Posterior Displacement Acromioplasty for the Surgical Repair of Tears of the Shoulder Rotator Cuff Hirokatsu HANAMURA et al. Kasugai Orthopedic Hospital Abstract In surgical repairs of rotator cuff tears, some types of acromioplasty have been done for the reconstruction of the suprahumeral gliding mechanism. We devised a new acromioplasty to prevent postoperative impingement without weakening the leverage of the deltoid. The incision was made along the gsuspender line hfrom the posterior edge of the acromion to the coracoid. The acromion was osteotomized anteroposteriorly just lateral to the acromioclavicular joint. The anterior portion of the deltoid muscle was split bluntly, and then the osteotomized lateral fragment was reflected laterally with the deltoid. After excising the entire coraco-acromial ligament and repairing cuff tears with Mc Laughlin or fascial graft methods, the lateral fragment was displaced posteriorly about 1.5 to 2cm (approximately one third of the width of the acromion), and reattached to the medial fragment with Kirschner wires or small plates. The undersurface of the anterior stump of the medial fragment was beveled. Postoperatively, the arm was fixed on an abduction splint, and passive motion exercises over 90 degrees while in the brace were begun within seven days. Active exercises were allowed from three weeks after operation. Fifteen cases, with complete tears in eleven and partial tears in four cases, were operated on with this procedure. Thirteen shoulders with longer than six months of follow-up period were evaluated with Wolfgang's key words : shoulder, rotator cuff tear, acromioplasty.
rating criteria from twelve to fifty-six months following surgery. Results of all cases were excellent. The value of this procedure is thought to be that it provides better exposure and facilitates appropriate surgical repairs, and that a residual impingement can be prevented without weakening the leverage of the deltoid muscle. 31 図458才,農 業,大 断 裂 の た めfascial graftを 行 った 症例 な か っ た 両 群 と も安 定 し た 除 痛 効 果 が 得 ら れ,ま 外 転 運 動 はP群 で は 全 例 と も早 期 に 正 常 範 囲 内 に 回 復 して い る が,L群 3.0で あ っ た L群 っ た2例 で,骨 は,い で は1 4点,平 でROMの 均 改 善 が特 に悪 か 法 に よ り修 復 した が,今 日 な ら ばfascial 外 転 筋 力 を み る と,P群 13例 中9例,L群 均3.7 均2.8と で は8例 graftに 劣って い 中3例 で で あ った よ る腱 板 修 復 の 一 番 の 問 題 は 十 分 な 筋 力 の 獲 得 が 難 し い 点 で あ る が,P群 例 は い ず れ も良 好 な 筋 力 が 得 ら れ,現 湾 労 務,建 築 吹 付 け 作 業,農 の4 在 営林作 業 に それ ぞ で は3 4点,平 均3.3で 均3.8,L群 で あ っ た 腱 板 が 断 裂 を 起 こ す 前 と 同 じ程 度 に 使 用 が 可 能 と な っ た 症 例 は, P群 で10例,L群 で は3例 腱 板 断 裂 の 進 入 路 と し て,肩 峰 を前 後 方 向 に で あ っ た ま たP群 で は 全 て 原 職 に復 帰 して い る 神 経 を 損 傷 す る 危 険 が 少 な く広 い 手 術 野 が 得 ら れ る こ と か ら 広 く普 及 して き た11) こ の 進 入 路 で は 三 角 筋 が 付 着 した外 側 骨 片 の 処 理 方 法 が 問 題 で あ り,Mc Laughlin12),Smith-Petersont14) ら はimpingementを 防 止 す る 目的 に て 外 側 骨 片 を積 極 的 に 切 除 し三 角 筋 の み を 内 側 骨 片 に 縫 合 す る 方 法 を 推 奨 して い る ま たCampbell ら の 教 科 書 的 な 手 術 書 に も広 範 囲 肩 峰 切 除 術 を む し ろ よ し と す る 記 載 が あ る が2,3,11),肩 れ 特 に 支 障 な く従 事 し て い る(図4) は2 4点,平 察 骨 切 り し上 方 よ り腱 板 に 達 す る進 入 方 法 が 腋 窩 の3 4点,平 で は1 4点,平 機 能 点 はP群 例 が 手 術 に 対 して満 足 し て い る 考 graft た ほ ぼ 正 常 な 筋 力 が 得 ら れ た もの は,P群 業,港 ら れ て い る こ と か ら,全 Laughlin を 選 択 す る症 例 で あ る fascial 能 的 に も悪 化 し た 症 例 は な く何 ら か の 改 善 が 得 ず れ も腱 板 の 変 性 が 強 い 大 断 裂 頭 内 に 骨 溝 を 作 り無 理 し てMc に 比 べ,L群 た機 切 除 に 伴 い 三 角 筋 のlever く な り,そ し,ま mentの armは 峰 必然 的 に弱 の 結 果 三 角 筋 の 機 能 不 全 を もた ら た 瘢 痕 や 化 骨 形 成 に よ る 新 た なimpinge招 来 な どの 弊 害 が 明 ら か に な っ て き た9,10,13,15) Neerは 剖 検 屍 体 の 肩 峰 を 観 察 し,
