1 特集肝臓の EOB プリモビスト造影 MRI 本検査を有効に活用するための理解すべき課題 8. における画像診断のストラテジー谷本伸弘, 上野彰久慶應義塾大学医学部放射線診断科 Imaging Strategy for Hepatocellular Carcinoma Akihiro Tanimoto, M.D., Akihisa Ueno, M.D. Department of Diagnostic Radiology, Keio University School of Medicine NICHIDOKU-IHO Vol.55 No (2010) Summary The development of hepatocellular carcinoma()in the cirrhotic liver is a multistep process,progressing from a low-grade dysplastic nodule(dn)through high-grade DN and early,well-differentiated and nodule-in-nodule,and ultimately to undifferentiated.regular non-invasive surveillance is important in all atrisk patients for the detection of carcinogenetic changes and early identification of smalldiameter cancerous lesions.the recently published Japanese evidence-based guidelines recommend that imaging is performed every 3-4 months in very high-risk individuals and every 6 months in high-risk subjects.detection of small focal liver lesions is crucial to provide optimal prognosis. The major guidelines consistently recommend that imaging is initially performed using ultrasonography(us),but an important limitation of its performance is that it is operator- and patient-dependent.the recent development of microbubble contrast agents has improved the previously limited ability of US to detect small focal liver lesions. In patients with advanced liver cirrhosis or obesity,evaluation of dynamic lesion enhancement using contrast-enhanced computed tomography(ct)or magnetic resonance imaging(mri)is regarded as being more effective.because of the more invasive nature of CT arterio-portography(ctap)and CT hepatic arteriography (CTHA),and the high false-positive rates without a substantial increase in sensitivity,these imaging techniques have been largely superseded by dynamic CT. Dynamic MRI using gadolinium-based extracellular fluid contrast agents is now acknowledged as providing superior diagnostic specificity and sensitivity compared with CT in the detection of.in addition to avoiding any potential risk of repeated radiation exposure,smaller volumes of contrast medium are required for MRI than CT,and adverse reactions are less frequent. Dynamic MRI and dynamic CT are principally useful for the diagnosis of hypervascular displaying typical enhancement patterns;however,it has been demonstrated that not all s display typical arterial-phase enhancement and washout in the equilibrium phase,which can make diagnosis difficult. Reticuloendothelial system-specific superparamagnetic iron oxide (SPIO)contrast agents allow the detection of nonhypervascular lesions that are difficult to see using dynamic MRI;however,due to the lack of dynamic-phase imaging, characterization of lesions is not always possible and distinguishing between well-differentiated and DN is difficult. Gadolinium-ethoxybenzyl-diethyleneaminepentaacetic acid(gd-eob-dtpa),also known as gadoxetic acid,disodium(eob- Primovist),is a liver-specific MR contrast agent developed for combined T1-weighted dynamic and hepatocyte-specific MRI in a single examination.in common with extracellular gadolinium-based contrast agents,gd-eob-dtpa provides information on vascular distribution during the arterial phase,portal-venous phase and equilibrium phase,and provides additional information during the hepatocyte phase(10-20 minutes post-injection).gd-eob-dtpa has the potential to provide not only improved detection,but also characterization,of focal liver lesions.the absence of functional hepatocytes in results in little or no uptake of Gd-EOB-DTPA in the hepatocyte phase.it has been documented recently that Gd-EOB-DTPA-enhanced MRI shows better diagnostic performance than dynamic MDCT and contrast-enhanced US.Many hypovascular early s are hypointense 日獨医報第 55 巻第 2 号 91-96(2010) 91(187)

2 in the hepatocyte phase,and could be distinguished from high-grade DN.Gd-EOB-DTPA-enhanced MRI may be incorporated into a new guideline for the management of both hypervascular and hypovascular nodules.in this article,the current diagnostic guidelines,results of recent studies of Gd-EOB-DTPA,and the strategic evaluation of Gd-EOB-DTPA in diagnostic imaging algorithms are reviewed. はじめに多段階発癌という特徴を示す () では典型的な多血性の診断のみならずいまだ乏血性等血性の段階にある肝細胞由来結節を早期に非侵襲的にかつ高い確信度で診断することが予防医療の点からも重要であるわが国を含め世界各国の診療ガイドラインが超音波検査を初回の画像検査としていたがここにEOB プリモビスト造影 MRIという新たな検査法が登場した EOB プリモビストが多くの診断技術のなかでどのような位置付けとなるか明確な意義は現在も模索中である本稿では従来のガイドラインの概略とEOB プリモビスト造影 MRIの最近の知見 EOB プリモビストを組み込んだ診断のストラテジーについて述べる現在のに対する診療ガイドラインの多段階発癌とは大再生結節 (large regenerative nodule:lrn) から軽度異型結節 (low-grade DN) 高度異型結節 (high-grade DN) を経て早期 (early) 高分化結節内結節型となりそして脱分化したへと至る過程である 1) このほか一部の癌はde novoに出現すると良性結節の鑑別は非常に重要であるが生検が小病変の鑑別の決めてとなるとは限らず侵襲性も伴うすべてのリスク患者に対して小さな癌を早期検出するための方法の標準化が望まれる 2003 年には米国 AASLD (American Association for the Study of Liver Diseases) やヨーロッパEASL(European Association for the Study of the Liver) が肝硬変患者に対する 6ヵ月ごとの検査を推奨した 2 3) しかしの疫学的事情が欧米諸国とわが国とで大きく異なるために日本肝臓学会は2007 年に独自にの診療マニュアルを作成した ( 図 1) 4) わが国ではの前癌病変が非常に高頻度であるためにダイナミックCT/MRIを径 1 2cmの病変の検出に超常磁性酸化鉄 SPIO 造影 MRI を非典型的病変の同定に推奨している点で欧米と異なる 4) 2cm 以上の病変の検出についてはダイナミック CT/MRIとより侵襲性の高いCTAP/CTHAとの間に差はない 5) しかしわが国の診療マニュアルでは SPIO 造影 MRIと同様いまだCTAP/CTHAが記載されている 4) また超ハイリスク患者には 3 4ヵ月ごとハイリスク患者には 6ヵ月ごとの画像検査と腫瘍マーカー検査が推奨されている 4) 予後改善のためには1cm 未満の小結節で検出することが望ましいが背景となる肝硬変の再生結節や線維化がその検出を難しくしている各国のガイドラインは超音波検査を最初の検査法としているが 2-4) 本検査は検者被検者での差が大きく結節の検出能は一定しない超音波造影剤ソナゾイドの出現により血流とKupffer 細胞機能がともに評価可能となった 6) いまだわが国でのみの承認であるがダイナミックCTと伍する診断能が報告されている 7 8) 超音波造影剤の進歩により小結節の検出能は向上しているものの進行した肝硬変や肥満の患者ではダイナミックCT/MRIによる検索がより客観的かつ有効でわが国でのガイドラインでも推奨している 4) 侵襲的検査であるCTAP/CTHAはダイナミックCT/MRIと比較しても感度の大きな改善はなく偽病変が多いため 9) その多くはダイナミックCT/MRIで置き換えられている繰り返しダイナミックCTを施行することで生じる被曝の問題は適宜 MRIを挟むことで軽減される 10) ダイナミックMRI は3D 脂肪抑制 T1 強調像による撮像が主体となりダイナミックCT 以上の感度特異度を示すようになってきた 11 12) 細胞外液性ガドリニウム製剤を用いたダイナミック MRIは動脈相でのwash-inと平衡相でのwash-outを示す多血性の診断に有用である造影効果の推移により等乏血性の良性結節と鑑別可能で CTと比較して被曝なく造影剤量造影剤副作用も少ない 13) しかしであっても等乏血性となる結節の存在がダ 92(188)

4 イナミックCT/MRIの限界を示唆している 14) SPIOは等乏血性の結節の検出に有効であるが DNと高分化との鑑別が難しい場合がある 14) EOB プリモビストはダイナミック MRIによる多血性の検出と肝細胞相による等乏血性結節の検出の双方に有効で通常のダイナミックMRIとSPIO 造影 MRIを加えた以上の情報が得られると期待されている肝細胞相が造影剤投与後分で撮像されるため細胞外液性造影剤によるMRI 検査より検査時間は延長する EOB プリモビスト市販後の知見る EOB プリモビストにおいて特筆すべき点はダイナミック MDCT でも CTAP/CTHA でも描出されない乏血性等血性結節を肝細胞相で低信号域として描出する能力である既出の検討で CTAP/CTHA で非典型的な 8 結節は B モード超音波で全結節が可視であったが 7 結節ではソナゾイド造影超音波 Kupffer 相で不明であった EOB プリモビスト造影 MRI では 8 結節すべてが肝細胞相で低信号で生検で高分化と証明された 18) 肝癌診断ガイドラインにおける EOB プリモビスト造影 MRI の位置づけ最近になりの検出におけるEOB プリモビスト造影 MRIと他のmodalityとの比較研究が発表されている韓国からのMDCTとの比較研究では 62 名 83 結節の切除されたを対象に3T 装置におけるEOB プリモビスト造影 MRIと64 列 MDCTを3 名の読影医がAFROC 解析により評価した 15) 統計的有意差に至らなかったが modality 間のAUCはMRIが MDCTがでややEOB プリモビストが優った特に1cm 未満の微小結節の検出にはEOB プリモビスト造影 MRIが優れていたイタリアからの報告では37 名 67 結節のに対しEOB プリモビスト造影 MRIとMDCT を比較した結果感度 (81.1% 対 65.5% P<0.05) 特異度 (94.7% 対 84.2% P<0.05) とも有意にEOB プリモビストが優れていた 16) また110 例 185 結節のを対象にした研究では EOB プリモビスト造影 MRIが三相撮影 MDCTより有意に感度が高かった (96% 対 84% P=0.009) 17) 最近超音波造影剤ソナゾイドとEOB プリモビスト造影 MRIを比較した報告がされている CTAP/CTHAを gold standardとした多血性の検出率はソナゾイド造影超音波の87.5% に比してEOB プリモビスト造影 MRIが98.3% と高率であった 18) また SPIOとEOB プリモビストの比較でも 27 例 38 結節の手術されたを対象にした検討では同等の診断能を示した 19) 89 例 118 結節のを対象にした検討では EOB プリモビストが AUC 感度 90.7% SPIOがAUC 感度 84.7% で有意差をもって EOB プリモビストが優れていた 20) これらは主に多血性のの検出に関する検討であ 2008 年 10 月に京都で施行されたEOB プリモビスト国際シンポジウムで多血性および乏血性結節における新しい診断および治療アルゴリズムが提唱された ( 図 2) 21) 特に多血性になる前の乏血性結節の長期経過観察を重視したこのアルゴリズムは今後も症例を蓄積して検討されるべきであるがヨーロッパ米国オーストラリア日本韓国タイ各国の参加者により支持された現在のエビデンスでは乏血性結節のほか多血性であるがwash-outが不明瞭な非典型的な結節はさらに検査を追加するか症例によっては生検まで考慮される 21) われわれの検討では肝細胞相で低信号に描出される1cm 前後の乏血性等血性結節はその約 1/3が 7ヵ月程度の doubling timeで増大しておりそのうち約 20% で1 年以内の多血化がみられたただしこれらの結節すべてを生検の対象とすることはリスク労力信頼性の点から臨床的に現実的でないまたどの施設でもMRI 検査はMDCTほど簡便に施行できるわけではなく通常のダイナミックMRIより時間のかかるEOB プリモビスト造影 MRIの検査枠には制約がある多血化を一つの明確な治療開始基準とすれば造影超音波や MDCTをリスクに応じて 3 6ヵ月に 1 回行うその合間に6 12ヵ月に1 回程度 EOB プリモビスト造影 MRIにて肝細胞相でのみ描出される結節の増大の有無をチェックする検査間隔が妥当かと思われるおわりに EOB プリモビスト造影 MRIはダイナミックMDCTと比較して同等以上の多血性の診断能を有し乏血 94(190)
