2 日本視能訓練士協会誌 Abstract Purpose: In the visual acuity test, intervention and encouragement to answer by the examiner for subjects who have answered, I donʼt know, often leads to a series of correct answers and an improvement in the test results. We evaluated the effect of the examinerʼs intervention on the visual acuity test results in patients with epimacular membrane. Subjects and Methods: Subjects were 39 eyes with epimacular membrane (average age, 73.3 ± 8.1 years) and 11 eyes with pseudophakia (average age, 75.6 ± 9.6 years) as controls. Epimacular membrane was classified into total adhesion with patent foveal pit (11 patients), total adhesion with absent foveal pit (10 patients), total adhesion with macular pseudohole (7 patients), and partial adhesion (11 patients) according to the report by Tsuchihashi et al. (2002). Before the visual acuity test, the subjects underwent complete refractive correction on the basis of objective or previous refraction values. They answered the directions of the gaps in Landolt rings, and the visual acuity was judged by the criterion of 3/5 (pre-intervention visual acuity). Then, the subjects were urged to give answers even when they were not sure of the directions of the gaps, and the results were regarded as the post-intervention visual acuity. In each subject, the values of the eye that was examined first were evaluated. Results: The mean±standard deviation of the improvements after intervention compared with the values before intervention was 0.04±0.06 log in the pseudophakia and 0.10±0.08 log in all eyes with epimacular membrane. It was 0.07±0.06 log in total adhesion with patent foveal pit, 0.14 ± 0.11 log in total adhesion with absent foveal pit, 0.12±0.07 log in total adhesion with macular pseudohole, and 0.07±0.07 log in partial adhesion, being large in total adhesion with absent foveal pit and total adhesion with macular pseudohole. Conclusion: Appropriate intervention by the examiner is considered necessary to allow patients with epimacular membrane to demonstrate their maximum visual performance. Ⅰ. 緒言視力検査の際 患者の中には視標が小さくなると注視する前から わからない と答えることがある あるいは はっきり見える視標以外は わからない と答えることがある このような患者に対し 何となくでも良いので切れ目の方向を教えて下さい と検者が回答を促すと 正答が続き視力の向上をみることがしばしばある このように検者が被検者に働きかけ 回答を促すことを本研究では 介入 と定義する この介入の効果について 浅川ら は 全 165 例のうちの 26.7% に 0.1 log 以上の視力の向上がみられ 特に黄斑疾患ではそれらの割合が眼内レンズ挿入眼 20.0% に対し 31.8% と大きかったと報告している これらの黄斑疾患の中でも特発性黄斑前膜 idiopathic epimacular membrane( 以下 EMM) は視力低下や変視症をきたすことが知られてお り 視力検査の際 視力の変動をみることがある 本研究では 特発性黄斑前膜の視力検査に関する検者の介入効果について検討した Ⅱ. 対象及び方法 1. 対象対象は 2011 年 4 月から7 月に当科を受診し EMMと診断された患者 39 例 ( 男性 11 例 女性 28 例 平均年齢 73.3±8.1 歳 ) であった なお 他に眼疾患があり続発性の黄斑前膜と考えられる症例は除外した 我々はそれらEMM の39 例のスペクトラルドメイン光干渉断層計 (RTVue-100 Optovue 社 ) の所見を土橋ら 2) の報告に従って分類した その内訳は 面癒着型中心窩陥凹存在 11 例 ( 男性 1 例 女性 10 例 平均年齢 70.7±9.4 歳 図 1-a) 面癒着型中心窩陥凹消失 10 例 ( 男性 5 例 女性 5 例 平均年齢 72.0±4.8 歳 図 1-b) 面癒着型偽黄斑円孔 7 例 172
3 第 41 巻 (2012) 図 1 スペクトラルドメイン光干渉断層計による特発性黄斑前膜の分類 a. 面癒着型中心窩陥凹存在 ( 前膜は網膜に密着し 中心窩陥凹は保たれている ) b. 面癒着型中心窩陥凹消失 ( 前膜は網膜に密着し 中心窩陥凹消失がみられる ) c. 面癒着型偽黄斑円孔 ( 前膜は網膜に密着し 中心窩陥凹が急峻となり偽黄斑円孔がみられる ) d. 架橋型 ( 前膜は網膜とところどころ分離しており架橋形成している 中心窩陥凹は消失している ) ( 男性 6 例 女性 1 例 平均年齢 76.6±9.2 歳 図 1-c) 架橋型 11 例 ( 男性 4 例 女性 7 例 平均年齢 75.1±8.4 歳 図 1-d) であった また 眼内レンズ挿入眼 ( 以下 IOL 挿入眼 ) で屈折異常以外に眼疾患のない患者 11 例 ( 男性 5 例 女性 6 例 平均年齢 75.6 ±9.6 歳 ) を対照群とした 年齢については EMMとIOL 挿入眼 (Mann-Whitney U test) 及び EMM を形態別に分類した4 群とIOL 挿入眼の間 (Kruskal- Wallis test) に有意差はなかった また EMM を形態別に分類した4 群間においても年齢について有意差はなかった (Kruskal-Wallis test) 2. 方法視力の求め方は浅川ら の方法に準じた 先ず前回の屈折値やオートレフラクトメータの値を参考にして自覚的屈折検査を行い 完全屈折矯正した その後 被検者にはランドルト環の切れ目の方向を回答させ 3/5の判定基準に基づき得られた視力を介入前視力とした その後 検者が被検者に なんとなくでも 歪んでいても 少しでも切れ目の方向が分かれば教え て下さい と介入し 再度視力を測定し得られた視力を介入後視力とした 介入前視力と介入後視力との差を向上値とした 分析の対象は 左右各眼のうち先に測定した第 1 測定眼のみとし 他眼には 既に検者による介入の効果が含まれていることを考慮し除外した またデータは経験年数 6 年の視能訓練士 1 名が測定したものとし 検者間による測定誤差に配慮した 3. 統計処理得られた視力値は 小数視力値からlog MAR 値に換算した 統計学的手法は 対応のある二元配置分散分析 Mann-Whitney U test Kruskal-Wallis test 母比率の差の検定を使用した 4. 倫理的配慮本研究の実施にあたっては川崎医科大学附属病院倫理委員会の承認 ( 承認番号 924) を得た 173
4 日本視能訓練士協会誌 Ⅲ. 結果 1. 検者の介入による視力の向上の程度 EMM 及びEMMを形態別に分類した4 群と IOL 挿入眼全ての介入前視力 介入後視力 向上値を表 1に示す 介入前視力と介入後視力の比較視力はEMM 及びEMMを形態別に分類した4 群とIOL 挿入眼全てにおいて 検者の介入により有意に向上した (P<0.01 対応のある二元配置分散分析 ) 2)IOL 挿入眼とEMM 及び分類別での比較 EMMの介入前視力 介入後視力はIOL 挿入眼に比べて有意に低値を示し (P<0.0 向上値は有意に大きかった (P<0.05 Mann- Whitney U test) また 分類別では 面癒着型中心窩陥凹消失と面癒着型偽黄斑円孔 架橋型の介入前視力 介入後視力がIOL 挿入眼に比べ 有意に低値 を示した (P<0.05 Kruskal-Wallis test) さらに 面癒着型中心窩陥凹消失と面癒着型偽黄斑円孔は IOL 挿入眼に比べ 検者の介入により視力が有意に向上した (P<0.05 Kruskal- Wallis test) 2. 検者の介入による視力の向上の割合 EMM 及びEMMを形態別に分類した4 群と IOL 挿入眼において検者の介入による視力の向上の割合を図 2に示す 検者の介入により視力が0.1 log 以上向上した割合は IOL 挿入眼が18.2% であったのに対し EMMでは35.9% であった EMMの分類別では 面癒着型中心窩陥凹存在 9.1% 面癒着型中心窩陥凹消失 60.0% 面癒着型偽黄斑円孔 57.2% 架橋型 27.3% であった 面癒着型中心窩陥凹消失と面癒着型偽黄斑円孔は IOL 挿入眼 面癒着型中心窩陥凹存在に比べ 視力が0.1 log 以上向上した割合が有意に大きかった (P<0.05 母比率の差の検定) 表 1 各群の介入前視力 介入後視力 向上値 各群の介入前視力 介入後視力の平均値 ± 標準偏差を logmar 換算値 ( 括弧内は少数視力の平均値 ) で示す 向上値は介入前後の視力の向上の平均値 ± 標準偏差を表す 視力は EMM 及び EMM を形態別に分類した 4 群と IOL 挿入眼全てにおいて 検者の介入により有意に向上した (** P<0.01, 対応のある二元配置分散分析 ) EMM の視力は IOL 挿入眼に比べ検者の介入により有意に向上した ( P<0.05, P<0.01, Mann-Whitney U test) EMM の分類別では 面癒着型中心窩陥凹消失と面癒着型偽黄斑円孔において IOL 挿入眼に比べ 検者の介入により視力が有意に向上した ( P<0.05, P<0.01, Kruskal-Wallis test) 174
5 第 41 巻 (2012) 図 2 検者の介入による視力の向上の割合下段の項目名は各分類群を示し n 数はその症例数を表す 検者の介入により視力が 0.1 log 以上向上した割合は IOL 挿入眼では 18.2% EMM では 35.9% であった EMM の分類別では 面癒着型中心窩陥凹存在 9.1% 面癒着型中心窩陥凹消失 60.0% 面癒着型黄斑円孔 57.2% 架橋型 27.3% であった 面癒着型中心窩陥凹消失と面癒着型黄斑円孔は IOL 挿入眼 面癒着型中心窩陥凹存在に比べ 視力が 0.1 log 以上向上した割合が有意に大きかった (*P<0.05, ** P<0.01, 母比率の差の検定 ) Ⅳ. 考按本研究では EMMの視力検査に関する検者の介入効果について検討した その結果 介入による向上値は 0.10 ± 0.08 log であり 35.9% に 0.1 log 以上の向上があった また EMMの分類別では 面癒着型中心窩陥凹消失と面癒着型偽黄斑円孔において介入により有意に視力が向上した 1. 検者の介入による視力の向上 3),4) 視力検査は心理物理学的検査であり 被検者の主観的な見え方を客観的な物理量を指標にして最小分離閾として評価している 臨床では簡略化した方法を用いているが 本来 閾値の決定には複数回の刺激を呈示した上でその正答率が50% に相当する刺激強度を求めなければならない したがって視力検査では 視標のぼやけや歪みなど視覚像の質に関係なく 被検者からギリギリの反応を引き出す必要がある 臨床では視標が小さくなると注視する前から わからない と答えることや はっきり見える場合以外は わからない と答える患者がしばしば見受けられる これは 質の低下した視覚像に対して回答できる という被検者の許容レベルに差があり 図 3に示すように検者が介入することにより許容レベルを閾値に近づけることができると推察され 本研究の結果はそれを裏付けるものであると考えられた そのため 被検者の最大限の視機能を引き出した上で正しく閾値を評価するためには 単に視力を測るだけでは不十分であり 検者の適切な介入が重要であることが示唆された 2. IOL 挿入眼とEMMにおける介入前と介入後視力の比較 EMMの視力は IOL 挿入眼に比べ検者の介入により有意に向上した これは EMMの視覚像の質が黄斑前膜によって低下したことで 検者の介入効果がIOL 挿入眼よりも強く生じたと考えられた 黄野ら 5) は 黄斑浮腫や中心性漿液性脈絡網膜症では視標を読み取る臨界時間 175
