Name of sample (name should be related to data to be submitted concerning individual homogeneous materials) 2. Preconditioning method: If an official standard was used, its name, and if other than an official standard was used, indicate so. 3. Measurement method: Enter the name of the measurement method or the name of the official standard. 4. Name of the analysis institution and its seal or signature 5. Name of the persons who were in charge of the analysis institution and made the measurements. 6. Measurement date 7. Measurement results (If N.D. {not detected}, the lower quantification limit is also reported.) 8. Measurement flowchart 9. Preconditioning results: For the preconditioning method for Cadmium, Lead, and Mercury, please be sure to indicate the fact that the sample has been totally absorbed and dissolved by entering Totally Dissolved. 10.
