2 日本視能訓練士協会誌 Abstract Purpose: We have reported a panel with a protractor, which can easily align the axis of a toric intraocular lens during cataract surgery, at the Annual Meeting of the Japanese Society of Ophthalmic Surgery in 211. Here, we report the results of surgery using this panel. Subjects: Seventy-eight eyes of 54 patients (age, years range, 76.1 ± 6.1 years) who underwent eye surgery were during June 21 to July 211 were included in the study. In these patients, a toric intraocular lens (Acrysof IQ Toric:SN6AT3-5) was inserted and aligned to the axis by using the panel method. Methods: Before surgery, an anterior ocular segment image was obtained with the patient in the sitting position by using a camera attached a slit-lamp microscope. Marks such as iris patterns or pigments were noted in the image. A personal computer was used to insert an image of a protractor over this image, and the angle from a mark to the axis of the lens was calculated. The panel was put on the ocular segment image during the surgery, and a protractor was attached to the panel. The angle from an iris mark to the lens axis was measured, and a line was drawn on the panel. After the intraocular lens was inserted, the surgeon rotated the lens and aligned the mark that indicated the lens axis to this line. uncorrected visual acuity, corrected vision, subjective astigmatism, and the objective astigmatism were measured before surgery and 1 week, 1 month, and 3 months after surgery. Results: The average uncorrected visual acuity was.43 before surgery and.84 at 3 months after surgery. The average corrected vision was.71 before surgery and 1.1 at 3 months after surgery. The average objective astigmatism was 1.79 ±.83 D before surgery and.9 ±.51 D at 3 months after surgery. The average subjective astigmatism was 1.31 ±.88 D before surgery and.55 ±.47 D at 3 months after surgery. The axial gap with a schedule axis was 6.22 ± 6.1 degrees. Conclusion: Our results for uncorrected visual acuity and reduction of subjective astigmatism with the panel method were equivalent to the results reported previously. Ⅰ. 緒言乱視矯正眼内レンズ (toric intraocular lens: 以下 トーリックIOL) は手術前の座位及び手術中の仰臥位での乱視軸の決定が重要である トーリックIOLは29 年 8 月の発売以降 3 時 9 時マーク法 6 時マーク法 前眼部写真法 Axis Registration 法 虹彩紋理法など さまざまな方法で軸合わせが行われている 1),2) マーキングを行わず OHP(overhead projector) シートに分度器を印刷したシートで乱視軸を合わせるパネル法を211 年の日本眼科手術学会で報告した 3) パネル法はマーキングや特別な検査機器 手術器具を必要とせず軸合わせが行えるため 患者の負担が従来の白内障手術と同程度で済む パネル法を用いて軸合わせを行った手術成績を報告する Ⅱ. 対象及び方法対象は 21 年 6 月から211 年 7 月に同一術者により トーリックIOL(Acrysof IQ Toric:SN6AT3 SN6AT4 SN6AT5 アルコン社 ) を挿入した54 例 78 眼 (61~88 歳 : 平均 76.1±6.1 歳 ) 白内障以外の視力に影響する眼合併症を有さず 術後の裸眼視力を過去の報告と比較するため 目標術後屈折をDに合わせた症例を対象とした 術前 術後 1 週間 1カ月 3カ月の裸眼視力 矯正視力 他覚球面 自覚球面及び他覚乱視 自覚乱視を測定した 視力検査は5mの字づまり視力表で 原則的に小数視力 1.2まで測定し 他覚球面度数 他覚乱視度数は トーメーコーポレーション製のオートレフトポグラファー RT-6を使用して測定した 球面度数はDとの差を絶対値に変換し平均及び標準 196
3 第 41 巻 (212) 偏差を算出し 乱視度数も絶対値に変換し平均及び標準偏差を算出した logmarによる術前術後の裸眼視力 矯正視力の差及び 術前術後の視力を比較した 検定は対応のあるt 検定で行った 術前検査時に患者の同意を得た上で 散瞳下で細隙灯顕微鏡に取り付けたカメラによる前眼部撮影を行った 角膜曲率半径計測時と共に 顎台に顔を乗せる際は傾き 向きに注意した 細隙灯顕微鏡に取り付けられた前眼部撮影カメラで撮影し 前眼部写真をパソコンに取り込んだ パソコン画面上で 前眼部写真に分度器画像の中心を 瞳孔中心に重ね合わせ印刷した 目印 ( 虹彩紋理 色素沈着 血管形状 瞼裂斑など ) を決め そこからトーリックウェブカリキュレータ ( com/) で算出されたレンズ軸までの角度の計算を行った 手術開始後 顕微鏡映像のモニタに液晶保護パネル ( 以下パネル ) を重ねた 術者は瞳孔中心がパネルの中央にくるように顕微鏡の位置を調整した 視能訓練士は前眼部写真とモニタ映像を照らし合わせ目印を見つけた 分度器シートをモニタに取り付け 目印から計算しておいた角度に印をつけ 乱視軸の線を引いた ( 図 1) 術者はレンズ挿入後 顕微鏡から目を離しモニタを見てパネル上の線とレンズの軸マークが合うよう レンズフックでレンズを回転させた レンズのマークが見えにくい場合はレンズフックを眼外に出し マークに重ね合わせ モニタ図 1 瞳孔中心と目印を見つける 図 2 術者はモニタを見て軸を合わせるに引いた線とのずれを確認した ( 図 2) 眼内レンズの予定軸との軸ずれは 手術翌日に散瞳下で細隙顕微鏡のカメラで撮影し 術前検査と同様にパソコンに取り込み軸の測定を行った 散瞳不良等の理由により軸マークが確認できない例もあり 軸ずれを測定できなかった症例もあった その場合 NIDEK 社製の角膜形状 / 屈折力装置 OPD-Scan Ⅲ( 以下 OPD-Scan Ⅲ) で撮影すると軸マークが確認できた例もあった Ⅲ. 結果 1. 裸眼視力 矯正視力の変化 ( 図 3) 裸眼視力は 術前.43(logMAR.44±.3) 術後 1 週間.81(logMAR.11±.14) 術後 1カ月.87(logMAR.8±.12) 術後 3カ月.84 (logmar.1±.15) 矯正視力は 術前.71 (logmar.19±.21) 術後 1 週間 1.5(logMAR -.2±.7) 術後 1カ月 1.1(logMAR -.4±.5) 術後 3カ月 1.1(logMAR -.4±.6) 裸眼視力 矯正視力ともに有意に改善した (p<.1) 術前と比較し 術後は裸眼視力 矯正視力の差が縮まった (p<.1) 2. 他覚球面 自覚球面の変化 ( 図 4) 他覚球面は 術前 2.1±1.22D 術後 1 週間.5±.43D 術後 1カ月.61±.45D 術後 3カ月.68±.55D 自覚球面は術前 1.63±1.38D 術後 1 週間.3±.39D 術後 1カ月.39±.42D 術後 3カ月.39±.42D 他覚球面 自覚球面ともに術前と比較し 術後は有意に正視 197
4 日本視能訓練士協会誌 -.2 マークを撮影できなかった場合 OPD-Scan Ⅲ logmar を使用して 眼内屈折の軸により軸ずれを測定した 軸マークは46 眼で確認ができ 予定軸からの軸ずれは6.22±6.1 だった p<.1 裸眼視力矯正視力 Ⅳ. 考按.8 術前術後 1W 術後 1M 術後 3M 図 3 裸眼視力 矯正視力の変化 (logmar) (D) 3.5 p<.1 3 他覚球面 2.5 自覚球面 術前術後 1W 1M 3M 図 4 他覚球面度数 自覚球面度数の変化 (D) 3 p< 他覚乱視自覚乱視 術前術後 1W 1M 3M 図 5 他覚乱視 自覚乱視の変化に近づいていた (p<.1) 3. 他覚乱視 自覚乱視の変化 ( 図 5) 他覚乱視は 術前 1.79 ±.83D 術後 1 週間.74±.47D 術後 1カ月.8±.5D 術後 3カ月.9±.51D 自覚乱視は 術前 1.31±.88D 術後 1 週間.51±.48D 術後 1 カ月.52±.46D 術後 3カ月.55±.47D 他覚乱視 自覚乱視ともに有意に改善した (p<.1) 4. 予定軸との軸ずれ術後 翌日に散瞳下で細隙顕微鏡で前眼部を撮影し 術前検査時と同様に分度器画像を張り付け 軸を測定した また 細隙灯顕微鏡で軸 パネル法は 術直前に座位で角膜や結膜へのマーキングを行う必要がなく 点眼 洗眼 4) 消毒などでマークが消える心配もないことは大きな利点である 一方で 前眼部撮影用カメラや手術顕微鏡カメラ 液晶モニタが必要となる また マーキングを行わないため 前眼部写真で特徴的な目印が見つけにくい場合に乱視軸を決定するのが困難な症例もみられた 見つけにくかった具体的な例として 老人環による虹彩の色素 紋理の透見不良や 洗眼によって血管が拡張変化し術前写真と同じ血管が見つけにくいことがあった しかし 術前に前眼部写真で見つけていた目印と違った目印を手術室のモニタで見つけ出せたため 軸を決定できた症例もあった 特に 瞼裂斑はモニタ上で確認しやすい傾向があった 術中 患者の視線が安定しない場合は 軸の決定が困難となるため 術者は患者に対し顕微鏡の光源を見るよう促す必要があった 患者の多くは高齢で 角膜曲率半径測定や前眼部撮影のたびに顔の傾きが変わることがあり 注意が必要だった 測定と撮影を1 台で続けて行える器械があれば理想的である 症例は全て上方からの2.4mm強角膜切開で行ったが 患者の顔が傾いていれば切開位置が変わり 惹起乱視度数も変わることも考えられる また レンズ挿入時に 顔の向きが変わって視軸がずれていれば正確な軸合わせができないため 術者は術中 患者の顔の傾きや向きに注意が必要であった パネル法による軸合わせによる結果と術後 1 5)~1) カ月の過去の報告との比較を表に示す ( 表 1) 乱視軽減効果 裸眼視力改善効果は同等と思われた 軸ずれに関して 説田ら 11) は 細隙灯顕微鏡上で角膜に強主経線をマーキングする方法で軸合わせした場合の軸ずれは術後に前眼 198
