1 青年期アスリートを対象としたメンタルヘルスの実態把握および 心理的援助へのニーズの解明 深町 花子 石井 香織 岡 浩一朗 抄 録 近年はアスリートのパフォーマンスの向上だけでなく, メンタルヘルス悪化の予防対策も求められているが (Lutkenhouse, 2007) 我が国では心理的援助へのニーズはほとんどが明らかにされておらず, 予防対策が十分であるとは言えない そこで本研究の目的は アスリートのメンタルヘルスの把握および心理的援助へのニーズを解明することとした 18 歳から 34 歳の参加者を電子メールで募集した 18 歳から 34 歳の選手 321 人がインターネット調査に回答した 調査尺度は オーストラリアで行われた以前の研究を参考に決定した (Gulliver et al., 2015) 参加者の平均年齢は 27.0 歳 (SD = 5.0) であり 平均競技年数は 9.6 年であった 抑うつ症状および全般性不安障害傾向において性差を示したが 摂食障害症状においては性差は見られなかった どれか 1 つの症状が基準値を超えている者は 205 名 (63.9%) と全体の 6 割を超える結果となった この割合は オーストラリアの先行研究での 46.4% の値よりも 23.8% 高い また 援助探索行動の平均点は 23.8 点であったが アメリカの大学生 354 名を対象とした研究 (Vogel et al., 2007) の対象者全体での平均得点 25.6(SD=5.3) 点と比較してやや低い値であった しかしながら 我が国の大学生の平均点 23.0 点 (SD=5.2) とはほぼ変わらない数値であった ( 植松ほか, 2012) アスリートも一般成人と同程度に心理的な問題について専門家に頼るという行動はとられていないと考えられる 以上より メンタルヘルスの症状を有している者が多く 援助探索行動もとっていないと考えられるため 我が国のアスリートのメンタルヘルス問題への取り組みは急務であると考えられる キーワード : 月経前症候群, 不安, 抑うつ, 援助探索行動 早稲田大学スポーツ科学研究科 埼玉県所沢市三ヶ島 -- 早稲田大学スポーツ科学学術院 埼玉県所沢市三ヶ島
2 ども 青少年スポーツの振興に関する研究The mental health problems and help-seeking behavior among Japanese athletes Hanako Fukamachi* Kaori Ishi** Koichiro Oka*** Abstract The prevention of mental illness among athletes has been emphasized by several recent research (Lutkenhouse, 2007). However, the help-seeking behavior of Japanese athletes is currently unclear, and there is a lack of preventative measures on this topic. Therefore, this study aimed to examine the mental health status of athletes in Japan, and to highlight the need for help-seeking behavior among Japanese athletes. Participants were recruited by . A total of 321 athletes aged years responded to the internet survey. Each survey took approximately 10 minutes to complete. Our survey items were based on the methods of a previous study conducted in Australia (Gulliver et al., 2015). The average age of participant was 27.0 years (SD = 5.0) and the average number of years of competition was 9.6 years. The results showed a sex difference in depressive symptoms and generalized anxiety disorder tendencies, but there was no sex difference in overall mental health or eating disorder symptoms. We found 132 participants (41.1%) experienced a serious psychological stress. This proportion was greater than the value of 46.4% reported by a previous help-seeking behavior of 23.8 points. While this value was lower than the average score of 25.6 (SD = 5.3) reported by a previous study among 子354 university students conducted in the USA(Vogel et al., 2007), it was similar with the average score 315 of 23.0 (SD = 5.23) reported in a sample of Japanese university students (Uematsu et al., 2012). 3 . はじめに 一般的な 34 歳以下の 4 分の 1 は 1 つ以上の精神疾患の臨床基準を満たすといわれている (Australian Bureau of Statistics, 2007) ほとんどのアスリートがこの年齢のカテゴリに該当し スポーツに関連する因子は精神衛生上の問題に影響を与える たとえば 試合で実力が発揮できないこと チーム内での人間関係 キャリア終了後への不安など 多様なスポーツに関連する因子がある しかしながら 我が国のアスリートのメンタルヘルスの現状については知られていない. 目的 したがって 我が国におけるアスリートを対象にした実態把握が不可欠であると考えられる また 近年はアスリートのパフォーマンスの向上だけでなく メンタルヘルス悪化の予防対策も求められているが (Lutkenhouse, 2007), 我が国では心理的援助へのニーズはほとんどが明らかにされておらず 予防対策が十分であるとは言えない そこで本研究の目的は アスリートのメンタルヘルスの把握および心理的援助へのニーズを解明することとする. 方法 従来の機縁的な調査であると 指導者等の近くで回答することに抵抗を示す対象者がいる可能性が考えられるため 本研究ではインターネット調査にて回答を求めた 調査会社 マイボイスコム株式会社 の登録モニター 歳に対して 電子メールにて募集を行った 募集時に 歳のアスリート 321 名に 10 分程度のインターネット調査への回答を求めた マイボイスコム株式会社の登録モニターのうち 年齢 歳の14 年制もしくは 6 年制大学の体育会運動部に所属している ( 大学院生やいわゆる同好会 サークルは除外 ) または2レジャー目的ではなくスポーツを実施し スポーツを生活の中心に据え 選手として試合に継続的に参加している のどちらかに当てはまる者をアスリートとして抽出した オーストラリアの先行研究にならい 調査測度を決定した (Gulliver et al., 2015) 精神疾患についてはいずれもカットオフ値を有する尺度を使用した 全般的なメンタルヘルスを測定するものとして Kessler 10 scale(furukawa et al., 2003) 抑うつ症状を測定するものとして Center for Epidemiologic Studies Depression Scale( 島ほか,1985) を使用した また 全般性不安障害を測定するものとして Generalised Anxiety Disorder 7 scale( 村松ほか,2010) 摂食障害を測定するものとして SCOFF questionnaire を使用した PMS や PMDD の判断基準として PMDD 評価尺度 ( 宮岡ほか,2009) を使用した また 心理的援助への態度を尋ねる尺度として Attitude toward seeking professional psychological help( 植松ほか, 2013) を使用した その他の基本データとして 性 年齢 競技種目 チームでの地位 競技レベル 競技年数 居住環境 怪我の有無などについても回答を求めた 怪我やチームでの地位 ( 例 : レギュラー 準レギュラー ) は抑うつ症状に関連することが先行研究で明らかになっているため 本研究ではその点も合わせてデータを収集した. 結果及び考察.. 対象者の特徴と性差 対象者の平均年齢は 27.0 歳 (SD=5.0) であり 競技年数は平均 9.6 年であった 何らかのチームに所属している 266 名のうち チームでレギュラーとして活動している者は 163 名 準レギュラーが 59 名 レギュラーでない者が 44 名であった 競技レベルは全国大会出場経験のある者が130 名と40.5% 含まれる対象者集団である 1 か月以内に試合を休まなければならない程度の怪我をした人数は 71 名 (22.1%) であった 平均値の差を見てみると 抑うつ症状および全般性不安障害傾向では性差が見られたが 全般的なメンタルヘルス 摂食障害では性差は見られなかった 心理的な困難に直面した際の援助探索行動についても 全体の平均点は 23.8 点 (SD=3.3) であり 性差は見られなかった.. メンタルヘルスの深刻度 オーストラリアでの研究のカットオフ値を参考に 基準を超え臨床レベルの症状を有する可能性のある者の割合を示す 心理的ストレスの深刻な者は 132 名 (41.1%) であった また 抑うつ症状および全般性不安障害傾向ではそれぞれ 170 名 (53.0%) と 40 名 (12.5%) であった この 2 つは性差が見られ 女性の方が基準値以上の者の割合が多かった (47.8% vs 62.1%,8.3%vs19.8%) 一方で女性の有病率が高いとされている摂食障害では 男性が 33.2% 女性が 42.2% と性差は見られなかった (t(319)=1.54, n.s.) 女性のみを対象とした PMDD PMS の判定基準 (Steiner et al., 2003) に照らしてみると 116 名の女性中 PMDD の者が 11 名 中等症の PMS が 19 名含まれていた どれか 1つの症状が基準値を超えている者は 205 名と 316
4 ども 青少年スポーツの振興に関する研究1 つ以上のメンタルヘルスの問題を抱えている者は 205 名と全体の 6 割を超えていた これはオーストラリアの先行研究の 46.4% より多い このため 我が国のアスリートのメンタルヘルスへの取り組みは急務であると考えられる また 抑うつ症状および全般性不安障害傾向では女性の状態がより深刻であり 基準値を超えた者も多かった PMDD および PMS の問題を抱える者も女性全体の 4 分の 1 に上ったことから 女性アスリートに特化した支援策が求められる 摂食障害傾向の得点およびカットオフ値を超えた人数で差が見られなかった 近年男性の身体像障害が増加し (Edwards and Launder, 2000) 一般男性の約 50% が体格を変えたいと述べている (Cohane and Pope, 2001) ことから 本研究の対象となった男性アスリートにも サポートが必要なアスリートが含まれる可能性がある.. 援助探索行動 援助探索行動の平均点は 23.8 点であったが アメリカの大学生 354 名を対象とした研究 (Vogel et al., 2007) の対象者全体での平均得点 25.6(SD=5.3) 点と比較してやや低い値であった しかしながら 我が国の大学生の平均点 23.0 点 (SD=5.23) とはほぼ変わらない数値であった ( 植松ほか,2012) アスリートも一般成人と同程度に心理的な問題について専門家に頼るという行動はとられていないと考えられる 本研究は従来の機縁募集ではなく インターネット上で対象者を抽出し アスリートのメンタルヘルスについて回答を求めた しかしながら モニター登録という有意抽出のためサンプルバイアスが生じている可能性がある. まとめ 1 つ以上のメンタルヘルスの問題を抱えている者は 205 名と全体の 6 割を超えていた オーストラリアの先行研究の 46.4% よりはるかに多い (Gulliver et al., 2015) ため 我が国のアスリートのメンタルヘルスへの取り組みは急務であると言える また 抑うつ症状および全般性不安障害傾向は男性と比較して女性の方がより深刻であった PMDD および PMS の問題を抱える者も女性全体の 4 分の 1 見られたことから 女性アスリートに特化した支援策が求められる可能性がある 今後は他国のアスリートとの比較だけではなく 我が国の一般成人と値を比較し アスリートに特に深刻 だと思われる心理的問題について明らかにしていく 参考文献 Australian Bureau of Statistics National survey of mental health and wellbeing: summary of results.
