A15 year old boy was admitted with 1 month historyof headache.ct studies and MR images on admission revealed a pineal region tumor with hydrochephalus.rightvertebral angiography revealed a marked tumor stain fedby duralbranchesof the vertebral artery and the medial and Iateral posterior choroidal arteries.leftvertebral angiography revealed a venous angioma in the leftcerebellar hemispherewhich drainedintothe superior sagittal sinus right transverse sinus and sigmoid sinus The initial titerof HCG (HCG 一 β)was 132mlU /m1 (15ng/ml )in the serum and 29mIU /ml (9.5ng /ml )in the CSF.Other tumor markers 一 includingan α fetoprotein(afp ) and cartinoembryonic antigen (CEA ) were negative.tumor removal was performed using an infratentorial supracer. ebellar approach after placement of a V P shunt.histopathologically the tumor was characterized by intense desmoplasia.the two cell pattern typical fora germinoma was not observed and placental alkaline phosphatase (PLAP ) staining was negative in the tumor cells.but the presence of multinucleated giant cells which were positivefor HCG and PAS positive granules in the cytoplasm of the tumor ce11 suggested that the tumor was a germinoma with a syncytiotrophoblastic giant cel1 (STGC ). No evidence for any other type of germ cell tumor was apparent.the mild increaseof HCG inthe serum and the CSFand the absence of the tumor followingradiation therapy (40Gy)and 2 courses of chemotherapy (carboplatin and etoposide ) supported the dlagnosis.the intense desmoplasia of thistumor was considered to be caused by growth of meningeal fibroblastsitlresponse to tumor invasionof leptomeningealtissue. In a very rare case of desmoplasticgerminoma with STGC the desmoplasticreaction did not influence the effectiveness of the radio chemotherapeutic regimen used for a germinoma with STGC. 2 STGC ) とよばれている.germinoma は two cell pat tern を示す組織像が特徴的で著明な膠原線維の増生を示すことは稀でありわれわれの渉猟し得た範囲では 1 例が報告されているにすぎない 6 }. 今回われわれは著明な desmoplasia を伴う germinoma with STGC と考えられた松果体部腫瘍に対して放射線化学療法を行い良好な結果が得られたので報告する. 症例患者 :15 歳男性主訴 : 頭痛家族歴既往歴 : 特記すべきものなし. 現病歴 :1996 年 3 月初旬より嘔気嘔吐を伴う頭痛が出現した. 3 月中旬より眼のかすみや複視が併発したため 3 月 22 日近医を受診し頭部 CT を施行された. 4 月 1 日松果体部腫瘍および水頭症が認められたため当科に紹介入院となった. 入院時現症 : 意識は清明で両側軽度のうっ血乳頭女性化乳房を認めた. その他の神経学的異常所見は認めなかった. 神経放射線学的検査 : 頭部単純 X 線写真で松果体部に軽度の石灰化を認めた. 単純 CT で松果体部に淡い high densitymass を認め造影 CT で同部は均一に造影された. 同時に閉塞性水頭症を認めた. 頭部 MRI で腫瘍は T1 強調像でほぼ iso intensity(fig.la) T2 強調像で iso high の mixed intensity を示し (Fig. 1b )Gd DTPA で均一に増強された (Fig.lcd). また右大脳基底核に T1 強調像で iso low intensity (Fig.1a) T2 強調像で high 一 intensity を示し (Fig. 1b) Gd DTPA で増強されるわずかな病変も認められた (Fig.1c). 脊髄 MRI で髄腔内播種を示す所見は認めなかっ硬膜枝認めたた. 脳血管撮影では右椎骨動脈撮影で椎骨動脈内側外側後脈絡叢動脈が流入する腫瘍陰影を (Fig. 箆 b ). また左椎骨動脈撮影にて左小脳半球に上矢状静脈洞右横静脈洞 S 状静脈洞より構成される vernous angioma を認めた (Fig.2c). 腫瘍マーカー : 3 月 27 日採取の血清中 HCG 132 miu /m HCG 一 β 15 ng /ml 4 月 15 日採取の髄液中 HCG 29 mlu /ml HCG 一 β9.5 ng /rn1 と高値を示した. α 一 fetoprotein(afp ) cartinoembryonic antigen (CEA } は血清髄液中ともに正常範囲内であった. 入院後経過 : まず水頭症に対し 4 月 4 日に脳室腹腔短絡術を施行した. 手術時に採取した髄液細胞診は ClassII (abnormal cells) で明らかな腫瘍細胞は認めなかった. 全身状態の改善を待って 4 月 18 目に in 一 Fig.1 Magneticre80nance image 騒 after the V P8h 皿 t and before surgical rejection a :AT1 weigkted MR imageshows an isointensitymass in the pineal region and an iso low intensitylesionin the right basal ganglia. b :AT2 weighted MR irnage shows a mixed intensity mass in the p 三 neal region and a hyper intensitylesionin the right basa 旦 ganglia. c d :An axia1 (c) and a sagitta1 (d) view.marked and diffus enhance ment of the pineai tumor was observ ed on a T1 weighted image the inj tion of Gd DTPA.The axial view showed an enhanced lesion in the right basal ganglia. 1 fratentorialsupracerebellar approach (Stein s approach ) にて腫瘍摘出術を施行した. 松果体部からテント縁に沿って淡赤色で弾性のある線維性の腫瘍を認めテントとの癒着も認められた. また腫瘍の深部には暗赤色でやや硬い部分を認めガレン静脈が埋没していた.

3 1 fratentorialsupracerebellar approach (Stein s approach ) にて腫瘍摘出術を施行した. 松果体部からテント縁に沿って淡赤色で弾性のある線維性の腫瘍を認めテントとの癒着も認められた. また腫瘍の深部には暗赤色でやや硬い部分を認めガレン静脈が埋没していた. 左小脳半球に vernous a ioma があり左側小脳の圧排が十分できなかったこと術中凍結標本で ger minoma の可能性が強く髄膜腫が否定されたため部分摘出にとどめた. 術後 MRI で松果体部腫瘍は右側部分のみ摘出されているのが確認された (Fig.3a). また右大脳基底核の病変は消失しており germinoma とは考えにくくその後の生検は実施しなかった (Fig.3a). 摘理組織学的所見 : 腫瘍組織には結合織が著明に増生しており (Hg.4a) その間に小リンパ球漫潤と類円形核をもつやや大型の細胞を散在性に認めた ( 恥.4b). この細胞の胞体には PAS 染色陽性顆粒を認めたが (Fig 4c) 典型的な two cell pattern は呈しておらず placental alkaline phosphatase (PLAP ) も陰性で germinoma と組織診断を確定するのは困難であった. しかし一部で胞体の広い多核細胞がみられこれらは HCG 一 β 陽性であることから STGC と考えられた (Fig. ca). その他の germ cel!tumor と考えられる組織像は認めなかった. 術後経過 : 5 月 7 日 6 月 3 日まで全脳照射 22Gy ヨ糾 4隔麟. ダ夢聴急尸. f. 〆 Fig.2 Vertebral angiograms a b :An antero posterior(a )and a latera1(b)views of right vertebral arteriography revealed marked tumor staining (arrow )supplied by the duralbranches of the vertebral artery and the medial and lateral posterior choroidal arteries. c :The venous phase on an antero posterior view of leftvertebral angiography revealed a venous angioma (arrowhead )drained intothe superior sagittal sinus right transverse sinus and sigmoid sinus. The Japanese Congress of Neurological Neurologloal Surgeons 拡大局所照射 18Gy の放射線療法を施行し化学療法 (PE 療法 :carboplatin etoposide ) を 5 月 13 17 日および 6 月 25 29 日の 2 クール併用した.

4 The Japanese Congress of Neurological Neurologloal Surgeons 拡大局所照射 18Gy の放射線療法を施行し化学療法 (PE 療法 :carboplatin etoposide ) を 5 月日および 6 月日の 2 クール併用した. 1 クール目は day 1 に carboplatin 450 mg day 1 5 に etoposide IOOmg を静注投与したところ著明な骨髄抑制をきたし 19 日間の Granulocyte Colony Stimula tion Factor (G CSF) 75 μ g/day の投与を要した. 2 クール目は薬剤を減量し day 1 に carboplatin 300 mg day 1 5 に etoposide 60 mg 静注投与したが再度骨髄抑制がみられ G CSF 75 μ g/day の 10 日間の投与を要した. しかしながら他には合併症はなく 1 クール目終了後の 5 月下旬には血清 HCG および HCG 一 β は正常値となり放射線化学療法終了後の 7 月 17 日の頭部 MRI で腫瘍はほぼ消失したため (Fig.3b) 29 日独歩退院となった. 7 月ず正常生活を送っている. 考察 germ cell tumor は多彩な組織像を示し組織型によって治療法および予後が異なるため臨床像血清髄液中の腫瘍マーカー免疫組織化学的検索を含む病理組織所見などからその組織型を特定することが重要である. 腫瘍マーカーの一つである血清 HCG 値は chor iocarcinoma では U 以上の高値を示すことが多く geminoma with STGC malignant teratoma embryonal carcinoma などでは多くが 10001U 以下であるとされている S ). 本症例は若年男性であること HCG 値が上昇しているとはいえさほど高値ではないことなどから ge inoma with STGC 力撮も可能性が現在治療後 1 年が経過するが腫瘍の再発はみられ高いと考えられた. しかし脳血管撮影で明らかな腫痛韈鑛覊韈. 蠶難靉戀繕轢 ^ 灘靆靉撫繖犠跏麟嬲 w 黜纔鞨講靆鑞纛懸鍵鐸唾騰藤. 糶嬲蠶蠶蠶瀏慧灘簸鰻融膿曲騾灘鷙翻齦メ Jpn J Neurosurg vol.7no NII-Electronic N 工工一 Eleotronlo Llbrary Library

5 陰影を認め椎骨動脈硬膜枝からの血流供給があること germinoma が血管に富むことが稀であることや MRI 上天膜に接し著明に増強されることなどから画像上は髄膜腫も否定できなかった. 摘出組織では著明な desmoplasia が広範にみられ腫瘍細胞と思われるやや大型の細胞は PLAP 陰性でリンパ球の浸潤は認めるものの定型的な two cerl pattern はみられず組織学的に germinoma と確定し得なかった. しかし散在する多核巨細胞が HCG 陽性で STGC と考えられること腫瘍細胞がグリコーゲンを有していると考えられること他の germ cell tumor を示唆する病理学的所見がないこと Shinoda ら 7) の報告によれば PLAP 陽性を示す germinoma は 76.5 % で必ずしも PLAP 陽性とは限らないことこの部の germ cell tumor では germinoma が最も多く (41 72 %) 1 ) 5 ) 血清 HCG 値は choriocarcinoma の混在を疑わせるほどは高くないことなどから著明な desmoplasia を伴った gerrninoma with STGC と診断した. germinoma with STGC に対しては通常の ger minoma と同様に放射線治療が著効を示すが比較的早期に再発する症例もあり通常の germinoma に比べ予後不良との報告が多いため 9 川 )12) 初回治療に放射線療法と化学療法との併用が推奨されていれに準じて放射線療法と化学療法を併用しる. 本症例でもこほぼ完全な腫瘍の消失を認めた. このことは本腫瘍が ger minoma with STGC 以外の組織型を含まないこと desmoplastic な部分も縮小したことを示しており著明な desmoplasia がみられる症例においても放射線化学療法が有効であることが示された. germinoma に著明な desmoplasia を伴った組織像を示す症例の報告は非常に稀でわれわれの渉猟し得た限りでは扇一ら 6} の 1 例のみであるがその治療結果については言及されていない. 彼らの症例では腫瘍は two cell pattern を示し一部血管周囲に HCG 陽性の合胞細胞が認められ渡銀染色では間質の一部に好銀性の線維が認められたが大部分は膠原線維より成っていた.
