1 Patellofemoral Joint Disorders after Total Knee Arthroplasty without Patellar Resurfacing Satoshi KIZUKI et al. Department of Orthopedic Surgery, Gunma University School of Medicine Abstract A total of 60 patients (76 knees) suffering from either osteoarthritis (40 patients, 48 knees) or rheumatoid arthritis (20 patients, 28 knees) underwent total knee arthroplasty (TKA) without patellar resurfacing. The average follow-up period was 4.3 years. The alignments of the patellofemoral joint (P-F) were measured by the skyline view and the Q- angles. Parapatellar pains after operation were more present in RA cases than in OA cases. This suggests that the pain was independent of the P-F alignment in RA cases, and therefore that patellar resurfacing must particularly be done in RA cases. The Q-angle is correlated with the tilting angle and the lateral shift. By measuring the Q-angle during the operation through taking a X-ray (antero-posterior view) on marking the center of the patella and the tibial key words: total knee arthroplasty, patellofemoral joint, Q-angle, follow up study
2 tubercle, the proper P-F alignment can be anticipated. It can be expected that P-F malalignment may bring about progression of the degeneration of the patella. The acquisition of an excellently aligned P-F is an important prerequisite for the proper treatment and care of P-F disorders.
3 31 度 は 減 少 傾 向 を 示 し た た め,可 働 域 と し て は 著 明 な改 善 は得 ら れ なか っ た 2)Q角:検 診 時 のQ角 4.3 (M±SD)で 3)膝 は8 27 で 平 均19.0± あ った 蓋 骨 周 囲 の 疼 痛:検 診 時 に 疼 痛 を 認 め た 症 例 は,76膝 中23膝(全 体 の30.3%),OAは48膝 5膝(OAの10.4%),RAは28膝 64.3%)で,OA-RA間 0.01),RAに 4)X線 中18膝(RAの で 有 意 の 差 を 認 め(P< 高 率 に認 め られ た 評 価:FTAは SD)が,検 術 前 平 均187.1±13.4 (M± 診 時 平 均177.2±6.0 (M±SD)と た Skyline なっ viewで 外 側 傾 斜 角(以下T-Aと と外 方 偏 位 度(以 下L-Sと 略 す)を に 増 加 傾 向 を 認 め た(図3).し 線 上 のP-Fのmalalignmentと 略 す) 術 前 と検 診 時 で 測 定 し比 較 す る と,T-AもL-Sも 検 診 時 に有 意 か し,こ れ らX 膝蓋骨周囲の疼痛 と の 相 関 は み ら れ な か っ た 特 にOAで alignmentが 中 はP-Fの た 膝 蓋 骨 の 軟 骨 下 骨 の 変 化 は,OAとRAと に 外 側 関 節 面 の 骨 硬 化 が 多 く,OAで 膝(OAの60.4%),RAで 60.7%)に 図3外 不 良 で も 疼 痛 を認 め な い 傾 向 に あ っ は28膝 認 め ら れ た RAの2膝 側 傾 斜 角(T-A)と T-A,L-Sは 検 診 時 に 有 意 に増 加 を 示 した., (P< 0.01) 中17膝(RAの には膝蓋骨の 高 度 な圧 潰 を認 め た 全 例 に膝 蓋 骨 の 脱 臼 や 骨 折 は認めなかった 図475歳sir..,RA(術 後4年)右 外 側 傾 斜 角:5 (術 膝蓋 骨軸 写像 前:3 ) 外 方偏 位 度:10.5%(術 痛 を認 め た 術 前-検 診 時 間の 推移 も は48膝 中29 P-Fのalignmentは 外 方 偏 位 度IL-S)の 前:7.7%) 良 好 で あ る が,膝 蓋骨 周 囲 の疼

6 34 図8術 中 のQ角 70歳 図9術 測定 女 性,OA.術 中Q角 は10 で あ っ た 中の 外側 支帯 解離 の適 応 rule of no thumbで Kichard 脱 臼 傾 向 を 示 す 場 合,Q角 D.Scott(1979年) が20 を越 える場 合 に外側支 帯解 離が 適応 となる い Merkowら は,脛 骨componentの 過 内旋位設 潰 が 膝 蓋 骨 の 外 側 脱 臼 傾 向 を 来 す と し て,脛 粗 面 内 側 縁 よ り内 側 にcomponentの 骨 正 面が回 旋 し な い よ う に 設 置 す べ き だ と指 摘 し て い る8,9) ま たScottl3)は,術 て,膝 中 にrule of no thumbと 蓋 骨 のtrackingを た ま ま の 状 態 で,他 行 き,膝 称 し 内側傍 膝 蓋切 開 を開 い 動 的 に膝 関節 を屈 曲 して 蓋 骨 が 脱 臼 傾 向 を 示 す 場 合 に外 側 支 帯 解 離 が 必 要 で,こ れ を 術 後 のP-F障 害 の 予 防 策 と して い る こ の ほ か に も外 側 支 帯 解 離 が 術 後 のP-F障 害 の 予 防 策 と して 有 用 で あ る と い う 報 告 は 多 い2,6,8,11,12,16) 今 回 の 調 査 で, 当 科 で は 外 側 支 帯 解 離 を術 中 に 併 用 し た 症 例 は2例 の み で,今 後 は,よ り積 極 的 に 外 側 支 帯 解 離 の 適 応 と併 用 を考 慮 す べ き と考 え て い る 他 に 諸 家 も 指 摘 し て い る よ う に3,9),大 腿 骨componentの 大 き さ の 決 定 の 際 に,特 に 前 後 径 が 大 き 過 ぎ る こ と に よ るP-Fのhyperpressureを さ け る こ と は 重 要 で あ り,使 用機 種 の サ イ ズ の 選 択 に も 注 意 を は ら うべ きで あ る
