感 染症 学 雑 誌 148 Table 4 Laboratory Table 5 Findings Histological at Biopsy Fig.2 Typhoid nodule,case 第60巻 第2号 4,HE ~100 Findings Fig.3 Focal tion,case necrosis and Kupffer cell mobiliza- 4,HE ~100 に は 肝 機 能 検 査 成 績 は お お む ね 回 復 し て い た. 今 回 の5例 示 し た が,共 の 肝 生 検 の 組 織 所 見 はTable5に 通 して み られ た 主 な所 見 と して は 肝 細 胞 のfocalnecrosis,門 脈 域 の 線 維 化 を 伴 う軽 度 の 拡 お よ びKupffer細 胞 の 動 員 な ど で あ っ た. ま た 肝 細 胞 索 の 二 層 性,肝 細 胞 の 二 核 性 な どの 再 い て は 血 清 ビ リル ビ ン2mg/dl以 GPTの U以 上 は16%,GOT, 最 高 値 は そ れ ぞ れ77,170U(正 下)で あ っ た と し て い る.ま たSoubeyrand 生 検 像 が す べ て の 症 例 に 認 め ら れ た.typhoid ら5)は1980年 noduleは1例 て 黄 疸 は3.2%,GOT,GPTの 異 常 は10.7 16.1% に 認 め た と 報 告 し て いる.わ れ わ れ の 今 回 の1979 う っ滞,中 た が,肝 に 認 め ら れ た(Fig.2).そ の他胆汁 心 静 脈 周 囲 の壊 死 を 認 め た症 例 が あ っ 細 胞 の 脂 肪 変 性 な ど は み ら れ な か っ た. に 腸 チ フ ス,パ 常 値15,20 年 か ら1983年 ま で の5年 ラ チ フ ス279例 につ い 間 で 血 清 ビ リル ビ ン2mg/ こ れ ら は い ず れ も軽 度 で 急 性 肝 炎 の 回 復 期 あ る い dl以 上 の 症 例 は2例(7.6%)で は 回 復 後 を 思 わ せ る 所 見 で も あ り,診 例 の 頻 度 は 上 記 二 者 の 報 告 よ り も 明 ら か に 高 く, non specific reactive hepatitisの 断 的 に は 所 見 で あ った (Fig.3). ラ チ フ スA合 GPTに 察 腸 チ フ ス の経 過 中 に み られ る肝 障 害 の頻 度 につ い て はRamachandranら4)は1974年 ラ チ フ ス92例 7.6%,GOT,GPTの た と 述 べ て い る.ま の報 告 で 腸 に つ いて黄 疸 の出 現 は 異 常 は24 35%に 認 め られ た これ ら の 異 常 値 の 程 度 に つ あ り,GOTが 比 し て 高 値 の 症 例 が 多 か っ た が,推 的 に は 有 意 差 は 認 め ら れ な か っ た.5001U/1以 を 示 し た 症 例 は2例 フ スAで み ら れ た.腸 はGOT,GPT,LDHと が 高 値 を 示 し た が,こ GOTの み5%の 異常 わ せ て26例(72.2%)で あ っ た.GPTは23例(63.4%)で 4.考 チ フ ス,パ 腸 チ フ ス,パ あ っ た.GOTの 計 学 上 チ フ ス とパ ラ チ もに 腸 チ フス 例 れ らの うち 推 計 学 的 に は 危 険 率 で 有 意 差 が 認 め ら れ た.

Hepatic Injury due to Typhoid and Paratyphoid Fever Akira MACHIP),Yoshiro NITTA2),Hiroko SAGARA'),Takehisa SEO') & Yoshio MATSUBARA') Department of Internal Medicine,Tokyo Metropolitan,Ebara General Hospital Department of Internal Medicine,Tokyo Motropolitan,Toshima General Hospital Department of Infectious Disease,Tokyo Metropolitan,Toshima General Hospital It's known that typhoid fever and paratyphoid fever often cause changes indicative of hepatic involvement and even jaundice. This study was conducted on 36 patients (27 cases with typhoid fever (TY) and 9 cases with paratyphoid fever caused by Salmonella paratyphi A (PA) who were admitted to Tokyo Metropolitan Toshima General Hospital from 1979 through 1983 and were all bacteriologically confirmed for the purpose of investigating the liver injury in these diseases. Liver function tests were analysed in all cases and liver biopsies were parformed in 3 cases with TY and 2 cases with PA.The serum bilirubin level was greater than 2 mg/dl in only 2 cases but one patient with jaundice had value over 10 mg/dl.abnormal elevation of the GOT level occurrd in 19 cases (70.4%) with TY and 7 cases (77.8%) with PA.The averages of elevated GOT values were 242.9 IU/1 in TY and 119.3 IU/1 in PA.The difference of GOT level between TY and PA was significant (p<0.05).the GPT level was elevated abnormally in 16 cases (59.3%) with TY and in 7 cases (77.8%) with PA.The averages of elevated GPT values were 212.6 IU/1 in TY and 111.3 IU/1 in PA.The maximum values of GOT and GPT were 780 and 660 IU/1 in a case of TY.These elevations of GOT and GPT level began around the 14th day of illness and improved to normal level around the 30th day of illness.the LDH level was abnormally high in 92.6% of TY and in 100% of PA.The ALP level was elevated in 63.9% of 36 cases with TY or PA.The y-gtp level was elevated in 41.1% of these 36 cases. The following changes were flequently observed in the liver:the focal necrosis of liver cells,the mobilization of Kupffer cells and the slight enlargement of portal areas.the regeneration of liver cells including two-cell-thick plate and double nuclears of liver cells were shown in all specimens.the typhoid nodule was found in only one case with TY.There were no histological differences between TY and PA. Above-mentioned histological findings were those of so called "non specific reactive hepatits".
