1 Pressure Distribution Measurement under the Foot on Walking in Rheumatoid Arthritis Junichi MONJ I et al. Department of Orthopedic Sargery, School of Medicine, Hokkaido University Abstract Foot pressure measurement under the rheumatoid arthritic foot has been performed using a newly designed gum sensor measurement device. The sensor can pick up the pressure from changing capacitance. The sensitivity of the sensor is below 20g/cm2 and the range is from 450g/cm2 to 2500g/cm2. The device is composed of 960 of these sensors and 8 frames/second of foot pressure measurement can be made. The clinical measurement was made on classical rhuematoid arthritic patients. The pressure distributions and foot deformities could be divided into 3 types. Type 1 had minimum foot deformity but the pressure under the toes was decreased (5 feet). Type 2 had a moderately deformed forefoot and there was almost no pressure under the toes (6 feet). Type 3 was a markedlly deformed foot with no forefoot pressure distribution (10 feet). In rheumatoid arthritic patients, the peak of the pressure was 2 times larger than that of normal persons (pressure/cm2 /weight) and the location of the peak was shifted laterally. These results suggest that sole pain in rheumatoid arthritic is related not only to static foot deformity but also to the dynamic abnormality in walking.
3 421 図1足 底 圧 計測 装置 に用 い たセ ンサ ーの 基 本特 性 セ ンサ ー にはJISゴ ム硬 度50度 の ゴ ム を 用 い,加 えた圧 に よ る電 気容 量 の 変化 を検 出 す る もの で あ る 図2足 底 圧 計測 装 置 の 外観 装 置 は 一 辺1cmの ゴ ム セ ン サ ー を960個 も の で あ り,こ れ を8KHzでScanし,1秒 し きつ め た 間 に8フ レー ム の計 測 が 可 能で あ る パ ター ン は くず れ て お り,立 脚 相 の か な り早 期 Type1:外 観 上 の 前 足 部 変 形 は 認 め られ な い か ら足 底 全 体 を接 地 す る傾 向 が認 め られ た こ か あ っ て も極 め て 軽 度 で あ る もの で,歩 行時足 れ を足 各部 の 立 脚 相 で の最 大 圧 で 評 価 す る と, 底 圧 計 測 で は踵 部 の 圧 が 正 常 に比 べ て相 対 的 に 外 観 状 の足 変 形 との比 較 で 以 下 の3つ の パ タ ー 減 少 して お り,一 方 中 足 骨 頭 部 へ の 圧 分布 が 増 ン に大 き く分 類 さ れ た 加 を 示 し,趾 へ の 圧 分 布 が減 少 して い る もの(図

5 423 の 値 と比 較 検 討 した 正 常 者 と し て は 足 変 形 の な い 成 人 男 子5名 考 を用 い た 中足 骨 頭 部 の 母 趾 側 と小 趾 側 との 幅 を100と し て,最 比 を 求 め る と,正 常 者 で は 母 趾 側 か ら 約30%の と こ ろ に ピ ー ク が 存 在 し,そ RAに 大 圧 の体 重 こ か ら小 趾 側 に 向 察 お け る前 足 部 変 形 は開 張 足 変 形 と称 さ れ る横 軸 扁 平 足 と外 反 母 趾 変 形,か とか ら な っ て い る RAの ぎ爪 趾 変 形 足 部罹 患 の進 展様 式 か っ て な だ ら か に値 が 低 下 す る 曲 線 が 得 られ る に は個 体 に よ る違 い が あ る もの の,典 型 的 な 前 の に 対 し,Type2のRA例 足 部 変 形 を 有 す る もの で は長 軸 扁 平 足 を呈 し, で は ピー ク の位 置 が 母 趾 と 小 趾 の ほ ぼ 中 間 に 位 置 し て い た RAで の ピ ー ク値 は 体 重 の 約3.5%で 1.8%に あ り,正 常対照 比 べ て 明 らか に 高 値 を 示 し た(図5) 図4a.外 足 根 骨 も癒 合 して い る もの が少 な くな い 従 っ てRA前 足 部 変 形 に対 す る整 形 外 科 的 治 療 の 立 案 に 当 た っ て は,こ れ ら足 部 変 形 の 全体 に対 す 観 上 の 変 形 は 軽 度 で あ る が,足 底 圧 の 計 測 で 趾 へ の 圧 分 布 が 減 少 し,中 足 骨 頭 部 へ の 圧 分 布 が 上 昇 し て い る もの (Typel:5足) 図4b.変 形 の 進 行 と と もに歩 行 時 足底 圧 計 測 で は趾 へ の 圧 分 布 が ほ と ん ど 認 め ら れ な い (Type2:6足)
6 424 図4c.変 形 が 高 度 で,足 底 圧 の 計 測 で は 趾 を 含 め た 前 足 部 の 圧 分 布 が 減 少 し,踵 部 に 相 対 的 に 大 き な 圧 分 布 を 認 め た も の (Type3:10足) る 治 療 が 考 慮 さ れ な け れ ば な ら な い 一 般 に RAのStageが 進 行 した もの で は 日常 生 活 で の 活 動 性 が 全般 的 に低 下 して い る こ とか ら,前 足 部 変 形 に対 す る整 形 外科 的 治 療 は疼 痛 の除 去 が 第一 義 とな り,加 え て靴 の 装 用 を 可 能 にす る こ とが 企 図 さ れ る場 合 もあ る こ の う ち 疼 痛 は MP関 節 の 滑 膜 炎 に よ る もの,裸 足 歩 行時 の 主 と して 足 底 の 有痛 性 ベ ン チ に よ る もの,さ らに 靴 の 装 用 に よ り生 じ る痛 み とに臨 床上 大 別 さ れ る この うち有 痛 性 足 底 ベ ンチ は骨 の 配 列 の 異 常 に よ り皮 膚 に対 す る圧 迫 力 が 上 昇 した 結 果 生 じ る 今 回 の 歩 行 時 の 足 底 圧 の計 測 に よ り以下 の 点 が 明 らか に な っ た RAで は歩 行 速 度 が 遅 くま た 体 重 が 軽 い こ とは,足 底 圧 を減 少 さ せ る因 子 図5中 RAで 足 骨 頭部 で の足 底 圧 分 布 は 体 重 が 軽 く歩 行 速 度 も遅 い の に か か わ ら ず,中 足 骨 頭 部 に加 わ る面圧 は 対 体 重 で正 常 に比 べ 明 らか に高値 を示 す また ピー ク は母趾 側 と小 趾側 との ほぼ 中央 に シ フ トして い る
