3 The Japanese-Language Proficiency Test (JLPT) July 2019 Instructions for Requesting Special Testing Accommodations -Overseas Edition- The Japan Foundation Center for Japanese-Language Testing Special testing accommodations may be provided for examinees with disabilities. Those who wish to apply for special testing accommodations should apply after reading this document in full. 1. Documents to be submitted: (1) Request Form (2) Application Form for JLPT (3) Attachment to Request Form (Medical Certificate, audiogram etc.) Please submit the above listed documents to the host institution in the city where you wish to take the test. It is also possible for a representative to apply on your behalf. The attachment (above (3) ) is not required in the case the same special testing accommodations has been given to you in the previous JLPT. Required attachment (above (3) ) is depending on special testing accommodations request. Please refer to 2-6 page of the Request Form for detail. Please refer to page 3-4 for extended test time. * In compliance with the GDPR, those who are applying from the European Economic Area (EEA) must also submit the Consent Form. Please refer to the privacy policy of the Japan Foundation for the applicants from EEA at page 2-4 of the Consent Form. 2. Application period: Please contact the host institution for confirmation of the application period. 3. Decision process regarding the implementation of special testing accommodations: (1) The Japan Foundation, which organizes the test, in consultation with the Committee for Special Testing Accommodations, will decide what special testing accommodations should be put in place based on the contents of request form and attached documents. (2) If the information provided on the request form is not sufficient, the Japan Foundation may contact applicants or their representative to request further information. (3) Applicants or their representative will be notified of the final decision regarding the special testing accommodations around the beginning of June. (4) All communication between applicants or their representative and the Japan Foundation will be conducted through the host institution. 4. Notification of results: The notification of test results will not make any mention of the special testing accommodations, except for Test exemption, unless applicant requests to the contrary.
