The purpose of this stud was to compare normal arch group and low arch group, to determine differences of the plantar pressure of longitudinal arch, and to track the center of foot pressure between those two groups during normal gait. The subjects were 1 health adults with 3 feet, and we classified 9 normal left arches of the foot, 9 low left arches of the foot, 9 normal right arches of the foot, and 9 low right arches of the foot. The left low arch group showed a significantl greater value of plantar pressure in 1 st metatarsal bone, 3 rd metatarsal bone, 5 th metatarsal bone, arch section, heels, and heel in comparison with left normal arch group. Right low arch group showed a significantl greater value of plantar pressure in the arch section in comparison with right normal arch group. The center of foot pressure was significantl higher than normal arch groups, and this was considered to show a relationship with pain of the flat foot. Tracking the center of foot pressure for low arch groups showed irregular waveforms l in comparison with normal arch group. We have speculated that there is a spring function at the arch of the foot to absorb shock. The low arch foot groups could not perform the spring function under load during gait, so the centers of foot pressure were high. The most likel reason wh the low arch group showed irregular and more interior waveform from the inside at the locus of the foot pressure was that absorption of the pressure was more inside due to the low longitudinal arch. Irregular waveform of the tracking center of foot pressure was considered to show unstable stance phase during gait. 2 17 The 1 st toe The ~5 th toe T~5 T~5 The 1 st metatarsal The nd metatarsal The 3 rd metatarsal The th metatarsal The 5 th metatarsal of foot Lateral heel Medial heel LM MH Fig. 1 Foot pressures b RS scan 1. 緒言扁平足を呈する人はスポーツや走行時などに何らかのストレスがかかり, 足底部に疼痛が出現することがある 1)-). 我々は, 著者の1 人が勤務している病院にて扁平足 ( 足の内側縦アーチが低下 ) の疼痛および胼胝の出現状態を調査した結果, 扁平足患者の疼痛および胼胝の出現頻度は, 足底全体の内側部に多く, 第 1 中足骨頭や第中足骨頭, 第 3 中足骨頭, 踵骨などの地面と接する面に多数みられた 7). また過去の先行研究において正常足と扁平足 ( 低アーチ足 ) を比較して足底部位を細かく検討したものが見当たらない. そこで今回は正常足と低アーチ足との違いを明確にすることで低アーチ足による足の影響を検討するために歩行時の各足底部位にかかる圧力と踵からつま先に抜ける圧力中心の位置 ( 足圧中心軌跡 ) の比較を行った.. 研究方法アーチの評価は, 床面から舟状骨下端までの距離を足長で序した値に1を掛けた内側縦アーチ高率を用いた. 正常アーチ足は内側縦アーチ高率が男性 1.5% 以上, 女性 1.7% 以上とした 1). 低アーチ足は内側縦アーチの低下で内側縦アーチ高率が男性 1.% 以下, 女性 1.% 以下とした ). 計測肢位は, 立位で左右上前腸骨稜を結んだ線と踵から第趾の線が直角となる方向を進行方向として足踏みを1 回行い, 静止した時の足幅を設定する. 設定した足幅を保ちながら進行方向に向けた足を身長の 5% 一歩前に出し, 出していない後方の足を足関節背屈角度は5 に設定して計測する ). 歩行時の踵接地からつま先離れまでの歩行立脚相の確認はハイスピードデジタルカメラ (EX- ZR1,CasioCo.) を1 台用いてシャッタースピード1/ 秒, 毎秒コマで撮影し, 各被験者にて右足 1 回, 左足 1 回の回撮影し, 画像解析ソフトウエア (DARTFISH,Dartfish Co.,Ltd.) を用いて解析を行った. 歩行路は幅が1.5m, 距離が助走路各 3m, 撮影範囲の走路は.mの計.mとし, 足部との撮影距離は.mにて行った. 足底圧力と足圧中心軌跡はモンテシステムソリューションの RSscan(3Hertz) を用いて歩行時の最大足底圧力 1zone( 母趾, ~ 5 趾, 第 1 中足骨部, 第中足骨部, 第 3 中足骨部, 第中足骨部, 第 5 中足骨部, アーチ中央部, 踵部外側, 踵部内側 ) と足圧中心軌跡の座標を測定し, 正常アーチ足群と低アーチ足群を比較した (Fig. 1). なお被験者には実験時, メトロノームを用い

5 スポーツ産業学研究,Vol.3,No.(13),177 ~ Spring function Spring functional decline Pressure increases Elevation of the arch lowering of the arch Fig. Mechanism of pressure increase b Spring functional decline 足が9 足, 年齢 5. 1±. 9 歳, 身長 15. 9±. 7cm, 体重 57.±5.3kg, 右正常アーチ足が9 足, 年齢.3±5. 歳, 身長 1. ±. cm, 体重 53. ± 7. kg, 右低アーチ足が9 足, 年齢 5. ±3. 1 歳, 身長 1.1±.cm, 体重 59. ±3. kgであった. また全例がボールを蹴る足は右であり, 骨折など特記すべき既往歴が無かった. なお本研究は名古屋大学の倫理委員会の承認を得ており, 対象者には本研究の目的と方法, 健常者としての権利および個人情報の保護について書面と口頭にて十分な説明を行い, 同意を得た.. 結果.1 足底圧力の比較 (Comparison of plantar pressure) 左は低アーチ群が第 1 中足骨部, 第 3 中足骨部, 第 5 中足骨部, アーチ中央部, 踵部外側, 踵部内側にて有意に大きい値を示し, 右は低アーチ群がアーチ中央部にて有意に大きい値を示した (Fig. ).. 足圧中心軌跡 (Comparison of center pressure trajector) 左右にて低アーチ群は, 正常アーチ群と比べて内側寄りで不規則な軌跡を示した (Fig. 3). ハイスピードカメラ画像からの立脚中期時の X 座標を比較したところ, 左がp=.153, 右がp=.179となり, 左足は低アーチ群が正常アーチ群と比べて有意に内側寄りの軌跡であった (p<.5). 5. 考察足底圧力は低アーチ群が有意に高く, 圧力が集中したことが扁平足の疼痛および胼胝の発生に関与していると考えられた 7). 通常, アーチが低下する扁平足は床との接触面積が増加するため, 地面と接する箇所の圧力は低下すると考えられるが, 今回の結果では低アーチ群の方が, 明らかに圧力が増加していた. これはアーチの衝撃を吸収する機能 ( スプリング機能 ) が低下したことにより圧力が増加した可能性があると思われた (Fig. ). また足圧中心の軌跡が正常足では, ほぼ同様の波形を示したが低アーチ足は波形が一定ではなく不規則な波形であったことから低アーチ足群は歩行時に踵接地からつま先離れまでの立脚相において安定しない歩行を行っているとも考えられた. さらに足圧中心の軌跡が内側に寄っていることは, 歩行時の重心が足部の内側に寄っているのではないかと推察した. アーチ低下による足底の圧力増加や足圧中心の内側寄りで不規則な軌跡から足部への負担増加による様々な疾患が出現する可能性が示唆された. 実際アーチ低下が原因の1つといわれている足部疾患は多数あるが, そのメカニズムが明確にされているわけではない. 本研究結果は, そのメカニズムを解明する一助となるとともに足部疾患の治療方法を考える上でも意義のある内容であると考える.. 参考文献 1) 井口傑, 足のクリニック, 南江堂,pp.119-,
