2 Message Organizing Committee Dear Colleagues: We often hear that PCI has reached maturity. If this is so, which area is mature among the long-term outcome, techniques, and devices? To our surprise, restenosis can occur unexpectedly after balloon dilatation in some cases. In others, dissection resolves and the severity of stenosis improves on late angiograms compared with that immediately after intervention. It was once difficult to predict the long-term outcome of PCI, and the need for repeat revascularization was considered a weak point of this technique. However, owing to the development of drug-eluting stents, the outcome of PCI can now be predicted. On the other hand, the way of thinking about repeat revascularization has also changed slightly. In the EXCEL study, repeat revascularization was not included among the primary endpoints. The efficacy of this intervention tends to be assessed from the results of long-term evaluation of events, and many people have called this advance maturity. Then what are our objectives in this period of maturity? In the Courage and BARI2d studies, the timing of revascularization was investigated in the setting of dramatic advances in drug therapy. In the FAME study, the clinical significance of deferring PCI was shown by the evidence obtained. It may be said that this could also be established owing to advances in complementary drug therapy. Briefly, there is not only a demand for optimal medical treatment but also for optimal PCI in the hope of obtaining a better long-term outcome. This means that not quantitative, but qualitative improvement, is required for PCI now that it is mature. On the other hand, it appears that there have been no appreciable changes over the past 10 years in the cases of trouble shooting reported at clinical conferences. In other words, slow flow in the presence of abundant thrombi, coronary artery rupture at calcified lesions, removal of stents, and bifurcation lesions all remain common problems. This indicates that passing our knowledge and techniques on to the next generation is very important to prevent the loss of information that is common knowledge at present. TOPIC has adopted an on-the-spot approach over the past 10 years, relaying all the treatments and techniques for complicated lesions including CTO and studying them together with many researchers. As a result of our planning and management in consideration of the needs at each time, the details of peripheral vascular treatment, co-medical sessions, and educational courses have been improved and the syllabus prepared every year has achieved widespread popularity. Next time, outstanding doctors will also be invited from overseas, and an international satellite relay is also planned. There will be a good chance to consider the present status of Japan in the world, so that you will not just be the big frog in a small pond. Updated topics and controversies will also be covered so that TOPIC 2013 will be exciting enough to blow away the summer heat of Tokyo. The TOPIC team will all strive together to ensure the success of the next conference. We are looking forward to your attendance.
