1 日エンドメトリオーシス会誌 63 ランチョンセミナー 卵巣チョコレート嚢胞の癌化 その発生メカニズムと鑑別診断 近畿大学医学部奈良病院産婦人科 小畑孝四郎 考えられている また排卵現象に伴い卵巣表層 はじめに 卵巣表層上皮から発生する悪性腫瘍が卵巣癌 上皮が陥入して 卵巣封入嚢胞 inclusion cyst であるが その発生には排卵が関与している が発生するが 間質に深く入り込んだ卵巣表層 すなわち 排卵により卵巣表層上皮の一部が破 上皮は容易にホルモンや化学物質の暴露を受け 綻し その後 破綻した卵巣表層上皮の修復 やすく 遺伝子異常を起こして発癌する過程も 再生過程で遺伝子異常が起こり発癌するものと 想定されている 最近 経口避妊薬 OC に よる排卵抑制が卵巣癌の発生を低下させること 表 経口避妊薬 内服期間 内服なし 年未満 年 年 年 年以上 が報告され 表 これらの発癌過程は 経口避妊薬の卵巣癌予防効果 epidemiological studies 実際存在するものと考えられる さらに 卵巣 相対危険度 症例数 FCI Cases controls RR and Collaborative Group on Epidemiological Studies of Ovarian Cancer : Lancet 子宮内膜症の合併頻度に関する病理組織学的な 検討 表 から 卵巣子宮内膜症が 卵巣明細胞腺癌や卵巣類内膜腺癌の発生母地と なっている可能性が注目され また最近の遺伝 子解析 から 癌抑制遺伝子のつで ある PTEN 遺伝子の異常が卵巣類内膜腺癌へ の癌化に関与しているといわれるなど 卵巣子 宮内膜症の癌化がクローズアップされている そこで 今回は 卵巣子宮内膜症 卵巣チョ 図 atypical endometriosis 細胞異型 構造異型を伴う卵巣子宮内膜症

6 68 小畑 陽性率 Inclusion Cyst 卵巣子宮内膜症性嚢胞 図 Calretinin Ber EP4 ER 抗体 図 92.3 PR Bcl 2 p 0.01 卵巣封入嚢胞および卵巣チョコレート嚢胞における免疫組織化学 染色 小池英爾 小畑孝四郎ほか 近畿大医誌 卵巣チョコレート嚢胞の発生仮説 A卵巣表層上皮の陥入と化生 図 卵巣チョコレート嚢胞の発生仮説 B子宮内膜類似上皮への化生 non cystic endometriosis typeの形成 卵巣癌における HER蛋白の発現は卵巣明 卵巣チョコレート嚢胞の発生機序として移植 細胞腺癌に多く 表 その発生に HER 説および化生説が有力であるが その進展様式 が関与しているかもしれない そこで 卵巣 として A癒着部分から広がる B封入嚢胞の 明細胞腺癌に合併する卵巣子宮内膜症におけ 化生から広がる C卵胞の生理的破裂部分から る HER蛋白の発現状態を検討したところ 広がるなどの諸説 がいわれているがいま 卵巣子宮内膜症 卵巣異型子宮内膜症 明細胞 だ明らかではない 卵巣チョコレート嚢胞の上 腺癌の順にその発現頻度は高くなった 表 皮には ER PR の発現が減弱していることや これらの結果から 卵巣子宮内膜症から卵巣 中皮のマーカーであるカルレチニンの発現がチ 明細胞腺癌の発癌過程に HER蛋白の関与が ョコレート嚢胞周辺に存在する non-cystic en- 示唆されるとともに 卵巣チョコレート嚢胞は dometriosis type やチョコレート嚢胞の上皮 卵巣明細胞腺癌の発生母地として最適な状態と の一部に認められることから 卵巣チョコレー 考えられた ト嚢胞も卵巣表層上皮の化生により発生してい 卵巣チョコレート嚢胞の発生と発癌 る可能性が高いと考えられる さらに 卵巣チ
