1 調査報告挿管を望まない患者に対する NPPV の限界場章宏 1, a, 蛯名正智 1, 櫻正明 2, 瀬尾太郎 1, 須賀将 1, 吉研 2, 有吉孝 1 Limitations of noninvasive positive pressure ventilation for do-not-intubate patients Akihiro Takaba 1, Masatomo Ebina 1, Masaaki Sakuraya 2, Ryutaro Seo 1, Masafumi Suga 1, Kenichi Yoshida 2, Koichi Ariyoshi 1 要旨 Noninvasive positive pressure ventilation(nppv) が気管挿管を望まない do-not-intubate(dni) 患者に対して使される頻度は増加しているが, 臨床的改善が乏しい場合に NPPV を継続することの妥当性に関するデータは乏しい今回,DNI 患者に NPPV を使し, 呼吸状態が改善しない場合の転帰について検討した 2011 年 4 から 2013 年 8 に, 急性呼吸不全に対して NPPV が 6 時間以上使された DNI 患者 19 例について後視的に検討した改善の定義を,NPPV 開始後 6 時間の時点で FIO2<0.6 で,SpO2 90% を維持可能, かつ来院時の呼吸数が 30/ 分未満の場合, または SpO2 90% を維持可能で来院時 30/ 分以上の頻呼吸があったが,6 時間後の呼吸数が 20% 以上減少した場合とし, それ以外を改善なしとした全体の平均年齢は 81 歳, 院内死亡率は 84% で, 改善群 11 例, 改善なし群 8 例であった存数中央値は改善群 19, 改善なし群 1 であり, 改善なし群では有意に存期間が短かった改善なし群のほとんどで,NPPV は死亡時まで継続されていた DNI 患者に NPPV を使し, 短期間で呼吸状態が改善しない症例にも NPPV が継続されていたが, 存期間は有意に短かったこのような患者では,NPPV が苦痛をめていないかに注意して, 継続の是を検討すべきである ( 救急医会誌. 2017; 28: 64-70) キーワード : 緩和ケア, 呼吸,noninvasive positive pressure ventilation,do-not-intubate,quality of life Noninvasive positive pressure ventilation (NPPV) is frequently used for patients who have declined endotracheal intubation. The groups of patients for whom NPPV is ineffective are not known. This was a retrospective observational study of all patients who received NPPV for acute respiratory failure between 2011 and The improved group was defined as follows: patients with a SpO2 that was maintained at 90% in 6 hours after NPPV was initiated, and patients with a respiratory rate that was less than 30/min on arrival, or that was more than 30/min but decreased by 20% within 6 hours. The cases that did not fulfill these criteria were defined as not improved. Of the 19 patients who had do-not-intubate orders, 11 were included in the improved group. The not improved group had a significantly shorter duration of survival. For most of the patients in the not improved group, NPPV was continued until the patient died. Even among do-not-intubate patients who received NPPV, the survival time was significantly shortened when respiratory parameters did not improve in the short term. In such cases, we propose that we should focus on the patient s comfort and determine whether NPPV should be continued with care. RR: respiratory rate 者に対して気管挿管の代替療法として使される頻度も増加しているとくに救急集中治療領域では, その開始の簡便さから齢者や終末期患者に対しても, 急性呼吸不全に対する救命処置として,NPPV が使されることも多い NPPV で呼吸困難が改善し,quality of life ( 以下 QOL) が向上する患者が存在するで,NPPV の圧迫感や閉塞感, 同調などによる不快感が強く, 不穏やせん妄, 拒否がじる患者も少なくない NPPV 施後に呼吸症状の改善がみられない場合や, 不快感が強く忍容性がない場合では, 呼吸困難の緩和に標を切り替え,NPPV を使してよいか慎重に判断すべきであるしかし, 臨床現場ではそのまま NPPV が継続されるケースが多いと思われる急性呼吸不全のため NPPV を使している DNI 患者で, 呼吸パラメータが改善しない場合における NPPV 継続の妥当性を検証するための研究をった対象と法 2011 年 4 から 2013 年 8 までの期間に,JA 広島総合病院 ICU および high care unit( 以下 HCU) で DNI 患者に NPPV を使した 21 例について後視的に検討した NPPV による改善 (improved) と改善なし(not improved) を以下のように定義した NPPV 開始後 6 時間の時点において,F I O 2 <0.6 で SpO 2 90% を維持可能で, 来院時の呼吸数が 30/ 分未満の場合, または F I O 2 <0.6 で SpO 2 90% を維持可能で, 来院時に 30/ 分以上の頻呼吸があったが,6 時間後の呼吸数が 20% 以上減少した場合を改善 (improved) とし, それ以外の場合を改善なし (not improved) とした (Fig. 1) 救急医会誌. 2017; 28:

5 場章宏, 他をいた研究をレビューし, 集中治療, 緩和ケア領域における NPPV の標設定に関するアプローチを提案している 5) その中で, 彼らは患者を挿管希望や治療標によって,3 群に分けている (Category 1-3) Category 1 は治療に制限を設けない群 (life support without preset limit) で, 気管挿管を回避しつつ酸素化換気を補助して呼吸困難を緩和することが標であり, 失敗した場合は気管挿管, 呼吸管理をう Category 2 は NPPV を治療の上限とした DNI 患者 (life support with preset limit: DNI) で, 気管挿管しないこと以外は Category 1 と同様だが, NPPV が失敗した場合は Category 3 である快適性を優先した緩和医療 (comfort measures only) に標を変更する Category 3 では患者の快適さを改善するときのみ NPPV の使が正当化され,NPPV を使するが不快な場合や, 患者に中を求められた場合, 意思疎通が不能になった場合には NPPV を中し, オピオイドなどその他の症状緩和を試みなければならないとされている NPPV で呼吸状態に改善がみられない場合, 本来ならば侵襲的呼吸に移する場であるが, DNI 患者でその選択肢はない病態が改善しない状況で DNI 患者の呼吸不全に対して NPPV を継続することには, 患者に層の不快感を与えている可能性や必要以上に延命して苦痛を引かせている可能性, 誤嚥のリスク, コストやマンパワーなど医療資源の問題が存在する上記のリスクや存期間を延させる必要性 ( 会わせたい家族がいるなど ) を鑑みたうえで,NPPV を継続するかどうか個別に検討すべきであるこのため, 開始時には NPPV の禁忌でないかを確認すること, 標と限界を明確化し, 短期間に再評価をうことが重要であるその結果 NPPV の不快感が呼吸困難を上回る場合や, 呼吸状態の改善がみられない場合は, 薬物的薬物的緩和療法を試することや,NPPV を中することの検討も必要であるまた,NPPV 継続中の判断は, 患者家族らの意思をふまえて, 複数の医師や, 多職種で検討すべきである再評価のタイミングや標変更を考慮する基準に関しての先研究はないが, 本研究での 6 時間以内に呼吸数や酸素化が改善しないという状況は,DNI 患者においては患者の安楽を最限にすることへ治療標を変更する安となる可能性がある NPPV 以外の呼吸困難への薬物的治療としては, モルヒネなどのオピオイドが般的で, 癌患者以外でもエビデンスは多い 6, 7) 近年, カニューレをいた流量酸素療法も普及しており,DNI 患者でも NPPV の代替として酸素化を改善できたという報 8) 告もあるため,NPPV で不快感が強い場合などには有かもしれない顔のクーリングなどの薬物的介の有性も報告されている NPPV 装着中に鎮痛薬鎮静薬を使することの科学的根拠は確していないが, 呼吸困難緩和, 忍容性の改善的によく使される 9) 本研究の症例でも半数以上で鎮静薬が使されているこれらの有効性をす研究もある 10-15), 鎮静薬と鎮痛薬の併で挿管または死亡が約 5 倍増えるなどの危険 16) 性も報告されている DNI 患者においても鎮静薬や鎮痛薬併時にはリスクにも分配慮し, 患者の快適性, 呼吸状態を注意深く守る必要がある NPPV 装着中の鎮静鎮痛による患者の症状緩和をアウトカムにしたデータは不しており, 今後の研究が期待される 1-4) これまでの DNI 患者に対する NPPV の報告にべ, 本研究では死亡率が 84.2% とい理由として,1 齢で ADL 不良な患者が多い,2 肺炎が多い, 3 重症度がい,4 本邦では予め DNI などの意思表があることは少なく, 重症で不可逆的な可能性がいと判断された場合に家族と医療者の決定で DNI とされることが多いため, 結果的に DNI 患者群には重症患者が集まりやすい,5NPPV が相対的禁忌にあたる, 侵襲的呼吸が必要な患者への代替として NPPV が開始された症例が含まれていることなどが挙げられる改善なし群で存期間が有意に短かった理由として, 新規の呼吸不全では NPPV による初期治療で改善がみられないことは予後不良の予 68 JJAAM. 2017; 28: 64-70

6 NPPV for DNI patients 測因であることが挙げられる 17) また, 本研究では意識障害など NPPV が相対的禁忌にあたる症例も含まれていることが交絡因となっている可能性もあるしかし, 実臨床では相対的禁忌でも診断や針を決定するまでの時間稼ぎのために NPPV を使することがあるので, それらの症例は除外していない本研究は後ろ向き研究であり,DNI の決定において倫理的な配慮が適切になされているかどうかを詳細に検討できていないしかし, 患者は 75 歳以上の齢者, 担癌患者, 慢性肺疾患や頸髄損傷などの基礎疾患がある患者のいずれかに属しており, 倫理的な判断はされていないと考える本研究の限界として,DNI 患者に対して NPPV を使した症例を後ろ向きに抽出していること, サンプルサイズが少ない, 患者の覚症状が評価されていないことが挙げられる NPPV の設定による影響も検討できていないまた, 本研究では再評価のタイミングを 6 時間とした般的に NPPV 開始後, 4-6 時間で標に達していない場合は気管挿管を考慮すべきとされているため 18), このように設定したが, 適切であるかどうかは不明である QOL や家族の満度などについても今後検討すべき課題である結語 DNI 患者に NPPV を使した際, 短期間で呼吸状態が改善しない場合は存期間が常に短かった NPPV は患者に不快感を与える可能性もあり, 呼吸困難と命予後のどちらも改善しないのであれば, 症状緩和に主眼を置いて NPPV を継続するかどうか慎重に検討すべきである本稿のすべての著者に利益相反はない献 1) Azoulay E, Kouatchet A, Jaber S, et al: Noninvasive mechanical ventilation in patients having declined tracheal intubation.
