Together with other professions, such as nursing, occupational therapy and social service, physical therapy plays an important and dynamic role in the medical, social and vocational rehabilitation of the physically and mentally handicapped. To appreciate the importance of a comprehensive programme of treatment for the maintenance of health, for the prevention of disability and for the rehabilitation of the sick and handicapped one must understand the scope of physical therapy and the role of the physical therapist. Physical therapy is the art and science of physical treatment by means of therapeutic exercise, instruction, heat, cold, light, water, massage and electricity. Among the aims of treatment are the relief of pain, the increase of circulation, the prevention and correction of disability, and the maximum return of strength, mobility and co-ordination. Physical therapy also includes the performance of electrical and manual tests to determine the amount of impairment of nerve supply and the strength of muscles; tests to determine functional abilities; measurement of range of joint movement, and measurement of vital capacity as diagnostic aids for the physician and for recording progress. As well as working to prevent disability and to rehabilitate the sick and handicapped, physical therapists are also active in preventive medicine and undertake clinical research. disability impairment disability handicapped rehabilitation

10 第 2 章理学療法の対象 図 2-4 人口ピラミッド ( 平成 17 年 10 月 1 日現在 ) 2. 障害と病気 1) 障害および病気の定義と役割 WHO 国際障害分類初版 (ICID-H) は以下の階層構造があった 障害とは機能障害 (impairment) 能力低下(disability) 社会的不利(handicapped) の 3 つのレベルがある ( 資料 13) 機能障害 : 解剖および生理学的に機能および構造の喪失および異常である 能力低下 : 人間として正常とみなされる態度や範囲で 活動していく能力のいろいろな制限や欠如 ( 機能障害に帰因する ) である 社会的不利 : 機能障害や能力低下の結果として その個人に生じた不利益であって その個人にとって ( 年齢 性別 社会文化的因子からみて ) 正常な役割を果たすことが制限されたり妨げられたりすることである 現在 2001 年 5 月 22 日第 54 回世界保健会議において新しい分類を承認した 国際障害分類改訂版 (ICF) は 3 階層をプラス包括用語で表す 1 心身機能構造 (body function structure): 機能とは 身体系の生理的または心理的機能で 構造とは 器官 肢体とその構成部分などの身体の解剖学的部分である 機能障害 (impairment) とは 著しい変異や喪失などといった 心身機能または身体構造上の問題である 2 活動 (activity) とは 個人による課題または行為の遂行のことである 活動制限 (activity limitation) とは 活動の遂行において個人がもつ困難のことである 3 参加 (participation) とは 健康状態 心身機能 構造 活動および背景因子に関連した生活 ( 人生 ) 状況 (life situation) への個人の関与のことである 参加制約 (participation restriction) とは 生活 ( 人生 ) 状況への関与の仕方または程度において 個人がもつ問題のことである 障害者とは 先天的か否かにかかわらず 身体的または精神的能力の障害のために 通常の個人的生活ならびに
