Extensor lag after the total knee arthroplasty for the knee oseteoarthritis Ryota Sakamoto Seiichi Takemasa and Norikazu Nakagawa ABSTRACT The purposes of this study were to clarify the relationship between the muscles activities during the knee extension and the extensor lag occurred after the total knee arthroplasty (TKA) for the knee oseteoarthritis. Twenty cases (25 knees, mean age 74.0 ± 5.79) performed the TKA at Suita Municipal Hospital were investigated. They were divided into the two groups based on the existence of the extensor lag at postoperative 3 weeks : extensor lag (+) group (15 knees) and extensor lag ( ) group (10 knees). The muscle activities around the knee joints during extension were examined and the longitudinal change of pre and post operative muscle activity in 6 knees was analyzed. As results, in the extensor lag (+) group, there was excessive activity in the tensor fasciae latae muscle and especially in the sartorius muscle. The excessive activity existing before the operation remained after the knee was aligned properly by the surgery. It was assumed that the excessive activity of the sartorius muscle is one of the factors that cause extensor lag after the TKA. These results suggest that postoperative architectural joint reconstruction dose not improve preoperative aberrant muscle acivity pattern immediately. Therefore, physiotherapy intervention from an early postoperative period is important for muscle activity pattern improvement.
