1 研修機関名 :Children s Hospital of Philadelphia 小児外科 PICU 研修期間 :3/26-5/18 Children s Hospital of Philadelphia (CHOP) の小児外科と PICU で一か月ずつ実習を行 いました 前者は elective(rotation, subinternship などと同義 ) 後者は observership ( 実習先によってどれくらい参加型になるかは異なるが 基本的には見学 ) でした CHOP 小児外科 (elective) 概要かなり有名な小児外科で myelomeningocele の胎児治療などで世界的に有名な Dr. Adzick 人工子宮の研究で世界的に有名な Dr. Flake など著名な先生方が多く在籍している 準備東大はペンシルバニア大学 (UPenn) と協定を結んでいるため 元々小児科志望で CHOP で実習したかった私は迷わず応募した 東大からは毎年最多で二名派遣されるが 応募者は私一人であった おそらく TOEFL で 26 (Speaking) + 26 (Listening) + 22 (Reading) + 22 (Writing) と高得点が必要であったことが原因かと思われる 私は長年英語圏で過ごしていたため問題なかったが 日本で育った学生でこの点数を超えている人もいるので 決して不可能ではないはずだ 余談だが 5 年前に受けた TOEFL の点数で大丈夫かどうか UPenn に問い合わせたところ受けなおすよう指示を受けたため 再受験せざるを得なかった (TOEFL は受験料が高額だが有効期限が二年しかない ) ただ elective で学費がかからないのは珍しく 小児以外の分野でも UPenn は有名で優秀な学生 有名な先生方も多いため 強くおすすめする 他大に応募した同級生の話を聞くと かなり高額なお金がかかる上にあまり留学先の対応がしっかりしていないということも少なくなかったようで その面では UPenn は素晴らしい留学先だったと感じる 逆に言うと TOEFL という壁さえ乗り越えれば (UPenn は USMLE もいらない ) UPenn への留学は米国の他の elective に比べるとかなり簡単である その他手続きについては国際交流室のサポート 受け入れ先のサポートともにしっかりしていたため 特に問題は生じなかった UPenn の elective は実習したい科をランク付けして提出する方式だが 私は小児外科は第 6 志望だった ( 第 1-5 志望は小児科の subspecialty) 一般的な小児科はアメリカでは不人気であるが やはり CHOP では人気らしい 実習内容アメリカの実習ということで実際に患者を数名担当し 毎日プレゼンしてプランを立てる というものを期待していたが 実際は手洗い ( scrub-in ) できるのみで患者を担当することはできなかった 患者の数がかなり多いこと (50-80 人の患者を fellow3-4 人と resident3-4 人で分担する ) と 小児病院でかなり複雑な症例が多いことから 学生が患者を担当するのは難しいということなのだと感じた 手洗いするのみとはいえ CHOP の小児外科で担当できる症例は一般の病院では見ることの
2 できないものばかりで かなり貴重な機会となった 小児科志望の医者でもこれほど豊富な症例を見ることはできないだろうと感じた 一日 3-5 件のオペで 症例としては laparoscopic G-tube placement などすぐ終わるものから tracheoesophageal fistula repair や myelomeningocele の胎児手術など複雑なものまで様々であった ( 具体的な症例については以下の one-liners を参照 ) まとめ手洗いできるという点以外では参加型実習とは言いづらい実習ではあったが 症例の豊富さで言うとものすごく濃い実習となった 小児外科を考えている学生や 私のように小児科に進むことを決めているが学生のうちに小児外科を見ておきたい学生にはおすすめの実習である One-liners An infant M with gastroschisis s/p repair p/w FTT and recurrent vomiting for laparoscopic funduscopy and GT placement A neonate with OEIS complex presents for 1st stage of cloacal reconstruction and ileostomy. A young F child p/w malnutrition of unknown etiology presents for lap GT placement and umbilical hernia repair. A F neonate with possilble VACTERL syndrome presents for thoracoscopuc TEF repair. A previously healthy teenager M p/w pain in buttock presents for perianal cyst resection. A teenager F with obesity c/o sudden onset pain presents for salpingoopherectomy. A young F child with posterior mediastinal cyst presents for right thoracoscopic excision of cyst. An infant M with gastroschisis and 2 intestinal atresias s/p repair and NEC presents for takedown of ileostomy. A F neonate w/ GERD presents for lap funduscopy. A M child with skin lesion presents for excision of skin lesion on L hip.
3 An infant F with CCAM presents for thoracoscopic left lower lobectomy. A F child with no significant medical problems p/w RUQ abdominal pain from OSH c/f cholecystitis, and presents for laparoscopic cholecystectomy. An infant M with inguinal hernia presents for inguinal hernia repair. A F child with rectal aneurysm s/p Swenson s, trisomy 21, rectovaginal fistula, and recurrent UTI presents for cystoscopy and EUA of rectovaginal fistula. An infant female with cloacal anomaly and mullerian duct anomaly with prolapsed colostomy s/p panendoscopy/cytoscopy and transverse colostomy presents for colostomy revision and cystoscopy+vaginoscopy (for planning vaginoplasty). A previously healthy teenager M p/w intermittent, severe umbilical pain that subsided. Afebrile. Lap appy. An ex-32 wk F female w prenatally diagnosed gastroschisis and intestinal atresia presents for delivery with c-section and gastroschisis repair. A previously healthy young M child p/w severe pain from last night and emesis. US and CT scan significant for appendicitis. Lap appy. A teenage M w multiple closed loop bowel obstructions presents for G-tube placement, LOA, and exploratory laparotomy. A teenage F w 1 day of abdominal pain and nausea. US showed no flow to right ovary. Lap detorsion. A F infant w direct hyperbilirubinemia and jaundice presents for Kasai procedure. A F neonate presents for rectovestibular fistula repair, r/o VACTERL. A F neonate w/ CDH, PH, PFO, and PDA presents for CDH repair. A teenager F w paraspinal chest mass presents for resection of mass.

CHOP PICU (observership) 概要 50 床を超える巨大な PICU 看護師 Resident や Fellow の教育用にシミュレーションプログラムも多く用意されており 勉強にはかなり適した環境であった 準備元々他大の observership に応募し 準備を進めていたが 連絡を取っていた担当者と実習開始の数週間前から急に連絡が取れなくなり ( 元々返信が非常に遅いなど対応はいい加減であった ) 急遽 CHOP のウェブサイトで見つけた PICU に在籍されている日本人の先生に連絡を取ったところ 快く受け入れてくださった 準備は elective より楽だが 2 週間ほどしか準備期間がなく 少し大変だった Observership は東大と UPenn の協定とは関係なく 費用は 9 万円ほど 実習内容やはり observership ということもあって患者の診療には一切かかわることができなかったが supervisor の先生の面倒見が非常に良かったこともあり かなり勉強になった 内容としては午前のラウンドに参加し 午後は手技などを見る というものだが 週に数回 supervisor の先生にその日見た症例 疑問に思った点 その疑問点に対し最新の文献を使って調べたレポートをメールで提出し フィードバックをもらうことができた ( やりとりはすべて英語 ) そのため PICU に関する知識がどんどん増えていき 4 週目にはレジデントが答えられないような質問にも答えられることが増えたほど まとめ実際に患者さんとふれあい 診療に参加したいという方にはやはり observership はおすすめできないが 知識をつけるという点では supervisor によってはかなり良い経験となるはずである
