1 症例報告乳腺アポクリン癌の 2 症例森下真由美 1) 芳林浩史 2) 宮木康夫 1) 小野一雄 3) 岡本 1) 日本赤十字社和歌山医療センター検査部 ) 日本赤十字社和歌山医療センター乳腺外科部 3) 日本赤十字社和歌山医療センター病理診断科部要大輔 1) 湯月洋介 1) 和歌山県和歌山市黒田旨アポクリン癌の 2 症例を経験したのでその乳腺エコー画像の特徴について報告する症例 1 は 60 歳代女性アポクリン腺症と診断後 6 年間の長い経過で乳腺腫瘤の大きさに著変ないものの乳腺エコーで前方境界線の断裂微細点状高エコースポットおよび血流シグナルを認めることより乳頭腺管癌を疑い再度の組織診にてアポクリン癌と診断された症例であった症例 2 は 60 歳代女性 MRI で病変の指摘あるも乳腺エコーで明らかでなくおよそ 3 年の経過にて乳腺エコーで不整な低エコー腫瘤を認め硬癌を疑い組織診にて非浸潤性アポクリン癌と診断された症例であったアポクリン癌の乳腺エコー画像の特徴として 2 症例とも不整な低エコー腫瘤として指摘されたいずれも良性病変として経過観察されていたが長い経過にて乳腺エコーで悪性の可能性がある所見が認められた腫瘤の大きさの変化に関わらず性状を注意深く観察することは重要であり長期間でのその性状変化をとらえるのに乳腺エコーは有用であったキーワード乳癌アポクリン癌非浸潤性アポクリン癌アポクリン腺症乳腺超音波検査 I はじめに II 症例乳腺の病理診断においてアポクリン化生 apocrine 症例 1 60 歳代女性 metaplasia は乳腺症の組織学的変化に含まれ良性既往歴子宮筋腫病変を示唆する指標の一つとされている生活歴出産 2 回また 1 ) 3 ) アポクリン化生を伴う癌細胞が主体の浸潤癌である家族歴父大腸癌アポクリン癌は細胞像で定義される組織型であり現病歴 2007 年 12 月乳がん検診のマンモグラさまざまな組織構築を呈するため特徴的な画像所フィで異常を指摘され当院外科受診された触診見はないとされているは異常なしマンモグラフィは頭尾方向撮影 cranio- 1) 今回われわれはアポクリン癌と診断された 2 症例 caudal; CC で右 O に局所的非対称陰影 focal を経験したので当院の倫理委員会の承認を得た上 asymmetric density; FAD を指摘された Figure 1 でその乳腺超音波以下エコー画像の特徴に乳腺エコーでは右 CD 領域に大きさ約 15 mm の不整ついてまとめて報告するな低エコー腫瘤が認められカテゴリー 3 と判断した Figure 2 穿刺吸引細胞診が施行されたがクラス II であった半年後の経過観察でも腫瘤が残存したので針生検を行いアポクリン腺症 apocrine 平成 27 年 3 月 23 日受付平成 27 年 9 月 15 日受理 692

4 Figure 9 Ultrasonography showed a tumor in BD region of the right breast that was irregular shaped, and irregular-surfaced, with a heterogeneous low-echoic level inside, interruption of the border of the mammary gland, and no amount of flow Figure 10 Microscopic findings revealed proliferation of cells with apocrine metaplasia, with cribriform papillary in dilated and transformed mammary duct, close to granulation tissue which seemed to biopsy scar 疑う漸増性濃染パターンの境界明瞭な腫瘤を指摘さごくまれに非浸潤性乳管癌の型をとりながら癌細胞れた Figure 8 このとき乳腺エコーで右 BD 領がアポクリン化生を示す症例もみられるがその組域に明らかな腫瘤は指摘されなかったその後経織学的分類については乳癌取扱い規約 2012 年の第過観察されていたが 2012 年 9 月右乳房の痛みの 17 版で注記としてアポクリン癌はあくまでも浸潤ため受診された乳腺エコーで右 BD 領域に大きさ癌であるが間質浸潤がなければ非浸潤性アポクリ 6 mm 程度の不整な低エコー腫瘤が認められン癌となると記載されている 2) Figure 9 エコーガイド下マンモトーム生検にてアポクリン癌の臨床的特徴は発生年齢は 60 歳前非浸潤性アポクリン癌と病理診断された後とやや高齢者に多く予後は良好特徴的な画像 Figure 10 所見が少ないホルモンレセプター陽性率が低いなどである 3) 8) さらに乳腺エコー所見としては不 III 考察整な低エコー腫瘤として指摘された報告が多いようであるが長谷川ら 4)は本邦における非浸潤性乳腺乳腺腫瘍の組織学的分類でアポクリン癌は浸潤癌アポクリン癌 27 例のまとめで明らかな腫瘤として乳腺での発生はまれであ描出できないものも約半数あったと報告しているりその頻度は乳癌全体の 1%程度であるが近年症例 1 は 6 年半の長い経過で乳腺腫瘤の大きさには増加傾向を示しているアポクリン癌はアポク著変ないものの乳腺エコーで性状変化を認め再リン化生の分化特徴を示す浸潤癌と定義されその度の組織診にてアポクリン癌と診断された症例で組織形態は浸潤性乳管癌と酷似しているさらにあった乳腺エコー所見は形状は不整形内部低の特殊型に分類される 1 ), 2 ) 3) 3) 695

5 エコー不均質境界は明瞭粗ぞう後方エコー不が認められなかった経過観察中の乳腺腫瘤におい変縦横比は小さかったさらに悪性を疑う所見とていくつかの modality で特に変化を認めない場合して前方境界線の断裂微細点状高エコースポットでも乳腺エコーで腫瘤の大きさの変化に関わらずおよび内部に点状の血流シグナルが認められエラ注意深く観察し性状に悪性を疑う変化を認める場ストグラフィはスコア 4 エコー性状より乳頭腺管合には積極的に再度の針生検および切除生検などの癌を疑った一方症例 2 はマンモグラフィと MRI 施行が必要であると思われたさらに質的診断に適で線維腺腫を疑う病変の指摘あるも乳腺エコーで明した乳腺エコーがその性状変化をとらえるのに簡便らかでなかったがおよそ 3 年の経過にて腫瘤としかつ有用であると思われたて指摘され組織診にて非浸潤性アポクリン癌と診断された症例であった乳腺エコー所見は形状は IV 結語不整形内部低エコー不均質境界は明瞭粗ぞう後方エコー減弱縦横比は大きかった前方境界線の断裂あり境界部高エコー像 halo 微細点状高エ乳腺アポクリン癌の 2 症例を経験したので報告したコースポットおよび血流シグナルは認められずエコー性状からは硬癌を疑ったアポクリン癌の乳腺エコー画像の特徴として 2 症例とも不整な低エコー腫瘤として指摘されたいずれも画像検査で良性病変として矛盾しなかったが経過観察中に乳腺エコーで悪性の可能性がある所見が認められた症例 1 では境界線の断裂微細点状高エコースポットおよび血流シグナル症例 2 では大きい縦横比と境界線断裂の指摘であったそのため症例 1 のように一度アポクリン腺症の診断後もやはり経過観察は重要であると考えるアポクリン腺症はアポクリン癌との鑑別が問題となる一方癌化にいたる過程は不明であるが Seidman ら 9)の研究では atypical apocrine adenosis では浸潤癌発生の相対リスクは 5.5 であったと報告しているまた症例文献 1) 日本乳腺甲状腺超音波医学会乳房超音波診断ガイドライン改訂第 3 版南江堂 ) 一般社団法人日本乳癌学会臨床病理乳癌取扱い規約第 17 版金原出版東京 ) 黒住昌史森谷卓也腫瘍病理鑑別診断アトラス乳癌文光堂 ) 長谷川久美他非浸潤性乳腺アポクリン癌の 1 例日臨外会誌 2011; 72: ) 原内大作他非浸潤性乳腺アポクリン癌の 1 例日農医誌 2013; 62: ) 尾田三世他非浸潤性アポクリン癌 Apocrine carcinoma in situ の細胞所見日臨細胞広島会誌 2012; 33: ) 佐藤文彦他興味ある両側アポクリン癌の 2 症例乳癌の臨床 2011; 26: ) 杢谷友香子他当院における乳腺アポクリン癌 24 例の検討乳癌の臨床 2012; 27: ) Seidman JD et al.: Atypical apocrine adenosis of the breast, Cancer, 1996; 77: はいずれもマンモグラフィおよび MRI で経時的変化 696

Case 1: A 60-year-old woman with a right breast tumor was initially diagnosed as having apocrine adenosis. No remarkable change was seen in the size of the mammary gland. Six years later, because of the interruption of the anterior border of the mammary gland, a fine high-echo spot, and a small amount of flow detected on her ultrasonograms, we suspected papillotubular carcinoma. Then, histopathological examination by US-guided core needle biopsy revealed apocrine carcinoma. Case 2: Screening mammography pointed out focal asymmetric density (FAD) in the right breast of a 60-year-old woman. Her lesion could be detected by magnetic resonance imaging (MRI) but it could not be observed by US. Three years later, US showed an irregular and low-echoic mass. At first, scirrhous carcinoma was suspected, and then histopathological examination revealed intraductal apocrine carcinoma. These two patients with apocrine carcinoma were diagnosed on the basis of irregular and low-echoic masses as shown in the US. Both patients were followed up as initially having benign tumor. A few years later, US findings showed that there is a possibility of malignancy. US was found to be useful for long-term follow up.
