4 b a Fig. 3 c パーキンソン病に合併した SCN 障害性腰痛の治療前後における姿勢の変化文献 28 から引用 a c 術前 b d 術 4 カ月後かにできることに貢献したさらに Kim ら36 は十分な神経剝離により末 d 神経周囲血管の血流も改善する点に 9 パーキンソン病と上殿皮神経障害パーキンソン病はその病態から腰痛を併発しやすい着目し術中 indocyanine green video angiography ICG がその治療にはしばしば難渋する Iwamoto ら28 は VA を SCN 手術へ応用した術中 ICG VA は ICG をパーキンソン病に合併した SCN 障害を検討し報告して静脈注射することにより特殊な顕微鏡装置のもと神経のいる彼らの検討によると Hoehn Yahr 分類 3 4 の血流を観察できるため上殿皮神経の発見や十分な神経パーキンソン病で強い腰痛を呈した 8 例で SCN 障害を認除圧を確認できる利点を有するめ 4 例では SCN ブロックのみで症状は軽減しほかの上殿皮神経障害の手術成績 4 例では神経剝離術を追加し腰痛は軽減した結果 8 例 3 Maigne らは外科治療を行った 19 例の SCN 障害を中 7 例で Hoehn Yahr 分類が改善し中にはパーキンソ平均 3.2 年経過を観察したところ 13 例で成績は良好でン病によると思われた姿勢異常が一部改善した症例も存あったと報告している Isu らのグループは 34 例 55 側在したとする報告は興味深い Fig の SCN 障害に対して手術を行った結果 Roland Morris Disability Questionnaire RMDQ は Japanese 10 おわりに Orthopaedic Association JOA スコアはと改 SCN 障害による腰痛はその頻度は思いのほか高く善したと報告したまた Aota らのグループは 113 日常臨床で遭遇しやすい中には強い症状にもかかわら例中 19 例に外科治療を行いその結果 Visual Analogue ず診断にいたらないことがある半面ブロックのみで Scale VAS は RDQ はとなったと報難治性腰痛が改善することも経験する高山らは前述の告したがその予後予測因子としては罹病期間が 3 年 1961 年の論文にてすでに保存療法の限界にきている慢以内と SCN ブロックによる 3 日以上の効果持続であっ性腰痛症の中には脊椎固定術などの療法を考慮する以た前に上殿皮神経の遮断を考慮にいれてみても大きな過例の椎体骨折に合併した SCN 障害に関する報告で誤をおかすものではないことを認めざるを得ないと結は 17 例はブロックのみで対応でき 10 例で外科治療を論しているが 21 世紀の現代においても彼らの結論は受追加することで Numerical Rating Scale NRS は容できるものであろうわれわれ脊髄外科医にとって RDQ は JOA スコアはと SCN 障害による腰痛治療を 1 つの治療選択肢としてもつ改善した29 意義は小さくない以上の報告は短期成績の報告であり今後中長期の手術成績の報告が待たれる 144 文献 1 Deyo RA, Weinstein JN Low back pain.
