One is Employee s Health Insurance, which is designed for workers in companies, factories, etc. Workers are obliged to enroll in this plan at their working places. The other is National Health Insurance which the residents are required to enroll in at their living area s municipal office. An employer is obliged to notify to the Social Insurance Office. Voluntary enrollment after Retirement When an insured person who had been a participant of Health Insurance Plan over two months continuously has retired from a company, he/she is entitled to continue to be insured as an individual for another two years. If he/she intends to continue to be insured, he/she is obliged to apply for the Health Insurance Office (or the Health Insurance Cooperation in which he/she had participated)within 20days after his/her retirement. The ratio of the medical costs one is responsible for is as follows. insured person and his/her family member (dependant) -30% person of 70 yeas of age and over- 10% ( those whose income exceeds certain amount 20 %) child under 3 yeas of age 20% (The same ratio will be applied both in National Health Insurance Plan and in Employee s Health Insurance Plan.) When the deceased (who had been insured) had no family, the sum which is equivalent to the funeral expense (within prescribed amount) will be given to the person who was in charge of the funeral. When one of the dependants died, 100,000yen will be given to the insured person, as a family allowance of funeral. 7 出 産 手 当 金 ( 健 康 保 険 のみ) Maternity allowance (only health insurance) 被 保 険 者 が 出 産 のため 会 社 を 休 み, 事 業 主 から 報 酬 が 受 け られないとき 欠 勤 1 日 につき 標 準 報 酬 日 額 の6 割 が 支 給 され ます ( 出 産 ( 予 定 ) 日 以 前 42 日 ( 多 胎 妊 娠 の 場 合 は 98 日 )か ら 分 娩 の 日 の 翌 日 以 後 56 日 の 範 囲 内 ) When an insured person was absent from a company due to childbirth and was not given the salary accordingly, 60 percent of the standard pay daily basis will be given to her for each absent day as maternity allowance. 老 人 保 健 法 による 医 療 給 付 Medical fee assistance by 市 区 町 役 場 Sanitation Law for the Aged (4) 医 療 費 の 公 費 負 担 Public financial assistance for medical treatment 老 人 医 療 費 Assistance for medical expense of the aged 昭 和 11 年 9 月 30 日 以 前 に 生 まれた 人 又 は 昭 和 11 年 10 月 1 日 以 降, 昭 和 14 年 9 月 30 日 以 前 に 生 まれた 人 (いずれも 70 歳 以 上 の 人 を 除 く )で, 市 町 長 が 一 人 暮 らしと 認 定 した 人 が, 医 療 機 関 で 医 療 を 受 けた 場 合 の 自 己 負 担 の 一 部 を 公 費 で 負 担 します ただし, 所 得 による 制 限 があります なお, 市 町 により, 対 象 年 齢 の 拡 大 などが 行 われている 場 合 があります Those who were born before September 30th 1936(Showa11) or those who were born during the period between October1st 1936(Showa11) and September 30th 1939(Showa14) [ in either case except those over 70 years of age ] and are acknowledged by mayor as a person living alone will be given medical fee by public expense for a certain part of the medical treatment fee he/she is originally responsible for. In this case, however, the person needs to meet certain condition in terms of his/her income. There is possibility that in certain specific city or region the beneficiary condition concerning age might be less strict. 重 度 心 身 障 害 者 医 療 費 Assistance for medical expense of severely disabled or mentally disabled person 身 体 障 害 者 手 帳 (1,2,3 級 ) 又 は 療 育 手 帳 (,A, )の 交 付 を 受 けている 人 が, 医 療 機 関 で 医 療 を 受 けた 場 合 の 自 己 負 担 相 当 分 を 公 費 で 負 担 しています ただし, 所 得 による 制 限 があ ります 平 成 18 年 8 月 から,1 医 療 機 関 あたり 1 日 100 円 の 自 己 負 担 が 必 要 となります (ただ し, 入 院 については,1 医 療 機 関 月 14 日 までの 負 担, 通 院 については,1 医 療 機 関 月 4 日 まで の 負 担 となります ) なお, 市 町 により, 対 象 者 の 拡 大 などが 行 われている 場 合 があります When a person to whom is issued the Handbook for the Disabled ( 1 st, 2 nd, 3 rd grade) or Handbook for those rearing the disabled children (A,A,B) received medical treatment in a hospital, he/she will be given the public financial assistance for the sum he/she is originally responsible for. In this case, the benefit can be restricted according
to his/her income. After August 2006,a patient needs to pay 100 yen a day for each medical facility. (However, when hospitalized, the patient should continue to pay 100yen a day only up to 14 days a month for each medical facility. As for outpatient, he/she should continue to pay only up to 4 days a month for each medical facility.) There is possibility that in certain specific city or region, beneficiary conditions might be less strict. 乳 幼 児 医 療 費 Assistance for medical expense of infant 対 象 乳 幼 児 ( 入 院 通 院 とも 就 学 前 児 まで)が, 医 療 機 関 で 医 療 を 受 けた 場 合 の 自 己 負 担 の 一 部 を 公 費 で 負 担 します ただし, 所 得 による 制 限 があります 1 医 療 機 関 あたり 1 日 500 円 の 自 己 負 担 が 必 要 となります (ただし, 入 院 については 1 医 療 機 関 月 14 日 までの 負 担, 通 院 につい ては 1 医 療 機 関 月 4 日 までの 負 担 となります ) なお, 市 町 によっては, 助 成 内 容 が 異 なる 場 合 がありますので, 詳 しいことは 居 住 地 の 市 区 町 役 場 の 窓 口 へお 問 い 合 わせください Financial assistance is available for the medical expenses of children up to the time before they start elementary school. When a father or a mother or children ( up to the first March 31st after the day one reaches 18 years of age) of a family with a single parent or an infant without parent received medical treatment, public financial assistance is available for the expense one is originally responsible for. The benefit will be restricted according to income. After August 2006, the patient needs to pay 250yen a day for each medical facility. (When hospitalized, however,the patient should continue to pay 250yen a day up to 14 days a month for each medical facility.. As for outpatient, he/she should continue to pay 250 yen a day up to 4 days a month for each medical facility.)
