3 第24巻 図1 645 第4号,1985年 koilocytotic (Pap.染 cell,atypiaの 強 い 細 胞 も認 め られ る 色, 400) 図3 koilocytotic atypiaを 示 すflat condylomaとdysplasiaの (H-E染 図2 keratosisを (Pap.染 示 す 細 胞.細 合 併 色, 100) 胞 質 は オ レン ジ好 性 を示 す 色, 400) cell hyperplasia,rete pegsの 延 長,acanthosis,koilocy- tosis,parakeratosis,dyskeratosis,multinucleationな ど に よ っ て 定 義 づ け られ て い る17,18,22). 組 織 形 態 と し て は 乳 頭 状 構 造 を 取 るclassical (warty type),flat type,spiked typeの type ほ か,inverted 図4 type(endophytic typeは り,組 ら れ,さ type)に 分 類 さ れ る12,19).特 コ ル ポ 診 上,dysplasia,CINと 織 学 的 に もdysplasia,CINと ら にHPV感 にflat golden の 共 存,移 行が認 め 染 と発癌 との 関連 が 指 摘 され る に 層 の 細 胞 が 色, 100) で も重 要 な 情 報 で あ る. 今 回,調 査 した 総 数 は14,445名 1.6%と dyskeratosis,multinucleationな ど が あ げ られ21,24),特 にkoilocytosisの 染 を示 唆 す る所 見 と し て 重 要 で あ る. 高 い.千 く,こ 染 を 示 す 細 胞 所 見 が 認 め られ る と い わ れ て い る が16,17), 者 のHPV感 0.46%と 染 と発 癌 と の 関 係 を 明 ら か に す る う え 以下 で 以上 に限 ら 以下 の受 診 者 が 少 な の 値 が 正 確 な 出 現 率 を 表 して い る と は 必 ず し も い 本 邦 に お け る 出 現 率 に 関 す る 報 告 は み ら れ な い.細 染 の状 況 お よび 推 移 の 把 握 の 代 別 で は30歳 葉 県 で は 集 検 受 診 者 は30歳 か し,性 風 俗 の 欧 米 化 と と も にHPV感 増 加 は 明 ら か で あ り,今 胞 診 で そ の う ち26例(0.18 認 め た.年 れ て い る と こ ろ が 多 い た め,30歳 え な い.し 欧 米 で は 細 胞 診 ス ク リ ー ニ ン グ 上1 10%にHPV感 み な らず,HPV感 色 陽 性 例,表 染 色 さ れ る(PAP染 %)にkoilocytosisを 細 胞 学 的 特 徴 と し て はkoilocytosis,parakeratosis, に お け る 出 現 率 はHPV感 brownに の鑑 別 が困 難 で あ 至 り4,5,9,16,22,24),注 目 さ れ る よ う に な っ た. 出 現 はHPV感 flat condylomaのpap染 染の 後 の デ ー タの 集 積 と と も に 若 年 染 の 実 体 も 明 ら か とな ろ う.60歳 比 較 約 高 い.こ れ に 対 し,50歳 以上は 代 は0.073%と 低 く な っ て い る が こ れ ら の 原 因 に つ い て は 明 ら か で な い. 臨 床 症 状 と して は 不 正 出 血3例,帯 下4例, 痒 感1
4 26 cases (0.18%) of koilocytotic atypia were cytolo- gically detected out of 14,445 women mass-screening for uterine cancer. They belong to 16 class II b and 10 class ilia. 10 of them were examined by smear, biopsy and colposcopy. Some of them were cytochemically subjected to peroxidase antiperoxidase technique (PAP method) for immunocytochemical detection of capsid antigen of papilloma virus. Comparative occurrences by age are 20th. 1.6%, 30 th. 0.18%, 40th. 0.19%, 50th %, and over 60th. 0.46%. by repeated smear for further investigation, suggesting inaccuracy of smear for the diagnosis of HPV infection. 5 out of 10 exhibited the finding of white epithelia by colposcopy. They consist of 2 mild dysplasia and 3 squamous metaplasia. Histological diagnosis of 10 cases are 8 squamous metaplasia, 1 mild dysplasia and 1 moderate dysplasia. 7 cases of 10, proved histological koilocytosis indicated positive in biopsy specimen. 3 (43%) out of 7, koilocytosis Only 4 cases (40%) demonstrated koilocytotic atypia indicated positive reaction by PAP method.
