病 巣 と反 対側 に小 脳 症 状 と感 覚 障害 を 呈 した橋 梗 塞 の 1 例 図 1 a :頭 部MRI水 平 断 T1WI (TR 平 断 T2WI 600, め る.b: 頭 部MRI水 TE80). 梗 塞 巣 は橋 上部 に 存 在 す る. TE (TR2, 000, 20). TE 16: 147 橋 底 と被蓋 の境 界 部 正 中 よ りに梗 塞 巣 を認 80). c: 頭 部MRI冠 状 断 T2WI (TR2, 000, 他 に 症 状 を説 明 し得 る病 変 が 確 認 で き な い こ と よ り, 症状 出 現 とは 反 対 側 の 橋 上 部,橋 底 部 と被 蓋 の 境 界 部 の 正 中 に 近 い 領 域 に 存 在 し た 梗 塞 が そ の 責 任 病 変 と考 え られ た.病 変 は 直 径 約5mm程 度 の類 円 形 病 変 で あ り 橋 傍 正 中 枝 領 域 のlacunar infarction と考 え ら れ る1) (図2).橋 infarction と して は,pure motor に お け るlacunar hemiparesis, clumsy hand が,Fisherが ataxic hemiparesis, syndrome dysarthria な ど が 比 較 的 よ くみ ら れ る 記 したlacunar syndrome の review 2)に は本症 例 に 該 当 す る症 候 は な く,lacullar syndromeと して は例 外 的 な 症 例 と考 え られ る. 本症 例 の 運 動 失 調 は 測 定 障 害,協 運 動 障 害,体 幹 運 動 失 調,歩 調 運 動 障 害,変 換 行 障 害 を 主 と して い た. 運 動 失 調 の 出 現 に 関 し て深 部 感 覚 障 害 や 前 庭 障 害 に よ る運 動 失 調 の 関 与 の 可 能 性 は 全 く否 定 は で きな い もの の,閉 眼 に よ る症 状 の 悪 化 を 認 め な い こ と,SEPに て 内側 毛 帯 が 障 害 さ れ て い る 所 見 は あ る もの の 臨 床 的 に は深 部 感 賞 障 害 を 認 め な い こ と,明 らか な 肢 節 運 動 失 調 も存 在 す る こ と よ り小 脳 性 の 運 動 失 調 が 主 体 と な っ てい る と考 え られ た3)4). 従 来,橋 の 片 側 性 病 変 に よ る小 脳 失 調 は,上 脈 症 候 群(Mills症 候 群),橋 外 側 症 候 群,Marie- FOix 小脳動 上 部 被 蓋 症 候 群,橋 中部 図 2 中部 被 蓋 症 候 群(Grenet症 候 群),橋 下 部 外 側 症 候 群 な 小 脳 脚 が,橋 どい ず れ も病 変 と同 側 に 生 じ る5)と さ れ て お り,病 変 hemiparesisは と反 対 側 に生 じ る 小 脳 失 調 は,1965年 脳 脚 が,ataxic 唱 したhomolateral のataxic ataxia hemiparesis7)の して は,Mills症 橋 の 血 管 支 配. 文 献1)よ り. cerebeller hemiparesis,橋 and にFisherら crural み と 言 え る.責 が提 paresis 6)のち 任 病巣 に関 候 群,橋 上 部 被 蓋 症 候 群 に 関 し て は上 中 部 外 側 症 候 群,Marie中 小 脳 脚 が,橋 hemiparesis Foixcerebeller 下 部 外 側 症 候 群 は下 小 は皮質 橋 小脳 路 が原 因 と 考 え られ て い る. 本 症 例 に お け る 小 脳 性 運 動 失 調 はMRIに 巣 部 位 よ り上 小 脳 脚,中 お ける病 小 脳 脚 と は 離 れ て お り,こ れ

Abstract A case of pontine infarction showing hemiataxia with homolateral sensory disturbance Yasuhiro Watanabe, M.D., Kenji Isoe, M.D., Hiromichi Tanaka, M.D., Jun Saitou, M.D. and Masuyuki Fukada, M.D. Department of Neurology, Shimane Prefectural Central Hospital We report a female case of pontine lacunar infarction showing hemiataxia and homolateral sensory disturbance. The ataxia consisted of limb ataxia and truncal ataxia on the left side. The sensory disturbance appeared as paresthesia on the left half of the body and hypesthesia on the left half of the face. MRI revealed a small lacunar infarction at an opposite site in the upper pons. The lesion was situated in the border zone between the base and tegmentum, and almost on the midline. It was thought to be an infarction in the territory of the paramedian artery. Contralateral cerebellar signs were thought to have been caused by interruption of the precrossed pontocerebellar fibers or destruction of the reticular tegmental pontine nucleus. Further, destruction of the medial lemniscus and ventral trigeminothalamic tract was thought to have elicited sensory disturbance. (Jpn J Stroke 16: 146-150, 1994)
