NSCET 法にて 90% の狭窄を認め Fig. 1C 同部 症例呈示 位は CT にて著明な石灰化病変に覆われていた Fig. 症例は 78 歳 男性 右陳旧性脳梗塞と両側内頚動脈 1 ま た MRI で は プ ラ ー ク は T1 強 調 画 像 black 狭窄症のため抗血小板薬 aspirin 100 mg/ 日 cilostazol blood 法. 法 にて 一部低信号域を認めるが 多 200 mg/ 日 による加療を行っていた 頚動脈エコーに くは胸鎖乳突筋よりやや高信号を呈しており不安定プラ て経過観察を行っていたが 徐々に両側とも狭窄が進行 ークが混在している可能性が考えられた Fig. 1 一 した 最大収縮期血流速度は右側 244 cm/sec 左側 方 左側病変は DS にて NSCET 法で 85% の狭窄 231 cm/sec と上昇しており プラークは両側とも低 を認め 狭窄部はやや蛇行していた Fig. 2C また 等 輝 度 で あ っ た 脳 血 管 撮 影 digital subtraction プラークは CT にて一部に石灰化を認め Fig. 2 angiography DS を施行すると 右側病変は North MRI. 法にて胸鎖乳突筋と比べやや高信号であり merican Symptomatic Carotid Endarterectomy Trial DS 上 右側よりもプラーク量がやや多い以外には 196 JNET Vol.5 No.3 February 2012
