Both the posterior cerebral arteries (PCA) directly originate from the aneurysm. 血と診断し全身管理を開始したところ第 5 病日に意有さない wide-neck 形状を呈していた Fig. 4A B 識レベルの改善 JCS 30 と体動がみられるようになっステント留置に備えて術直前に胃管よりクロピドグレた第 16 病日に頭部 MRI CTA を施行したところル 300 mg およびアスピリン 300 mg の loading dose を散在性脳梗塞所見がみられたのみで有意な脳血管攣縮所注入し血小板凝集能の測定は行わなかった術中は見は認めなかった Hunt & Kosnik grade 3 と判断しヘパリン 3200 単位をボーラス投与した後ヘパリン第 20 病日に血管内治療を施行した単位 / 時の持続静注を開始した右大腿動脈血管内治療経過治療は全身麻酔下に施行した動脈瘤にシースイントロデューサー XEMEX, 5Fr, 25cm ゼは幅 mm 高さ 3.5 mm 径で明らかな neck をオンメディカルを挿入し co-axial method で Medikit 268 JNET Vol.7 No.4 September 2013

An: aneurysm, BA: basilar artery, PCA: posterior cerebral artery, SCA: superior cerebellar artery E F The final angiogram shows a nearly complete occlusion of the aneurysm using four coils, while preserving the patency of both the PCA. 4Fr JB2 メディキットを右椎骨動脈に誘導後 5Fr VRD mm を左 P1 から BA に留置した single Cerulean G メディキットに交換し右椎骨動脈 V2 stent assisted technique でコイル塞栓を試みたが右 P1 に留置した続いて左大腿動脈にシースイントロデュにコイルが逸脱するため Y ステントテクニックを行うーサー XEMEX, 6Fr, 25 cm ゼオンメディカルを挿べく Prowler Select Plus を 1 つ目のステントの cell を入し Chaperon 6Fr STR を左椎骨動脈 V2 に留置した通して対側の右 PCA に誘導し Enterprise VRD 4.5 まず 6Fr 側から CHIKAI 14 朝日インテック愛知 22 mm を右 P1 から BA に留置し Y ステントとしたを用いて Excelsior SL-10 を動脈瘤内に誘導留置した Fig. 4C あらかじめ動脈瘤内へ留置しておいた 6Fr 次に 6Fr 側から Prowler Select Plus と CHIKAI 14 を用側の Excelsior SL-10 を用いて jailing technique でコイルい左 PCA に Prowler Select Plus を誘導し Enterprise 塞栓を行うこととしたが瘤頚部から両側 SCA が分岐 JNET Vol.7 No.4 September
