当 院 では 撮 影 者 が 画 像 を 撮 影 し 検 像 システムに 転 送 します そして 検 像 者 が 画 像 を 確 認 し PACSに 画 像 を 転 送 しています しかし 検 像 者 から 撮 影 者 へ 撮 影 時 の 患 者 状 態 の 問 い 合 わせが 多 く 発 生 しま した そこで 撮 影 画 像 の 最 終 確 認 には 撮 影 時 の 患 者 状 態 を 加 味 する 必 要 があると 考 え 今 回 RISに 検 査 状 況 入 力 画 面 を 追 加 し 検 像 者 へ 撮 影 時 の 患 者 状 態 と 自 己 評 価 も 送 るように 変 更 したので その 内 容 を 報 告 したいと 思 います Radiographers take medical images and transfer them to the image checking system at our institution. The administrator of the system checks them,once accepted, they will be transferred to PACS. Since its installation, there have been many inquiries from the administrator to radiographers, most of which are about the patients condition at medical tests. This situation led us to fix patients status as one criterion in judging image quality to fully utilize the image checking system. Reporting window was added to the existing RIS to inform the checkers of patients condition and image evaluation by the radiographers.
1.まず 病 院 側 にて 各 モダリティで 撮 影 時 の 患 者 状 態 を 示 す 項 目 やチェ ック 項 目 の 洗 い 出 しを 行 いました 2. 次 にRISメーカに 依 頼 し RIS 画 面 上 に 検 査 状 況 入 力 画 面 を 追 加 しま した 3.そして 検 像 メーカには 検 査 状 況 入 力 画 面 から 入 力 された 内 容 を 検 像 システムの 画 面 に 表 示 し 必 要 に 応 じてアラート 表 示 を 行 わせ 検 像 者 に 注 意 を 促 すよう 依 頼 しました At first, all the items regarding patients condition and check items in each modality were reviewed and summarized in the hospital side. We, then, had the RIS manufacture make a new window for patients status in RIS. We also had Image check system manufacture make a new window showing valuable information from the radiographers in the system. So the administrator can be informed of the message from the radiographer or can be alerted to if necessary.
こちらがRISに 追 加 された 入 力 画 面 です ポイントは 全 項 目 の 入 力 必 須 モダリティで 入 力 項 目 変 更 モダリティごとの 自 己 評 価 基 準 が 必 要 ( 個 人 差 をなくすため) 患 者 待 機 有 の 時 は 急 検 像 扱 い 入 力 項 目 を 加 味 して 自 己 評 価 を 入 力 です This is an example display representing examination situation. What we would like to emphasize here are All boxes to be checked. Check items vary from modality to modality. Self-evaluation standard should be well established in each modality. (In order to avoid bias of judgment) In case of awaiting patients, the administrator will check their images immediately. Self-evaluation score must be checked considering all the other points above.
こちらが 検 像 システムの 画 面 です 入 力 項 目 のチェック 内 容 により 検 像 システムの 画 面 に 各 種 アラートを 表 示 させる 患 者 待 機 有 の 入 力 により 急 検 像 ステータスが 追 加 され 業 務 のスリム 化 が 実 現 した This is an example display of the Image checking system. Image checkers can be alerted to some contents appeared on the image checking screen. He is encouraged to check the image immediately, when the patient is awaiting status.
検 像 者 から 撮 影 時 の 患 者 状 態 に 関 する 撮 影 者 への 問 い 合 わせが 減 少 しま した 撮 影 者 は 撮 影 業 務 に 集 中 することが 出 来 るようになりました 共 通 認 識 のもと 自 己 評 価 を 入 力 することによって 検 像 者 の 検 像 作 業 が スムーズとなりました The number of inquiries on patients condition to the radiographers has been reduced. Radiographers have been able to concentrate more on their jobs! Image management has proceeded well, as both the radiographers and administrators understand the concept and aim of this system.
また 当 院 では 使 用 する 予 定 はありませんが 今 回 の 機 能 によって 検 像 システムから 自 動 でPACSに 画 像 を 転 送 する 機 能 がつきました 運 用 例 をこちらに 示 します まず 撮 影 者 がRISを 実 施 します その 時 の 自 己 評 価 によって 二 つに 分 かれます 1つは 自 己 評 価 1~4では 画 像 保 留 状 態 にし 検 像 者 が 実 施 内 容 をチェックし PACSへ 画 像 を 送 信 する ここでは 最 終 画 像 確 認 者 = 検 像 者 になります 2つ 目 の 自 己 評 価 5では 設 定 時 間 後 は 自 動 でPACSへ 画 像 を 送 信 する ここでは 最 終 画 像 確 認 者 = 撮 影 者 になります これは 検 像 作 業 に 人 員 を 割 けない 施 設 で 有 効 ではないかと 考 えています Although it does not run on regularly, we would like to mention that our institution installed an automatic image transfer function to PACS from the image checking system. This slide shows how this function works. The operation branches into two, according to the self-evaluation by the radiographer. One is that of 1-4 All the images are to be suspended waiting for the administrator to accept and transmit to PACS. In short, the last check is done by the administrator. 各 モダリティの 項 目 洗 い 出 し 時 点 では 多 種 多 様 な 要 望 により 多 くのチェ ック 項 目 が 寄 せられました しかし 項 目 数 には 限 度 が 有 るため 目 的 を 明 確 にし 項 目 数 の 絞 り 込 みを 行 う 必 要 がありました またマウスの 移 動 を 少 なくし 入 力 の 手 間 を 簡 略 化 するため チェック 項 目 の 画 面 配 列 についても 工 夫 が 必 要 であると 考 え ます 今 後 は 再 撮 影 回 数 の 把 握 と 患 者 の 被 ばく 管 理 についてなど 検 像 シス テムに 機 能 の 追 加 が 出 来 れば 良 いと 考 えています As expected, so many check items have been turned in from each modality. It should be essential to clarify the objective to slim-down the check-points to fit limited space on the display. We also should see to it that the points be arranged in proper order to increase the efficiency. After this, recording the number of X-rays and the amount of irradiation can be next candidate for installation in the radiation management tool.
撮 影 時 の 患 者 状 態 を 加 味 した 検 像 作 業 については 十 分 満 足 がいく 結 果 とな りました We would like to conclude that medical image management referring to patients s condition at medical test work very well.
