Tentorial DAVFs were classified into six types by Lawton et al. in In their literature, Type I Lawton classification was defined as a Galenic DAVF, all of which were Borden Type 3. Neuroendovascular therapy has become the predominant treatment modality for intracranial DAVFs because the arterial supply from the external carotid artery (ECA) can be embolized safely, and the localization near the dural venous sinuses facilitates access and occlusion through that sinus. The combination of transarterial and transvenous embolization results in high obliteration rates for most DAVFs, but tentorial DAVFs are an exception. Their arterial supply is extensive, involving meningeal arteries from the internal carotid artery and vertebral artery, both of which are difficult to cannulate, and embolization is more risky compared to that of the ECA feeders. Transvenous navigation in deeper locations around the tentorium is difficult. More importantly, tentorial DAVFs (especially, Galenic DAVFs) often drain exclusively into the subarachnoid veins rather than into their associated sinuses (Borden Type 3), which prevents transvenous access. Therefore, the management of tentorial DAVFs may require microsurgical interruption, unlike most other DAVFs. We report a case of an isolated Galenic DAVF associated with Marfan syndrome that could be completely cured with transvenous embolization alone. はじめに小脳テント硬膜動静脈瘻 (dural arteriovenous fistula: DAVF) の発生頻度は, わが国の疫学調査 8) では全 DAVF の 3.2% と低く非常にまれな疾患である. しかし, その小脳テント DAVF の 97% は頭蓋内出血や静脈うっ滞による脳虚血をきたすなど重篤な経過をたどることが多く 1), 積極的な治療が必要となる.

2 はじめに小脳テント硬膜動静脈瘻 (dural arteriovenous fistula: DAVF) の発生頻度は, わが国の疫学調査 8) では全 DAVF の 3.2% と低く非常にまれな疾患である. しかし, その小脳テント DAVF の 97% は頭蓋内出血や静脈うっ滞による脳虚血をきたすなど重篤な経過をたどることが多く 1), 積極的な治療が必要となる.Lawton ら 9) はこの小脳テント DAVF を 6 型に分類 (Fig. 1) し, その治療には分類を問わず開頭手術によるシャント遮断術が必要と報告している. 今回われわれは,Marfan 症候群に合併した isolated galenic DAVF(Lawton Type 1=galenic DAVF) に対し経静脈的塞栓術単独で根治し得た症例を経験したので, 若干の文献的考察を加え報告する. 症例症例 :39 歳, 男性. 主訴 : 意識障害, 右半身不全麻痺既往歴 :Marfan 症候群 (9 年前に胸部下行大動脈人工血管置換術を,8 年前に腹部大動脈人工血管置換術を,6 年前に Bentall 手術および胸部上行全弓部人工血管置換術を,5 年前に僧帽弁置換術を,2 年前に胸腹部大動脈人工血管置換術を当院心臓血管外科にて施行され, ワーファリン内服中であった ) 現病歴 :2011 年 7 月 20 日に突然の意識障害, 右半身不全麻痺が出現し, 当院へ緊急搬送となった. 来院時神経学的所見として Japan Coma Scale(JCS)2, 右半身不全麻痺 (MMT 4/V) を認め, 頭部 CT では脳室内穿破を伴う左視床出血を認めた (Fig. 2A). 入院後経過 : 出血源検索のため CT angiography を施行したところ,vein of Galen にシャントを有する DAVF を認め, さらに大動脈は手術による影響でソーセージ状となっていた (Fig. 2B). そのため, 脳血管撮影検査は両側経上腕動脈アプローチにて行ったが, 左総頚動脈へは手術による影響でカテーテルがカニュレーションできず, 右側内頚動脈撮影と両側椎骨動脈撮影のみの施行となった. 流入血管は, 右後頭動脈およびテント動脈から認め vein of Galen 後半に収束しており,isolation された vein of Galen は拡張してポーチを形成した後, 左 basal vein に流出し uncal vein を介して下錐体静脈洞へドレナージされていた (Fig. 3A, B, E). 直静脈洞は静脈相でも描出されず (Fig. 3C, D), また後方循環からの流入血管は認められなかった. 以上より,isolated galenic DAVF(Borden type 3) と診断した. 治療 : 再出血予防のため DAVF に対する治療を行うこととした. 根治を得るためにはシャント部の閉塞が必要であるが, 全身性血管病変があること, 病変が深部に存在することより, 経静脈的塞栓術を第一選択とした. 入院第 32 病日に全身麻酔下で脳血管内治療を施行した. 左大腿静脈より 6 Fr ガイディングカテーテルを, 右上腕動脈より 5 Fr シャトルシースを挿入した.6 Fr ガイディングカテーテル内には 4 Fr のカテーテルをコアキシャルに挿入し右横静脈洞に留置し, マイクロカテーテルとマイクロガイドワイヤーを用いて閉塞している直静脈洞を頭側へ掘り Fig. 1 Tentorial dural arteriovenous fistulas (DAVFs) were categorized into six types based on the anatomic location, dural base, associated venous sinus, and direction of venous drainage. E: Three-dimensional rotation angiography showed convergent shunts at the back of the Galenic DAVF (yellow arrows). 進めていった. すでに直静脈洞は閉塞していたうえ,vein of Galen から直静脈洞への移行部で強い屈曲があり, その抵抗に難渋したが, 最終的に通過に成功した (Fig. 4A). ポーチ内でマイクロカテーテルを反転させて後方のシャント部近傍からの塞栓を試みたが, カテーテルの追従が悪く, 無理をするとワイヤー穿孔も危惧されたため, 流出静脈側からシャント部にかけてポーチ全体を可及的に速やかに計 19 本 265 cm のコイルで充塡した (Fig. 4B). 最終造影にて DAVF が完全消失 (Fig. 5) しており, その他の血管に異常がないことを確認し手術を終了した. 474 脳卒中の外科 43: 2015

転帰 : 術後経過は良好で,modified Rankin Scale(mRS) 3 で回復期リハビリテーション病院へ転院となった.

考察小脳テント DAVF は周囲の解剖学的特徴 ( テント切痕前方に脳幹部が走り, 前外側縁には上錐体静脈洞が, 後外側縁には横静脈洞が,

Fig. 5 The isolated Galenic DAVF completely disappeared as shown on the final right internal and external angiography. 転帰 : 術後経過は良好で,modified Rankin Scale(mRS) 3 で回復期リハビリテーション病院へ転院となった. 治療 1 年後の頭部 MRI/MRA では DAVF の再発を認めていない. 考察小脳テント DAVF は周囲の解剖学的特徴 ( テント切痕前方に脳幹部が走り, 前外側縁には上錐体静脈洞が, 後外側縁には横静脈洞が, 正中部には直静脈洞が, 後端には静脈洞交会が存在する ) より,Matsushima 10) らにより 4 型に, Picard 12) らにより 3 型に,Lawton 9) らにより 6 型に分類されている. 本例は Lawton 分類のうち Type I に属している (Fig. 1). 頭蓋内 DAVF の治療は, 一般的には血管内治療が第一選択である. 部位が海綿静脈洞部や横 -S 状静脈洞部などの静脈洞付近にある場合には, 経静脈的アプローチにより静脈洞を介してシャントを閉塞することが可能である 4)5)13)-15). また, 静脈洞を介さないタイプや,isolate さ Surgery for Cerebral Stroke 43:

5 れた静脈洞に対しては, 主として外頚動脈の枝を介した経動脈的塞栓術が適応となる 11). また, 両者の組み合わせにより頭蓋内 DAVF の高い治癒率も報告されている 2)3)5)-7)15). Lawton らは, 自験例で直達手術を行った小脳テント DAVF 31 症例をまとめて報告している. その中で小脳テント DAVF は,1 流入血管が多岐にわたる ( 硬膜血管を含めた内頚動脈系から椎骨動脈系まで ) ため, 外頚動脈系よりカテーテルをカニュレーションすることが難しく, 塞栓術を行うことに危険を伴うこと,2 局在が深部であり経静脈的ナビゲーションが困難であること,3 Borden Type 3 であることが多く特に galenic DAVF(Lawton Type I) は,Lawton の自験例にて 100% が Borden Type 3 であった, 経静脈的アプローチが困難であること, などの特徴を有していると報告している 9). それゆえに, 小脳テント DAVF の治療には他部位の DAVF と違い, 直達手術が一般的に有効であるといわれている 2)3)5)-7)15). 今回われわれの症例では,vein of Galen と直静脈洞との交通がなく vein of Galen が isolate された形となり, 深部静脈の圧が上昇し出血したと考えられたため, シャントの完全閉塞が必須であった. 経動脈的塞栓術も考慮されるが, 本例では大動脈が数回の人工血管置換術により通常の分岐状態でないうえ, テント動脈の塞栓におけるリスクもあり, 経静脈的アプローチ以外に血管内治療の可能性は考えられなかった. 幸い, 本例では軟膜動脈の関与がなく,isolate されたポーチ後半にシャントが収束しているというシンプルな構造であったため, ドレナージルート側での閉塞で根治が得られた. また, 本例でもそうであったが, 血管内治療が不可能であった場合には, 開頭によるシャント閉鎖をただちに行えるようにスタンバイしておくことが必要である.isolated galenic DAVF のような深部の DAVF は, すべてが血管内治療により根治できるわけではないが, 本例のように全身性血管病変を有し, 開頭術のハイリスクな例などに関しては, 血管内治療は有用な治療オプションと考えられる. 文献 1) Awad IA, Little JR, Akarawi WP, et al: Intracranial dural arteriovenous malformations: factors predisposing to an aggressive neurological course.
