかった.腹睾丸は mm,白に保たれていた.割囲よりエコー丸の超音波断層像では,左円形の腫瘤像が認められ,周左高位除睾術を施行した. 丸組織においては精細管の成熟傾向が認められた.高にも異常は認められなかった. 且2mmで症が疑われたため,1988年9月19日摘出標本:左般生化学検査には異常中AFP,CEA,HCGも高値を示し,血 OHCS値液,一分泌学的には,血 190ng/mlと性声は認められなかった. 検査所見尿所見,血を認めず,血面平に相当した. よる1{llメL 山来のテストステロンの分泌増加にともなう仮性早熟す腫瘍細胞の増生が認められた(Fig.4).明 Reinkecrystalは認められなかった.以りLeydigcelltumorとらかな上の所見よ確認された. 術後40日目の血1中テストテロン値は51.3ng/dlと術前の1/3以下に低下していた. 考本邦におけるLeydigcelltumorの察症例はわれわ
