6 ADR (The Naranjo adverse drug reaction probability scale) について副作用が疑われるときは必ず Score を出して入力してください The Naranjo adverse drug reaction probability scale; To assess the adverse drug reaction, please answer the following questionnaire and give the pertinent score Yes No Do not know 1. Are there previous conclusive reports on this reaction? Did the adverse event occur after the suspected drug was administered? Did the adverse reaction improve when the drug was discontinued or a specific antagonist was administered? 4. Did the adverse reaction reappear when the drug was readministered? Are there alternative causes (other than the drug) that could have on their own caused the reaction? 6. Did the reaction reappear when a placebo was given? Was the blood detected in the blood (or other fluids) in concentrations known to be toxic? Was the reaction more severe when the dose was increased or less severe when the dose was decreased? 9. Did the patient have a similar reaction to the same or similar drugs in any previous exposure? Was the adverse event confirmed by any objective evidence? Total Score The Naranjo adverse drug reaction (ADR) probability scale. The Naranjo criteria classify the probability that an adverse event is related to drug therapy based on a list of weighted questions, which examine factors such as the temporal association of drug administration and event occurrence, alternative causes for the event, drug levels, dose response relationships and previous patient experience with the medication. The ADR is assigned to a probability category from the total score as follows: definite if the overall score is 9 or greater, probable for a score of 5-8, possible for 1-4 and doubtful if the score is 0. The Naranjo criteria do not take into account drug-drug interactions. Drugs are evaluated individually for causality, and points deducted if another factor may have resulted in the adverse event, thereby weakening the causal association. The Naranjo ADR probability scale (Clin Pharmacol Ther 1981;30:239-45) 6

8 資料の送付方法についてお願い : 必須文書などを郵送される際下記の 1~6 を同封の上送付してください 1) 必須文書 ( 透明なクリアファイルに挟んでください ) 1 紙媒体電子ファイル製造販売後医薬品の副作用に関する報告書 ( 書式 12-1) の Word と PDF ファイル 2 紙媒体電子ファイル有害事象詳細調査票もしくはそれに準じたものの複写とその PDF ファイル (+ ADR or DIPS) 3 紙媒体契約書電子ファイルとして 1~2 の PDF ファイルと 1 の Word ファイルを電子媒体に入れるなお電子媒体は USB メモリなどで受け付けるこれらの電子ファイルと紙媒体の書類および事前に会計課担当者と内容を確認した契約書を薬剤部治験担当者に提出する契約書以外の書式への押印は原則不要とするただし依頼者により書式に押印された書類は受け付けるが医療機関から依頼者や責任医師へ交付する契約書以外の書式については押印不要としても構わない 2) 契約書等を返信するために宛先を記載し切手を貼付した封筒と担当者の名刺 3) 透明のクリアファイル 5 部 4) ブランク DVD-R 5 枚 ( リライタブルメディアは不可 ) 5) 次ページに示すようなファイルケース ( 四角い箱のような感じで積み上げて崩れないものであれば何でも OK です色も指定はありません ) に 1)~4) を入れて提出してください 8
