The arrow shows the left trigeminal nerve. 本症例の現象すなわち GKS が CPA 類上皮腫に伴っ cross talk 拡散を防止していると電気生理学的に解説しているすみやかに症状緩和に導かれる原因として ④ た三叉神経痛を消失させた機序について検討した CPA 類上皮腫が三叉神経痛を発症する原因としては重要な因子と思われるなお類上皮腫は三叉神経を Guo らは文献的検討から以下の 4 つの可能性 ①神経取り囲むように存在し神経に対する血管圧迫がないこ走行を歪曲させる腫瘍の直接的圧排効果 ②囊胞壁からとが多い17 三叉神経が類上皮腫により埋没あるいは浸潤するコレステロールの局所的な刺激 ③腫瘍神経圧排されている傾向 14 例中 11 例がある13 など諸家間の強い癒着 ④脱髄した軸索への局所的な圧痕や歪曲の報告 Table を合わせると三叉神経により生じる脱髄領域内の自発インパルス活動 sponta- 痛を伴う類上皮腫症例の総じて 80 程度に血管は関与 neous impulse activity を提案しているこのうち ① していないと推測されるさらに類上皮腫の術後長期と③に関して三叉神経痛を伴う類上皮腫症例の疼痛の観察中に画像上腫瘍の再増大がないにもかかわらず三叉緩和には手術により三叉神経に付着した腫瘍被膜の除去神経痛を再発した症例 11 例中 4 例ではその手術所が必要4 あるいは癒着の除去だけでなく nerve axis の見から三叉神経周囲の術後のくも膜肥厚癒着など術前矯正も必要など外科的治療戦略上不可欠な因子とすとは異なる機序で発症した可能性が提唱されているる報告があるただし ②については囊胞壁からのコレ以上のことから本症例は手術後のくも膜肥厚や癒着ステロール浸潤による化学的作用の可能性を提唱する報により生じた異常な電気的活動のために三叉神経痛が増告は他にも少なからず認めるもののそれを証明悪していた可能性を推定しているまた照射後画像上した報告はなく否定的な見解もあるまた Love 明らかな変化がないにもかかわらず摘出術後軽度ながらは臨床的に迅速な改善をもたらす microvascular ら残存していた三叉神経痛も含め症状が完全に消失して decompression は圧迫の解放が局所的な圧痕や歪曲にいることから GKS には直達手術で得られる減圧効果とより生じている脱髄軸索部のインパルスの異所産生はまったく異なる効能があると考えられた木田ら8 は ectopic generation of impulses を削減し脱髄軸索の分類上皮腫に対する臨床効果を GKS による神経除圧離が非シナプス性結合部位における電気的短絡 ephaptic radiosurgical nerve decompression と報告しまたの 36 脳外誌 28 巻 1 号 2019 年 1 月

Surg Neurol Int 7 suppl 4 S116 S120, 要旨 radiosurgical neuromodulation 脳外誌 28:33 39,2019 Editorial Comment 三叉神経痛を伴う類上皮腫に対するガンマナイフ治療千葉大学大学院医学研究院脳神経外科樋口佳則血管圧迫による三叉神経痛は, その背景に三叉神経の脱髄などの変化が存在することが考えられ (Love S, 2001, Brain), tractography を用いた研究により構造的変化が生じていることが示されている (DeSouza DD et al, 2014, Pain). 三叉神経痛のメカニズムに関しては議論の絶えない分野である. 腫瘍による症候性三叉神経痛は, 腫瘍による三叉神経の直接圧迫, 腫瘍により血管, 神経が偏倚し生じる三叉神経への血管圧迫が原因となる. 三叉神経痛をきたす類上皮腫の治療は, 腫瘍摘出術が第一選択である. しかし, 再手術が困難な症例では, 他の治療オプションも検討せざるをえない. 神経鞘腫, 髄膜腫に対するガンマナイフ治療とは異なり, 類上皮腫の報告はきわめてわずかである. 当然のことながら至適処方線量は存在しない. 著者らは, 聴神経腫瘍に対するガンマナイフ治療における治療線量と類似した辺縁線量により治療し, 腫瘍縮小が確認されない状態で三叉神経痛の改善を認めている. 典型的三叉神経痛に対するガンマナイフ治療では, 最大線量 70~90 Gy と高線量を処方することと比べると, 本論文での線量は低い. 筆者らは radiosurgical neuromodulation を提唱している.Kondziolka らの primate model で三叉神経への 80 Gy の照射により axonal degeneration を確認しており (Kondziolka D, 2000, Neurosurgery), 本報告での効果を組織学的な変化に求めることはできない. 低線量による三叉神経痛改善の機序に関しては, 今後の知見の蓄積が必要である. 39
