Title: Transfemoral carotid artery stenting using proximal balloon protection for patients with severe elongation of the aortic arch - The method of inner-catheter exchange with the balloon guide catheter anchored: BGA Exchange method- 1 1 1 1 1 1 1 1 0 1 Abstract Objective: Anatomical factors such as aortic arch elongation or angulation associated with elderly patients with arteriosclerotic change often make it difficult to perform carotid artery stenting (CAS) via the transfemoral approach under proximal balloon protection (PBP). We report a novel technique of transfemoral CAS using the inner catheter exchange method for a patient with a type Ⅲ aortic arch. Case presentation: A -year-old man presented with a symptomatic left carotid artery stenosis. A balloon guide catheter (BGC) was needed for the PBP via the transfemoral approach. First, the modified Simmons-shaped inner catheter could not follow the half-stiff long guidewire to the external carotid artery (ECA) by the co-axial method. Therefore, the inner catheter was safely exchanged with the flexible JB catheter with the balloon of the BGC fully inflated on the way to the left common carotid artery (CCA). Over the half-stiff long guidewire and with the JB catheter advanced into the distal portion of the left ECA beforehand, the BGC was successfully navigated into the left CCA below the bifurcation. Further procedures were performed 1
without any complications. 石 灰 化 を 認 めた Arch elongationの 程 度 は 左 CCA 起 始 が 大 動 脈 弓 内 側 カ ーブの 低 位 から 分 岐 するaortic arch type III ) に 該 当 した (Fig. 1A, B) これらの 所 見 は 経 大 腿 動 脈 アプローチでのCASが 困 難 であることを 示 唆 した しかし 本 症 例 は 症 候 性 の 高 度 狭 窄 病 変 を 有 していたため distal embolic protection deviceの 病 変 通 過 時 に 遠 位 塞 栓 のリスクが 高 い と 判 断 し 経 大 腿 動 脈 アプローチによるPBPを 選 択 した 1 1 1 血 管 内 治 療 手 技 日 前 からクロピドグレル mg とアスピリン 0 mg を 投 与 し 局 所 麻 酔 下 で 手 技 を 開 始 した BGC には Fr OPTIMO temporary occlusion balloon ( 東 海 メディカルプロダクツ, 愛 知 ) そして 誘 導 用 インナーカ テーテルにシャフト 剛 性 が 非 常 に 高 く 角 度 保 持 力 が 強 い. 1 1 1 1 1 1 1 1 0 1 左 大 腿 動 脈 遠 位 に 誘 導 した ( Fig. A) ガイドワイヤーを 残 して JB を 抜 去 し exchange 法 で SIB を 左 総 腸 骨 動 脈 に 誘 導 した SIB を 総 腸 骨 動 脈 の 合 流 部 分 まで 引 き 戻 しシモンズ 形 状 を 作 成 した ( Fig. B) その 形 状 を 保 持 した 状 態 で 胸 腹 部 大 動 脈 内 を 上 行 させ 左 CCA 起 始 に SIB を 挿 入 した ( Fig. C) Half-stiff 0.0 inch 00 cm ロングガイドワイヤー (Radifocus Guidewire M half stiff type; テルモ, 東 京 )を ECA の 十 分 遠 位 ま で 進 めた SIB に traction をかけつつ バルーンを half-inflation した 状 態 の BGC を SIB 先 端 の 高 さまで 誘 導 し た ( 凧 揚 げ 法 : Kite-flying method) ( Fig. D 矢 印 ) さらに balloon を 最 大 拡 張 させて BGC を 固 定 した 状 態 で SIB を 遠 位 に 誘 導 することを 試 みた しかし SIB が 先 進 せ ず 逆 に BGC が kickback し システム 全 体 が 滑 落 しかけた そのため 追 従 性 の 高 いインナーカテーテルへ の 交 換 を 企 図 した その 手 技 (The method of inner catheter exchange with the balloon guide catheter anchored: BGA Exchange method) の 詳 細 を 以 下 に 示 す BGC の 滑 落 を 防 ぐために BGC の バルーンを 最 大 拡 張 し た ロングガイドワイヤーを 残 し て BGC 内 腔 か ら SIB を 慎 重 に 抜 去 した ( Fig. E 点 線 矢 印 ) 次 に exchange 法 で JB を 挿 入 し ECA 遠 位 まで 誘 導 した ( Fig. F 矢 印 ) 最 後 にバ ルーン 拡 張 を 解 除 し BGC を 総 頚 動 脈 遠 位 に 留 置 しえた ( Fig. G 矢 頭 ) バルーン 閉 塞 時 間 は 分 であった こ れ 以 降 は double balloon protection の CAS を 施 行 した PBP 下 に Carotid Guardwire/ PercuSurge 00 cm( Covidien, Minneapolis, MN, USA) を 病 変 通 過 させ 左 内 頸 動 脈 に 誘 導 した distal balloon protection を 追 加 し 前 拡 張 は Sterling Monorail Balloon Catheter.0 mm-0 mm (Boston Scientific, Marlborough, MA, USA) ステントは Protege RX
Carotid Stent System, Tapered / mm-0 mm (Medtronic, Dublin, Ireland) そして 後 拡 張 は Sterling.0 mm-0 mm (Boston Scientific) を 使 用 した 術 後 の 血 管 撮 影 ( Fig. B)では 有 効 な 拡 張 がえられ 合 併 症 なく 手 技 を 終 了 した ( 全 手 技 時 間 1 時 間 分 総 血 流 遮 断 時 間 1 分, heparin 総 投 与 量 000 単 位, ACT 最 高 値 秒 ) 術 翌 日 の DWI では 新 規 虚 血 巣 の 出 現 を 認 めなかった 1 1 1 1 1 1 考 察 BGA Exchange methodは 経 大 腿 動 脈 アプローチが 困 難 な 大 動 脈 弓 形 状 を 有 する 頚 動 脈 狭 窄 病 変 に 対 し PBP 下 でのCAS 手 技 の 成 功 に 寄 与 した 既 知 の 報 告 1) では 目 標 血 管 の 開 口 部 から 下 行 大 動 脈 起 始 までの 距 離 が 長 いことが 手 技 困 難 の 予 測 因 子 で あ った 本 症 例 では 左 CCA orificeから 大 動 脈 弓 の 頂 点 までの 水 平 距 離 が 長 く 左 CCAが 大 動 脈 弓 の 低 位 から 狭 い 角 度 で 分 岐 していた さらにtype Ⅲ aortic archと 石 灰 化 を 合 併 し 経 大 腿 動 脈 アプローチが 困 難 である 解 剖 学 的 特 徴 を 複 数 有 していた その ために 今 回 提 唱 するガイディングカテーテルを 誘 導 するための BGA Exchange method を 要 した 1 1 0 1 本 手 技 の つの 特 徴 は 1. 形 状 の 異 なる 種 類 のインナーカテーテルを 使 用 したこと そして. BGC とインナーカテーテルを 目 標 血 管 へ 誘 導 するために 適 宜 バル ーンを 拡 張 させた まず 凧 揚 げ 法 ( Kite-flying method)では 血 流 に 乗 せるよう 先 端 バルーンを half-inflation し BGC を SIB 先 端 まで 誘 導 した さ らに BGA Exchange method では balloon を 最 大 拡 張 し BGC を 左 総 頚 動 脈 に 固 定 ( anchoring ) し た BGC のバルーン 拡 張 は SIB 抜 去 時 に BGC を 安 定 化 させ 大 動 脈 弓 へ システム 全 体 が 滑 落 することを 防 いだ また 強 いサポートをもたらし JB を ECA 遠 位 まで 確 実 に 挿 入 することに 寄 与 した 本 法 は 経 大 腿 動 脈 アプローチが 困 難 な aortic elongation を 有 する 症 例 そして 右 上 腕 アプローチが 困 難 な 左 頚 動 脈 病 変 に 対 して 有 効 であることが 示 唆 された その 他 BGC を 併 用 した stent-retriever によ る 急 性 期 血 行 再 建 術 への 応 用 も 可 能 である 本 手 技 は BGC の 誘 導 中 に インナーカテーテルの 交 換 とバルーンの 使 用 という 通 常 の CAS とは 異 なる 過 程 が 加 わるの で 以 下 の 留 意 すべき 点 が 存 在 する まずインナーカテーテルを 交 換 せずに BGC を 総 頚 動 脈 遠 位 まで 誘 導 することを 試 してみる 例 えば balloon を 拡 張 させて BGC を 固 定 (anchoring)し た 状 態 で Simmons 形 状 カテーテルを ECA へ 先 進 させる ことを 試 みるべきである 虚 血 不 耐 性 例 では BGC の バルーン 拡 張 時 間 を
