Arts and Sciences Double Inversion Recovery Serial Double Inversion Recovery Brain Imaging: Diagnostic Value in the Detection of demyelinating lesion 1 1 1 1 2 1 2 Key words: 3D DIR, 3D FLAIR, demyelinating lesion, multiple sclerosis Abstract DIR double inversion recovery is a MRI pulse sequence, which includes two inversion recovery pulses suppressing signal from CSF and normal white matter resulting in improved delineation of white matter lesions. We conducted a comparative study between DIR and 3D FLAIR fluid attenuated inversion recovery in their ability to diagnose demyelinating disease. Our results demonstrate that DIR allows significantly improved visualization of lesions at the corticomedullary junction compared to FLAIR. We conclude that DIR is more sensitive than 3D FLAIR in the detection of demyelinating disease. 資 料 学 脱髄性疾患におけるDouble Inversion Recovery法の有用性の検討 FLAIR 共に同等に描出できた Fig.5 Case8 41 歳 術 Arts and Sciences 男性 多発性硬化症の症例 脳幹部辺縁の病変は 脳 3 考 察 脊髄液抑制不良のため評価が困難であった Fig.6 多発性硬化症は 最も頻度の高い中枢神経系の炎症 性脱髄性疾患で 白質を中心に多数の脱髄斑が時間的 空間的に多発し 再発寛解を長期にわたって繰り返す 疾患である 6, 7 脱髄斑は 脳室周囲白質 視神経 脳幹 脊髄に好発し 細動脈の密度の高い所や髄液に 接する所に多い傾向がある 8 多発性硬化症の診断に おいて MRI は他の諸検査よりもはるかに病変検出能 が高く 潜在性病巣も含めて検出することができ 予 後予測や治療効果のモニターとしての役割を担ってい る 9, 10 従来 多発性硬化症は白質疾患と考えられて いたが 近年の病理組織学的研究により 脱髄斑は必 ずしも白質だけに限局するわけではなく 皮質 皮質 Fig.4 Fig.4 Case6. 13-year-old boy, multiple sclerosis. FLAIR image. 下も病変の好発部位であるとの報告もあり 皮質病変 の重要性が議論されるようになった 11, 12, 13 この点を ふまえて 皮質や皮髄境界の病変に着目し検討を行っ た われわれの検討では 白質病変の検出に対する視覚 評価では 3D DIR の方が高い評価を得たが 3D DIR と 3D FLAIR の間に有意差は認められなかった 脳 幹病変の評価においても 3D DIR と 3D FLAIR に有 意差は認められなかった しかしながら 3D FLAIR の方が 3D DIR よりもわずかに検出能が高いという結 果だった 3D DIR で低い評価だった症例に 脳脊髄 液の抑制不良により病変が不明瞭な例を経験した 原 因として 体動によるモーションアーチファクトが考 えられる 3D DIR は IR パルスを 2 回印加する撮像法 Fig.5 Fig.5 Case3. 34-year-old female, multiple sclerosis. FLAIR image. であり 十分な信号減衰と信号強度維持のために 3D FLAIR よりも長い撮像時間を必要とする そのため撮 像時間の延長が原因と考えられる体動やそれに伴う脳 脊髄液の抑制不良が発生し 視覚評価に反映したと考 える 皮髄境界の視覚評価においては 3D DIR が 3D FLAIR よりも有意に優れていた 3D DIR の特徴であ る正常白質信号抑制により 白質と灰白質のコントラ スト差が大きく 白質と灰白質の境界部 皮髄境界が 3D FLAIR よりも明瞭に描出できたことで 皮髄境界 に位置する病変信号について視認性が向上したと考え る 複数回 MRI 検査を施行した症例の中に 病変が新し Fig.6 Fig.6 Case8. 41-year-old male, multiple sclerosis.
