Transcription
1 Seborrheic keratoses (SK) are verrucous dark-brown nodules or elevated macules, which develop on the face and the body mainly in elderly patients. They are often multiple in seborrheic areas and show epidermal proliferation with hyperkeratosis. Lipophilic yeasts of the genus are members of the normal human cutaneous microflora and are also associated with several skin diseases including atopic dermatitis and seborrheic dermatitis. We investigated the roles of genus in SK by microflora analysis using nested PCR. and were preferentially detected in SK group. Aging and a lack of face cleansing are important factors in the detection rate for both species. DNA microarray analysis in normal human keratinocytes incubated with for 24 hours revealed transcriptional changes toward epidermal proliferation, keratinization, and up-regulation of lipid metabolism, which provide beneficial environments for the growth of spp.

3 高齢者顔面における常在真菌マラセチアのフローラ 脂漏性角化症との関連について 腫状に隆起しており 表面に鱗屑が付着して角化性である 族との関連を想起させる 図 2e の症例は入浴の際にほと ことがわかる 図 1c, d 顔面以外について観察すると んど石鹸を用いないという方であるが 臍と臍周囲に脂漏 非露光部である腹部の溝に脂漏性角化症が多発する例 図 性角化症が多発していた 特に臍の脂漏性角化症は油粕の 2a や代表的な脂漏部位である頭皮に脂漏性角化症が多発 ような厚い鱗屑があかつき状に付着しており ここからも する症例 図 2b があった 腋窩付近の側胸部に脂漏性 多種のマラセチアが検出された 角化症と癜風様の紅褐色斑が共存する場合もあり 図 2c この紅褐色斑を擦過すると多量の鱗屑が得られ KOH 検 鏡で菌糸 後にマラセチアと同定 を確認できた 図 2d ３-２ マラセチア族の菌相解析 マラセチアの rrna 遺伝子について nested PCR により 図 2a c の症例はいずれも多汗ないしあぶら症と自覚し 菌種同定し 図 3a クロモアガーマラセチア / カンジダ ており 脂漏性角化症の発生と汗や脂質を好むマラセチア 生培地 図 1 顔面に生じた脂漏性角化症の観察 a: 長期日光暴露者に生じた脂漏性角化症 ｂ : 脂漏部位であ る生え際と外耳道に脂漏性角化症が多発する例 c: 脂漏性 角化症の拡大像 d: ダーモスコープによる脂漏性角化症の 観察 を用いた培養で各マラセチアを分離した 図 3b 図 2 顔面以外に生じた脂漏性角化症の観察 a: 腹部の皺溝に多発する脂漏性角化症 b: 被髪頭部に多発 する脂漏性角化症 c: 腋窩付近に多発する脂漏性角化症と癜風様紅褐色斑 d: KOH 検鏡で菌糸を確認 e: 臍と臍周囲に多発する脂漏性角化症 入浴の際にほとんど 石鹸を用いない例 図 3 マラセチアフローラの菌相解析 a: PCR によるマラセチアの菌相解析 b: クロモアガーマラ セチア / カンジダ生培地 を用いたマラセチアの分離培養 87
