Information for foreign residents living in Higashikurume. 5 Medical Care, Health and Welfare For a healthy lifestyle Visiting a doctor in Japan may differ from visiting a doctor in your own country. What should you do in cases of sudden illness, or if you need a doctor on a holiday or at night? You can check what to do here. In addition, the city provides various services such as periodic health examinations and vaccinations. This booklet includes a medical questionnaire in to help you explain your symptoms to a doctor. The map at the end of this booklet shows medical centers and clinics in Higashikurume which support foreign languages. 5 Medical Care, Health and Welfare Medical Care Visiting Hospital What to take Health Insurance Card; Medical Care Certificate (for infants, elementary & junior high school students and children of single-parent households); money in cash: Please note, if you forget your health insurance card, you may have to pay all medical costs in full. Infants can receive medical subsidies (medical care certificate holders) regardless of family income. Elementary & junior high school students and children of single-parent households can receive medical subsidies (medical care certificate holders), subject to family income. (For information on how to apply for subsidies refer to booklet [Childbirth, Parenting and Education] pages 5-6) Visiting a hospital or clinic Reception Desk: Present your health insurance card, medical care certificate or other documents. Complete an examination application form. Medical Examination Cashier Counter: When your name or reception number is called, pay at the cashier counter. The clerk will return your patient's registration card (Shinsatsu-ken), a receipt and a prescription for medicine. The prescription has an expiry date so you should go to a pharmacy as soon as possible. Most hospitals in Japan provide only medical treatment, medicine is obtained from pharmacies. Using a Pharmacy What to take Your prescription for medicine from the hospital; Medication Notebook (Okusuri-techo); Health Insurance Card; Medical Care Certificate; money in cash: Medication Notebook (The pharmacy will provide you with one.) All prescribed medicine is recorded in the Medication Notebook. Emergency Medical Treatment; Medical Treatment on Holidays; Medical Treatment at Night: In cases of emergency, telephone 119 (ambulance). Calling an Ambulance In cases of sudden illness or serious injury, telephone 119 for an ambulance. Points of caution when calling 119. Don't panic, keep calm and carefully dial "119" Prepare to give your name, address and telephone number in Japanese. Write your name, address and telephone number on paper and put it where you can easily see it. Find clear landmarks near your house. Examples: the name of a store; the name of road intersection (hanging under traffic lights); the name of a bus stop: After calling 119, someone should go outside and wait for the ambulance. If you need to call 119 when driving, park your car first in a safe place before calling 119. If the child is not well, arrange the vaccination for another occasion. Influenza Vaccination for Elderly Persons (once a year) (For details see the monthly City Newsletter or the City homepage.) This vaccination is available to people who are; 1. Over 65 years old at the time of the vaccination. 2. Over 60 years old, but under 65 years old and have disabilities due to heart or kidney failure equivalent to First Grade Certificate of Physical Disability (Shintai Shogaisha Techo). (For details contact Health Section in City Hall.) Pneumococcal Vaccination for Adults (once only) (For details contact Health Section.) This vaccination is available to people who; 1.
