CT scan obtained on first medical examination, showing a mass shadow with spicule and pleural indentation in the left lower lobe. 5a),こ の 異型 細 胞 は 肺 腺癌 手 術 時 の 中 分 化 型 腺 S8に 辺 縁 不 整 で 胸 膜 嵌 入 像 を 伴 う腫 瘤 影 を 認 め た が(Fig.2),平 成5年7月 の 手 術 後,肺 野異常 陰 影 の 出 現 や 所 属 リ ン パ 節 腫 脹 は 認 め な か っ た. 側 頭 骨CTで は 冠状 断像 で 外 耳道 後 壁 を破 壊 し中 耳 腔 へ 進 展 す る 腫 瘤 を 認 め(Fig.3a),水 平 断像 で は 左 乳 突 洞 内 の 気 胞 が 消 失 し一 部softdensity 癌 の 組 織 像 と 類 似 し て お り(Fig.5b),中 耳の病 rnassに 変 は 肺 癌 の 遠 隔 転 移 と 考 え ら れ た.背 部 痛につ 置 き 換 わ っ て お り側 頭 骨 錐 体 部 の 一 部 に 骨 溶 解 像 を伴 っ て い た(Fig-3b).骨 ム で は 左 顎 関 節 付 近 と 第8胸 シンチ グラ 椎 とに 異 常 集 積 を い て は 胸 椎X線 で 第8胸 osteoporosisま た は 骨 転 移 に よ る も の と考 え ら 認 め た(Fig.4).腹 部超 音波検査 では異常所 見は れ,鎮 認 め ず,頭 部CTで は 脳 内へ の転 移 は 認 め な か っ 検 後,再 た. 臨 床 経 過:左 聴 に 対 して 耳 洗 浄 を続 け る も 症 状 改 善 し な い た め,中 術 を 施 行 し た.生 痛 剤 投 与 に よ り 改 善 し た.中 耳内腫瘤の生検 検 組 織 標 本 で,上 皮下組織 内 の リ ンパ 管 と骨 内 に 異 形 細 胞 の 増 生 が 見 られ(Fig. 現 を 認 め,肺 て 両 肺 に び まん 性 の 粟粒 影 の 出 内 転 移 に よ る も の と考 え た た め 手 術 は 不 能 と判 断 し た.対 症 療 法 と して 耳 洗 浄 を 続 け た が 耳 漏 は 軽 減 せ ず,難 た.以 耳 内腫 瘤 生 度 他 臓 器 の転 移 巣 の有 無 を検 索 した と こ ろ,胸 部CTに 耳 漏,難 椎 に 圧 迫 骨 折 を 認 め. 聴 も改 善 しな か っ 後 全 身 状 態 悪 化 し 転 院 と な っ た.

Metastatic Tumor Presenting as Otitis Media in Primary Lung Cancer Atsushi Kobayashi, Koichi Maeda, Keiichi Mikasa Takahiro Yoneda, Masayoshi Sawaki and Nobuhiro Narita Second Department of Internal Medicine, Nara Medical University A 67-year-old woman received a left lower lobectomy and dissection of regional lymphatics due to an adenocarcinoma of the lung (T2N1M0, Stage II) in July 1993. Nineteen months after surgery, she complained of left rhinorrhea and hearing loss. Temporal bone computed tomography revealed a tumor in the middle ear and osteolytic changes in the temporal bone pyramid. The histopathological findings of the tumor was adenocarcinoma, and it was diagnosed to be distant metastasis from lung cancer. Symptoms similar to otitis media were the main complaints in the case of distant metastasis from lung cancer.
