Because anorexia due to dementia follows a loss of decision-making capacity, a surrogate's consent is generally required for PEG tube insertion. Unfortunately, surrogate decisions are often inaccurate, and elderly patients generally reject PEG feeding for a condition of dementia. General deficiencies in informed consent also contribute to the misapplication of PEG tubes. In addition, decisions for PEG may reflect an emotional response to avoid "starvation." Lastly, the personal, nonevidence-based views of many health professionals about tube feeding for cases of dementia may also contribute to PEG misuse.

13 PEG Regarding Issues Percutaneous Endoscopic Gastrostomy: Application of PEG for Elderly People with Dementia NAKAGUCHI Michiko Abstract: This paper aims to identify the issues about percutaneous endoscopic gastrostomy (PEG), through focusing on the activities of academic societies of medical sciences in the U.S.A., Europe, and Japan. This research clarifies the following. First, PEG was developed in the U.S.A. and has spread widely. PEG is applied to many more patients in Japan than in Europe or America today. Second, the reason for this trend is said to be the recently reinforced limitation of the hospitalization period. The reason why PEG has not spread so much in Europe and America is the existence of guidelines from academic societies, such as the ESPEN Guidelines on Enteral Nutrition: Geriatrics of The European Society for Clinical Nutrition and Metabolism. A certain application standard is stated there based on the cognitive level of elderly people with dementia. Third, a study that investigated the application of PEG to elderly people with dementia in present-day Japan showed the result that the European and American standard had negative efficacy. The application of PEG according to cognitive level will probably be introduced in Japan, but the situation in Japan is different from the U.S.A. and Europe, so further consideration on this matter is necessary.
