レボカルニチン塩化物 (Levocarnitine chloride) の効果 (120612) MR が置いて行ったパンフレットの中にレボカルニチン塩化物 ( エルカルチン ) があった 効果 についてよく分からなかったので 一度調べてみることにした 大塚のニュースリリースには以下のような記載がある おそらく 肝硬変や透析の患者に対して は使う可能性があるかもしれない カルニチン欠乏症は 先天性代謝異常症によるものや 肝硬変など後天的医学的条件によるもの さらには長期間の透析や薬剤の投与などの医療行為によるものがあります 発症原因により異なる多くの因子が関与しており そのためカルニチン欠乏による臨床症状も多岐にわたります PECO P:Adult patients with end-stage renal disease receiving long-term hemodialysis. E:Supplemental L-carnitine for at least 8 weeks. C:placebo for at least 8 weeks. O:intradialytic cramping or hypotension 長期透析療法中の末期腎不全の成人患者に対して L-carnitine を 8 週間投与すると プラセボ と比較して 透析療法中の筋痙攣や低血圧が改善するかどうかを検討した研究であることが分か る 妥当か 抄録に All published English-language reports of randomized placebo-controlled trials of L-carnitine supplementation とあり RCT のメタ解析であることが分かる 結果 L-carnitine 群で筋クランプや低血圧の頻度が改善している傾向であった 筋クランプについて
は 95% 信頼区間をみると 少なくとも悪い結果の可能性はかなり低い Using data from all 6 relevant trials, the pooled odds ratio for cramping after L-carnitine supplementation was 0.30 (95% confidence interval, 0.09 to 1.00; P = 0.05). Analysis of the 5 studies examining the response of intradialytic hypotension to l-carnitine supplementation yielded a pooled odds ratio of 0.28 (95% confidence interval, 0.04 to 2.23; P = 0.2). E:LAC (n=13) C:placebo (n=11) O:hepatic coma (The aim of this work was to evaluate the effects of LAC in the treatment of cirrhotic patients with hepatic coma, with the primary end point being improvement of neurological function.) 肝硬変の患者に対して アセチル -L- カルニチンを投与すると プラセボと比較して 肝性昏睡が 改善するかどうかを検討している 妥当か 本文中に randomization の記載があり 本文の Statistical analysis には Data were analyzed according to the intention to treat principle. とある 結果 Glasgow Scale はアセチル -L- カルニチン群で改善 アンモニアや BUN などのパラメーターは有 意では無いものの 低下傾向であった Statistically significant differences in neurological findings, as evaluated by the Glasgow Scale, as well as in ammonia serum levels and BUN were found following LAC treatment. The improvement in the neurological picture was evident at between 1 and 4 hr after the end of treatment, remaining until 24 hr after. No side effects were observed in our study series.
