It is necessary to construct an efficient medical system path for all the staff involved in the treatment of acute stroke treatment. While Drip & Ship has a high home return rate, a high rate of symptomatic intracranial hemorrhage was observed and its superiority over Mothership has not been established. Tokushima University Hospital introduced a stroke diagnosis and treatment system using the smartphone in 2012, which has contributed greatly to the medical treatment of stroke in the areas served by the hospital. All the members of the stroke team are able to instantaneously share patient information, making it possible to transmit information and diagnosis any relevant information quickly and accurately. はじめに超急性期脳梗塞に対する治療はここ数年で目覚ましい発展を遂げ新たな時代に突入した発症早期の前方循環系の主幹動脈閉塞例に対して内科治療 (intravenous recombinant tissue plasminogen activator;rt-pa 静注療法含む ) と血管内治療を比較したランダム化比較試験 (randomized controlled trial: RCT) では 2015 年に主にステントリトリーバーを用いた5つのエビデンスが報告され血管内治療の有効性が証明された ( 表 1) 1~6) これらを受け経皮経管的脳血栓回収用機器適正使用指針第 2 版 7) や 2015 American Heart Association/ American Stroke Association focused update of the 2013 Guidelines for the early management of patients with acute ischemic stroke regarding endovascular treatment(aha/asaガイドライン2015 改訂版 ) 8) などの改訂ガイドラインが相次いで公表された脳卒中の急性期治療を担う施設は 24 時間体制で脳卒中専門医を含む専門スタッフが高度診断機器を駆使し迅速な診断を行い血管内治療を含む最良の治療を提供することが求められる本稿では脳梗塞超急性期治療で求められる対応体制や我々の取り組みについて概説したい 66(156) 日獨医報第 62 巻第 2 号 66-70(2017)

( 文献 6) より転載 ) 受け入れ体制と診断治療本邦では 2005 年にrt-PA 静注療法が認可されて以降脳梗塞超急性期診療体制は大きな変化を遂げたしかしながら rt-pa 静注療法は急性期脳梗塞例の5% 発症 3 時間以内 ( 現在は4.5 時間以内まで投与可能 ) の超急性期脳梗塞の16% に実施されているに過ぎない 9) rt-pa 静注療法の対象症例を確実に治療するために必要な受け入れ体制や診断治療について述べる 1. 発症より来院までの対応救急隊員が脳卒中を疑う患者を適切に専門施設に搬送するために病院前脳卒中スケールが用いられている代表的なものとしてはシンシナティ病院前脳卒中スケール ( Cincinnati Prehospital Stroke Scale: CPSS) ロサンゼルス病院前脳卒中スケール (Los Angeles Prehospital Stroke Scale: LAPSS) 倉敷病院前脳卒中スケール ( Kurashiki Prehospital Stroke Scale: KPSS) があり近年では脳主幹動脈閉塞のスクリーニングに特化した the rapid arterial occlusion evaluation(race) スケールが提唱された 10) 救急隊員から脳卒中が疑われる患者の情報が入ると脳卒中診療チームは速やかに受け入れ体制を整える必要があるまた発症から来院までの時間を短縮するためには救急隊員への指導のみならず市民への啓蒙も重要である市民が脳卒中と気づき直ちに救急車を呼ぶといった適切な行動をとるように啓発活動を行うべきであり本邦でもメディアや学校教育により脳卒中の知識と適切な対処法の定着を目指した市民啓発の試みが報告されている 11 12) 2. 来院から画像検査まで急性期脳主幹動脈閉塞の臨床転機は一般に発症から再灌流までの時間が長くなるほど不良になるためいかに早くrt-PA 静注療法さらには血管内治療を行えるかが重要となる患者が来院後直ちに病歴聴取神経診察臨床検査画像検査 rt-pa 静注療法チェックリストの確認と判定 rt-pa 静注療法および血管内治療の説明と同意の取得などを複数のスタッフで同時に行わなければならない医師や看護師をはじめ急性期脳卒中診療に携わるスタッフ全員の効率的な診療体制の構築が必要である実臨床では血液検査と静脈ルートの確保を速やかに行いいかに早く画像検査を行うかが予後に寄与しうる 3. MothershipとDrip & Ship 血管内治療が施行できる脳卒中診療施設 (Comprehensive stroke center: CSC) への直接搬送 (Mothership) は血管内治療が施行できない (rt-pa 静注療法は可能 ) 脳卒中診療施設 (Primary stroke center: PSC) を経由しrt-PA を開始した上での転院搬送 (Drip & Ship) に比して発症から動脈穿刺までの時間が短時間 ( 中央値分 vs 67(157)

3 275 分 ) であり転機 (90 日後 modified Rankin Scale 2) も良好な可能性 (66% vs 49%) が示されている 4) 一方で Drip & Shipは米国のrt-PA 静注療法の17~23.5% を占めており在宅復帰率が高いとされている 13) しかしながら症候性頭蓋内出血が多く (5.7%) 院内死亡率が高い (10.9%) ことも指摘されており 14) 搬送中の管理や出血時の責任所在が問題となる最近の報告では PSCにおいて搬入から30 分以内にrt-PA 静注療法が施行できる条件下で PSCからCSCまでの搬送が45 分以上かかる場合や発症から覚知まで時間を要する場合においてはDrip & ShipがMothershipに比し有効である可能性が示唆された 15) 現時点ではMothershipとDrip & Ship の優劣は未だ確立しているとは言えず現在スペインにおいてMothershipとDrip & ShipのRCTである RACECAT 試験 ( Clinical Trials. gov: NCT ) が進行中である 4. 画像検査からrt-PA 投与まで AHA/ASAガイドラインでは閉塞血管の同定が行われていない時点での投与が容認されているため最初の画像検査が終了した時点で血液検査血小板数血糖値 PT-INR(prothrombin time-international normalized ratio) aptt(activated partial thromboplastin time) が律速段階となる現在はさまざまな迅速キットが販売されているため施設内でこれらの迅速キットを常備すべきであるまた rt-pa 静注療法の適応を迅速かつ確実に行うためには脳卒中医または神経放射線科医が画像検査時に立ち会うことが重要であり rt-pa 投与までの時間短縮につながると考えられる実際には一人の医師がrt-PA 静注療法の説明と同意の取得を行いもう一人の医師が画像検査に立ち会い rt-pa 投与の準備を看護スタッフに指示するという流れが最もスムーズであると思われる 5. rt-pa 投与から血管内治療までこの段階では血管内治療医も治療に参加していなければならない理想的には脳卒中医がrt-PA 静注療法の説明に引き続いて血栓回収療法の説明を行うことが望ましく血管内治療医はrt-PAの投与を行いながら血管内治療室への入室を采配し血栓回収療法の準備を速やかに進めるべきである AHA/ASAガイドライン2015 改訂版では血管内治療開始前に rt-pa 静注療法の治療効果判定のために経過を観察する必要はなく勧められない ( ClassⅢ) とされており 8) rt-pa 静注療法の再開通効果が乏しいと考えられる内頸動脈や中大脳動脈近位部の閉塞症例では効果判定を待たずに迅速に血管内治療による血栓回収療法を開始すべきである 6. 血栓回収療法の最前線前述の5つのRCTが報告された後 2016 年には HERMES collaboratorsによるこれらのメタ解析の結果が示され 16) rt-pa 静注療法を含む内科治療に血栓回収療法を追加することにより90 日後の日常生活自立度は有意に改善し 80 歳以上の高齢者や rt-pa 静注療法の禁忌例においても有効であることが示されたまたその有効性は発症から治療開始までの時間が早いほど高く発症 7.3 時間までは内科治療を上回ることが明らかとなった 17) また治療適応の拡大として DAWN trial 18) は発症時間不明例を含む最終健常確認から6~24 時間の内頸動脈または中大脳動脈のM1 閉塞症例において神経徴候と虚血コア体積のミスマッチが大きい症例では血栓回収療法を行った群で有意に転帰が良いことを示し治療可能時間の拡大をもたらしたさらに Rebelloらは虚血コアが広範囲な症例でも皮質領域にミスマッチが存在する場合には再開通療法が有効な可能性を示した 19) 遠隔診療システムについて医療過疎地における専門医不足にも貢献できる技術としてインターネット通信衛星通信などの通信技術を活用した遠隔医療 (telemedicine) があるが脳卒中急性期診療では迅速な情報のやりとりが必要とされ脳卒中に特化した遠隔医療 (telestroke) が重要となる徳島大学病院は2012 年に国公立大学で初めてスマートフォンを使った脳卒中診療システム 20) を取り入れ現在まで 1,700 件を超える送信実績があり近隣地域の脳卒中診療に大きく貢献している ( 図 1) 脳卒中の治療で最も大切なのは来院から治療開始までの時間短縮でありスマートフォンを用いることにより脳卒中チームが所在を問わず患者の情報を瞬時に共有することができるようになったこれまで上級医に電話で状況を説明していたため情報を伝えることに時 68(158)

経験豊富な専門医がその場に居なくても指示することが可能となっているスマートフォンを用いた診療を始めたことにより患者の治療開始までにかかる時間が大幅に短縮されている遠隔医療の普及推進に向けた国の政策には現在主に次の4つ 1

4 図 1 スマートフォンを使った脳卒中診療システム図 2 迅速で正確な情報伝達と診断間を費やしたり誤認識につながる恐れがあったがスマートフォンを使った脳卒中診療を導入することにより医用画像例えばMRI CT 血管撮影などの画像をスマートフォンで電子カルテと同じ解像度で鮮明に見ることができるようになり迅速で正確な情報伝達と診断ができるようになった ( 図 2) またリアルタイムの手術動画がスマートフォンで確認できるため経験豊富な専門医がその場に居なくても指示することが可能となっているスマートフォンを用いた診療を始めたことにより患者の治療開始までにかかる時間が大幅に短縮されている遠隔医療の普及推進に向けた国の政策には現在主に次の4つ 1 遠隔診療の要件の明確化 ( 遠隔診療と医師法との関係の明確化 ) 2インセンティブの付与 ( 補助事業保険適用 ) 3 医療従事者のリテラシー向上 ( 遠隔医療に関する知識技能修得 ) 4 遠隔診療の有用性の評価 ( 厚生労働省科学研究の実施 ) が挙げられて 69(159)
