We analyzed the effect of morphological features on angiographic outcome after coil embolization for ACoA aneurysms. We conducted a retrospective case review of 78 consecutive patients (35 men and 43 women) with ACoA aneurysms treated at our institution from September 2004 to October The patterns of A1 direction and aneurysm projection allowed the classification of ACoA aneurysms into four types: S-S type, A1 with superior direction and aneurysm with superior projection; S-I type, A1 with superior direction and aneurysm with inferior projection; I-I type, A1 with inferior direction and aneurysm with inferior projection; I-S type, A1 with inferior direction and aneurysm with superior projection. The percentage distribution of each type is as follows: S-S, 28.2%; I-I, 44.9%; S-I, 11.5%; and I-S, 15.4%. The I-I type is the most common type of aneurysm in this classification. Thirty-five patients were treated with endovascular coil embolization, and the average volume embolization ratios were as follows: S-S, 34.2%; I-I, 28.2%; S-I, 25.8%; and I-S, 26.6%. Complete occlusion (Raymond grade 1) was achieved at 90% in S-S, 35.7% in I-I, 42.9% in S-I, and 0% in I-S. The S-S and I-I types are likely to result in complete occlusion or neck remnant. The S-I and I-S types are likely to result in body filling. It is thought that coil embolization was likely to have resulted unfavorably in direction mismatch types (S- I, I-S) because the relation between A1 direction and aneurysm projection directly affects the intraoperative deliverability and stability of the microcatheter. In conclusion, this classification provides useful information for ACoA aneurysm treatment in a simple and immediate manner. 2 はじめに脳動脈瘤に対するクリッピング術, コイル塞栓術のいずれにおいても動脈瘤を安全かつ確実に処理するためには, 術者が動脈瘤およびその周辺血管の位置関係をイメージして手術に臨む必要がある 2). このために 3D 画像などによるオリエンテーションの確認が行われるが, それに先立っておおよその位置関係をイメージするために, 簡便な分類法があれば有用であると思われる. 前交通動脈瘤において, 発育方向別に破裂率や治療成績を検討した報告は散見されるが 3)7),A1 の向きと動脈瘤発育方向をもとにした検討は報告されていない. 本研究では, 手術に先立って母血管と動脈瘤のおおよその位置関係をイメージするために,A1 の向きと動脈瘤発育方向の関係から前交通動脈瘤を簡便に分類した. さらに, 各型におけるコイル塞栓術の治療成績を検討した. 対象と方法 2004 年 9 月から 2013 年 10 月までに当科で治療を行った前交通動脈瘤連続 78 例を対象とし, 診療録を分析して後ろ向きに検討した. これらの症例を脳血管撮影正面像をもとに 4 型に分類した. 治療法に関して, 開頭ネッククリッピング術かコイル塞栓術かの選択は, 症例ごとに治療チーム内で検討し決定した. コイル塞栓術は, 全例全身麻酔下に行い, ヘパリンの全身投与による抗凝固を行ったうえで施行した. 治療手技は原則的に simple technique, 7 Fr+4 Fr ガイディングカテーテルを用いた triple coaxial system で行った.3D-DSA を撮影し,working angle を決定したうえで, マイクロカテーテルを動脈瘤へ誘導し塞栓を行った. 使用したマイクロカテーテルは Excelsior SL-10 または Headway 17, 主なコイルは Target,GAL- AXY,Axium,ED coil であるが, その選択は症例ごと, 治療場面ごとに血管内治療チーム内で検討し決定した. 本研究の検討項目は, 各型の分類, 各型の頻度, コイル塞栓術施行例の angiographic outcome, 周術期合併症である. 結果全 78 症例の内訳は, 男性 35 例, 女性 43 例, 平均年齢 63.6 歳 (41-87 歳 ) であった. 破裂 67 例, 未破裂 11 例であった. コイル塞栓術を 35 例, 開頭クリッピング術を 38 例に行った. 保存治療を行った症例は 5 例であった. 1.A1 の向きと動脈瘤発育方向による前交通動脈の分類検査時または治療時における血管撮影正面像をもとに 4 型に分類した.S-S 型 :A1 が上向きで, 動脈瘤が A1 の走行 ( 軸 ) に対して上向きのもの.S-I 型 :A1 が上向きで, 動脈瘤は A1 の走行 ( 軸 ) に対して下向きのもの.I-I 型 :A1 が下向きで, 動脈瘤が A1 の走行 ( 軸 ) に対して下向きのもの.I-S 型 :A1 が下向きで, 動脈瘤は A1 の走行 ( 軸 ) に対して上向きのもの. 各型の代表的な症例の血管造影と, 各型における A1 の向き, および動脈瘤発育方向を矢印で示した図を示す (Fig. 1). この分類においては, A1 の向きは A1 起始部と終末部の位置関係のみで行い, A1 の弯曲 ( 上弯か下弯か ) は加味していない. 2. 各型の頻度 I-I 型が 35 例 (44.9%) と最も多く, 次いで S-S 型 22 例 (28.2%),I-S 型 12 例 (15.4%),S-I 型 9 例 (11.5%) であった. コイル塞栓術を施行した例では,I-I 型 14 例,S-S 型 10 例,S-I 例 7 例,I-S 型 4 例であった (Fig. 2). 3. コイル塞栓術施行例の angiographic outcome コイル塞栓術を施行した 35 例について, 各型に分類し, おのおの検討した. 各検討項目の結果を表に示す (Table 1). 年齢, 性別, 重症度については各型間で差はなかった. 有意差はないものの, 動脈瘤サイズ ( 最大径の平均値 ) は,I-I 型でやや大きい傾向であった. 1コイル塞栓術後の angiographic outcome について検討した. 平均体積塞栓率 (volume embolization ratio: VER) 9) は,S-S 型 34.2 %,I-I 型 28.2 %,S-I 型 25.8 %, I-S 型 26.6% であった. 2 Raymond 分類 8) で術直後の塞栓状態を評価すると, S-S 型および I-I 型では complete occlusion または neck remnant が多く,body filling で終わった症例はなかった. 一方,S-I 型および I-S 型では,body filling で終わった症例が多くみられた.S-S 型に比べると S-I 型では complete occlusion が得られにくい傾向であり (p=0.06), I-S 型では有意に complete occlusion が少なかった (p= 0.005)(Fig. 3). 4. コイル塞栓術の周術期合併症周術期合併症は 2 例で発生した. 術後再出血は I-S 型で 1 例発生したが,minor bleeding であり予後には影響しなかった. 虚血性合併症は S-I 型で 1 例発生したが, これも小梗塞であり予後には影響しなかった. これらとは別に, 無症候性であったが coil compaction をきたし再治療を要した症例が I-I 型で 1 例あった. 考察 A1 と動脈瘤の解剖学的関係をもとにした前交通動脈瘤の研究は, 過去に複数報告されている 1)4-6).Kasuya ら 4) は A1 と A2 のなす角度を分析し, 動脈瘤を有する群で A1 126 脳卒中の外科 43: 2015

と A2 のなす角度が急峻であったと報告している. 同様に Lin ら 6) は, 未破裂例に比較して破裂例で有意に A1 と A2 のなす角度が急峻であったと報告している.

の血管径比が大きいという結果であった.Matsukawa ら 7) の報告では, 前向き瘤で破裂率が高いとしている. コイル塞栓術の治療成績と関連した報告も散見される.

一方,Uemura ら 10) は, 上向き瘤で incomplete occlusion が多いとの結果を報告している.

D: I-S type: A1 with inferior direction and aneurysm with superior projection. と A2 のなす角度が急峻であったと報告している. 同様に Lin ら 6) は, 未破裂例に比較して破裂例で有意に A1 と A2 のなす角度が急峻であったと報告している.Flores ら 1) は,A1 と A2 の血管径およびその比率を分析した結果を報告しており, 前交通動脈瘤が存在する群で病変側 A1 径が有意に大きく,A1-A2 の血管径比が大きいという結果であった.Matsukawa ら 7) の報告では, 前向き瘤で破裂率が高いとしている. コイル塞栓術の治療成績と関連した報告も散見される.Gonzalez ら 3) は, 解剖学的要素とコイル塞栓術の成績の関連を調べており, 前向き瘤で complete occlusion が得られやすく, 後ろ向き瘤で再発しやすいと報告している. 一方,Uemura ら 10) は, 上向き瘤で incomplete occlusion が多いとの結果を報告している. また,Li ら 5) は A1 の発達の程度で分類し, 両側 A1 が発達している症例では前交通動脈を含めた塞栓を行い, 片側が hypoplasia または aplasia の症例では瘤内のみの塞栓を行うという治療方針を推奨している. 本研究では A1 の向きと動脈瘤発育方向の関係に注目して前交通動脈瘤の分類を提唱した. 本分類の特徴は, 血管撮影の正面像のみをもとに分類したということである. 中大脳動脈瘤を血管撮影の正面像のみで 3 型に分類した藤岡ら 2) の報告に着想を得て, 前交通動脈瘤に応用した. 血管 (Case) (44.9%) (28.2%) Conservative Clipping Coil 12 (15.4%) 9 (11.5%) I-I S-S I-S S-I (Type) Fig. 2 Distribution of the aneurysm types. I-I type is the most common type of aneurysm in this classification. 撮影の正面像のみで判定するため, 非常にシンプルであり, 煩雑な計測を必要とせず瞬時に分類することが可能である. また, 血管内治療のエキスパートでなくても母血管と動脈瘤の解剖学的位置関係を理解しやすい. 本分類で前交通動脈瘤を分類した結果, 得られた疫学的情報として, タイプ別では I-I 型が最も頻度が高く全体の 45% を占めていた. どのタイプが破裂しやすいかという点では, 有意差を認めなかった. S-I and I-S type are likely to result in body filling. 本シリーズは, 前交通動脈瘤を対象としているため小型瘤が多く, 各型とも VER は高い傾向であった. しかし, Raymond 分類による評価では, 方向一致タイプ (S-S 型および I-I 型 ) で結果がよく, 方向不一致タイプ (S-I 型および I-S 型 ) で結果が悪い傾向であった.A1 の向きと動脈瘤発育方向との関係が, マイクロカテーテル誘導の難易度や術中のカテーテルの安定性に直結しており, 方向不一致タイプではこれらの点において不利になるため結果が劣ったと考えられる. このように, 本分類によって塞栓術結果のおおよその予想が立つため, 治療戦略を計画する際の一助となると考えられる. 本シリーズの S-I 型に関しては,9 例中 7 例でコイル塞栓術が選択されていたが, 結果的に完全閉塞が難しかった. 治療選択の段階において, 年齢的要素, 全身状態などを考慮してコイルを選択した症例が多く含まれるが, この型において身体条件のよい症例では, クリッピング術を優先的に考慮すべきと考えられた. 本分類はあくまで初期分類を表しており, 治療手技そのものを行うものではない. 当然, 前後方向や左右方向の projection も治療上重要となるので, コイル塞栓術においては,3D-DSA を撮影し安全で確実な working angle で手技を行うべきである.A1 の弯曲の方向や程度もマイクロカテーテルの安定性に関与する可能性が考えられるが, より複雑になるため今回の分類を行ううえでは考慮しなかった. 診断機器の進歩により,3D-DSA,3D-CTA,volume rendering 法を用いた 3D-MRA など, 三次元画像で動脈瘤やその周辺の血管構築を任意の方向から描出できるようになった. クリッピング術およびコイル塞栓術のいずれにおいても, これらを参考に動脈瘤を術者自身がイメージして手術に臨むのは当然のことであるが, それ以前の段階で簡便な分類を行うことによりシンプルに整理することが可能となる. 本分類は前交通動脈瘤において, その一助になると考えられるので, 今後はこの分類に基づいて脳動脈瘤コイル塞栓術の成績を前向きに検討していく予定である. 結語 A1 の向きと動脈瘤発育方向との関係をもとにした前交通動脈瘤の簡便な分類法を提唱した. 本分類では,I-I 型が最も頻度が高いという結果であった. また, 方向不一致タイプ (S-I 型,I-S 型 ) ではコイル塞栓術による動脈瘤の完全閉塞は得られにくい傾向があった. 128 脳卒中の外科 43: 2015
