2 間部 2 最後端の距離を平均したものを DH とした Fig. 1 a また自然歴による椎間板高の低下の程度を調査するため今回対象となった全 22 症例の C2/ C7/T1 までの罹患椎間を除く頚椎全椎間板高位における DH も上記と同様の手法にて測定した DH all level Functional segmental height FSH 中間位頚椎側面写真における罹患椎間を挟む上位椎体の頭側終板およ 2 1 び下位椎体の尾側終板に平行線を引きその前方端 1 中間部 2 最後端の距離を平均したものを a FSH とした Fig. 1 b また局所後弯角 focal kyphosis FK Cobb s angle に準じ側弯角 scoliosis 罹患椎間板の角度の変化 focal angle ΔFA 前後屈位での棘突起間距離の変化 spinous process distance ΔSPD すべり ΔSlip を測定した統計学的処理には paired t test unpaired t test を用いて検定した術式 ACF 体位仰臥位にて頚部を軽度伸展位とし回旋は行わず正中位とする皮切から椎体前面へのアプローチ罹患側よりアプローチする皮切から頚椎前面にいたるまでは通常の ACDF と同様である前述のごとく術前に全例脊髄造影を施行しておりその際同時に CTM も撮像し病変の局在を同定する椎体削除の部位を決定するに際してはこの CTM 所見が最も重要であるまず CTM の冠状断再構成画像にて罹患神経根あるいは硬膜管の陰影欠損が最大になるスライスにて病変の局在を決定する次にこの地点をマークしつつ CT のスライスを椎体前縁までずらし罹患椎間椎体における骨削除部分を決定しこれを目標に骨削除を進める骨孔の作成顕微鏡下にドリルを使用する椎体椎間板の削除の目安としては罹患椎間板を骨窓の中で見失わないよう留意しつつ幅 7 mm 長さ 10 mm 程度を b 1 2 Fig. 1 The measurement methods of disc height DH and functional segmental height FSH a Measurement of DH using plain radiography in neutral lateral position. Two parallel lines are drawn to the cranial endplate and caudal endplate and the following are measured 1 Ventral end of disc FSH 2 Medial FSH Dorsal end of FSH Average FSH was defined as 1 2 /. 原則とし深部では末広がりになるようにする Fig. 2 b 外側はできるかぎりルシュカ関節は温存し椎体外対側から深部を覗き込むようにして除圧の操作を進め側縁の骨皮質を残したまま骨孔を作成する Fig. 2 a るこの際に硬膜外静脈叢からの出血に遭遇することがこうすることで椎骨動脈を露出することなく操作可能多いがインテグランなどを用いて圧迫止血することで椎骨動脈損傷を防止できるまた深部に掘り進めで対処可能である後縦靭帯は圧迫因子が除去されている際にも椎間板を正中に置き頭尾側を均等にドリリると判断される際には摘出にはこだわらないし外側病ングすることで disorientation になることを防止できる変の場合には除去する必要のない場合が多い除圧操作最深部の骨皮質が卵殻状に薄くなった段階後療法術翌日から離床を許可しているソフトカでマイクロキュレットや 1 2 mm のケリソンパンチなラーの装着を 1 週間程度行う術後 7 日で抜糸を行いどで皮質骨骨棘脱出椎間板塊などの除去を行う通常術後 10 日ほどで退院となる深部にいたった段階で術者の立ち位置を健側へ移動し脊髄外科 VOL. 2 NO 年 4 月 89
