33 Discussion
34 今回の結果は血小板の予防的輸血の必要性を支持する 出血イベントの減少という点で予防の benefit がある しかし 予防群でも 43% の出血イベント発生がある 今回の研究参加者は自家幹細胞移植患者が多いが これらの患者では出血イベント発生率は二群間でほぼ同じ最近の trial で 自家移植患者でも WHO grade2 の出血イベントは非予防群で多いという結果が出ており 今回の結果と異なるしかしその trial では自家幹細胞移植患者では血小板の治療的投与のみでよいだろうと結論されている 今回の研究からはそこまでの断定はできず さらなる research が必要 化学療法や同種幹細胞移植の患者では予防的輸血の効果が大きい
35 今回の trial の長所はプロトコールの遵守率 脱落がほぼ 0 であることであった WHO の出血グレードの決定に関しても コンピュータアルゴリズムの使用は manual grading method に比較して有効であった limitaaon として 各施設での出血評価が不均一である可能性があるこの点に関しては 書類や出血の記録の標準化 また出血の評価者がトレーニング モニターを受ける といった手法がとられた 血小板数の許容範囲を決定することは難しい ITT 解析と per protocol 解析が両方行われたが 両者の結果は異なった primary outcome の設定として輸血の回数ではなく 臨床的に関連のあるアウトカムの選択は難しい 造血器腫瘍患者において予防的血小板輸血は出血を減少させる

37 低増殖性血小板減少の患者で 予防的血小板数のトリガー値を へ下げ 使用する血小板の dose を半分にしても出血頻度は変わらないという研究が報告されている (Rebulla et al. NEJM 1997;337:1870-5) (Slichter SJ et al. NEJM 2010;362:600-13) そこで次に出てくる疑問は予防的血小板輸血が本当に必要なのか治療的血小板輸血のみで効果的に support できるのかということである 本研究では先行研究より出血低リスクの患者を対象としていたが非予防群で出血頻度が増大するという結果になった
38 血小板輸血を受けた率は予防群 > 非予防群だが 患者を出血リスクにさらすことは血小板輸血を減少させるというだけでは正当化できないであろう今回の研究も academic な施設だからできたことである 今回の study では 16 歳以上が対象であった先行研究では小児では自家移植患者でも有意に出血が多かったとの報告がある (Josephson et al. Blood 2012;120:748-60) 血小板輸血に関する出血リスクの評価は小児へは適用しにくい

40 予防的輸血を受けても 40% 強が出血率というのは多いのではないか 非予防的投与の非劣性は示されず 今回の結果からは予防的投与を続けることに妥当性があると思われる非予防群で 1 例であるが頭蓋内出血を起こしていることも無視できない また 患者を盲検化できないため 非予防群に割り当てられた患者が出血を引き起こす行為を忌避していた可能性もある 本研究のプロトコールを実際の臨床に適用すると出血率がさらに高くなるかもしれない しかし 成人の自家幹細胞移植患者では出血率に差がついておらずこのカテゴリーの患者では治療的輸血のみでよい可能性もある今後は 治療法 疾患ごとの予防的輸血の必要性についての研究がのぞまれる
