1 2015; 10(2): 活動報告緩和ケアチームデータベースの改良に向けた取り組み渡邊裕之 1,4), 江藤美和子 2,4) 3,4), 山﨑圭一受付日 2014 年 9 月 10 日 / 改訂日 2015 年 1 月 29 日 / 受理日 2015 年 2 月 3 日 2011 年に, ベルランド総合病院緩和ケアチーム ( 以下,PCT) で構築した PCT 情報共有データベース ( 以下,DB) の有用性を報告した. 今回は,PCT のニーズに応じて DB の改良を行った. おもに以下の 5 点について DB の改良を行った. 1) 各 PCT メンバーが患者介入時の記録を自由記載するフォームを設けた.2) 問題点ごとに提案内容を入力するフォームの改良を行った.3) 患者情報データシート上に STAS J の評価を時系列に表示できるようにした.4)Palliative Prognostic Index の計算フォームを追加した.5) 日本緩和医療学会に提出する緩和ケアチーム登録に必要なデータを簡便に抽出できるようにした. この DB は市販の DB 作成ソフトで構築されており, どの施設でも利用可能であることが有用な点であると思われる. Since then, we updated DB depending on the requests of PCT. We revised DB mentioned below. We developed a form for keeping a record of PCT members care for patients, their family members or others, and a record of recommendation for each problem list. We also updated another form so that the evaluation of Support Team Assessment Schedule of Japan(STAS J)had been showed sequentially, added an entry form of Palliative Prognostic Index, and extracted data required by Japan Society for Palliative Medicine in order to evaluate the activity of PCT. The database could be used by many hospitals, because it was created by the commercially available software.
