3 耳展 小 54 1 林 賀 子 他 図 1 耳介局所所見 初診時所見 左耳甲介にびらん 痂皮付着 黒色化があり 易出血性であった 再診時所見 皮膚の欠損 軟骨の露出 中心部に肉芽 周囲にびらんや黒色化 疼痛 出血がみられた 図 2 再診時病理所見 H E 染色 N/C 比の高い細胞が大小多数の不規則な胞巣を形成し 辺縁に核の柵状配列が存在していた 軟骨へ の浸潤があった 基底細胞癌と診断した 図 3 頸部造影 CT 左耳介から外耳道入口部にかけて造影効果のある境界 不明瞭な領域があり 一部耳下腺内への浸潤を疑った 矢印 図 4 術中所見 腫瘍摘出後 耳下腺浅葉を合併切除した 欠損部を側 頭筋弁で充填した 摘出部位を黒枠で示す

It is difficult to dignose BSCC efore surgery. The recurrence rte of BSCC fter surgery is high, since the surgicl mrgin frequently shows the fetures of BCC. We present cse of BSCC originting in the uricle. The ptient ws dignosed s hving BCC invding the crtilge efore the opertion. We performed n opertion indicted for invsive crcinom.
