250口 腔 腫 瘍20巻4号2008 柴 図5疾 原 孝 彦 ほか 患 別 にみ た症 例 数 が多 い 異 な る腫 瘍 で あ っ て も部 分 的 に 共 通 した 組織 像 と 悪 性 腫 瘍 とみ な され,明 細 胞 性 歯 原性 癌 と命 名 され た,(5) して 認 め られ る こ と もあ り,歯 原性 腫瘍 の 診 断 を よ り困 難 歯 原 性 癌 肉 腫 は分 類 か ら除外 さ れ た,(6)語 句 と亜 型 分類 の な もの に して い る まず,新 分類 の特 徴 に つ い て 述べ,次 変 更,な い で 今 回 の実 態 調 査 で判 明 した 点,そ して 諸 外 国 の報 告 と 対 比 して考 察 を行 う どが 挙 げ られ る 2005年 版 新WHO国 際 分類 で は,全 臓 器 に わ た っ て 臨 床 的 意 義 を重 要 視 す る観 点 か ら悪 性 腫 瘍 を先 ず 挙 げ,次 い 1,新 分 類 に つ い て で 良 性 腫 瘍 を列 記 して い る 歯 原 性 肉腫 は従 来 通 りで あ る WHOで は 歯 原 性 腫 瘍 を系 統 的 に 分 類 す る た め1971年 が,歯 原 性 癌 腫 はエ ナ メ ル上 皮 腫 の悪 性 型 が4つ の亜 型 に に 歯 原 性 腫 瘍 の 組 織 型 分 類 を 発 表 し た5,6) 分 か れ る な どの 変 更 が あ る7) と1992年 と くに後 者 の 分 類 は広 く用 い られ,歯 原性 腫 瘍 に 関連 した 研 究 発 表 も多 く認 め ら れ る が,い これ まで と同 様,歯 原 性 上 皮 と外 胚 葉 性 間葉 との 間 の誘 ま だ語 句 の 統 一 と分 類 導 現 象 の 有 無 を も と に,(1)純 上 皮 性 腫 瘍,(2)外 胚 葉 性 問葉 法 に 関 して の 論 争 は 絶 え な い の が 現 状 で あ る そ ん な 中, 組 織 の 誘 導 を伴 う上 皮 性 腫 瘍,(3)間 葉 性 腫 瘍 の3グ ル ー プ WHOは に 大 別 して い る 頻 度 の 高 い(1)グル ー プの エ ナ メ ル上 皮 腫 これ ら腫 瘍 の 定 義 と組 織 型 分 類 を2005年 で更新 した4 7) 今 ま で の もの と比 較 して 新 しい 分 類 に は6つ の は 充 実 型/多 大 き な変 更 点 が あ る (1)裏装 上 皮 が 錯 角 化 を示 す 歯 原性 角 型,そ 化 嚢 胞 は 歯 原 性 上 皮 か ら分 化 した 歯 原性 腫 瘍 と判 断 され, 嚢 胞 型,骨 外 型/周 辺 型,線 維 形 成(類 腱) して 単 嚢 胞 型 に新 た に分 類 さ れ た4.7) 歯 原 性 病 変 と は 直接 の 関連 は 認 め られ な い が, 骨 性 病 角 化 嚢 胞 性 歯 原 性 腫 瘍 と命 名 さ れ た(正 角 化 を示 す もの は 変 の 中 に 中 心 性 巨 細 胞 性 病 変,ケ 含 まれ て い な い),(2)腺 腫 様 歯 原性 腫 瘍(腺 様 歯 原 性 腫 瘍) 胞 な らび に 単 純 性 骨 嚢 胞 が 挙 げ ら れ て い る ま た, そ の は歯 原 性 外 胚 葉 性 組 織 を伴 わ な い 上 皮 性 腫 瘍 に分 類 さ れ 他 の腫 瘍 で は乳 児 の 黒 色 性 神 経 外 胚 葉 性 腫 瘍 が 取 り入 れ ル ビ ズ ム,脈 瘤 性 骨 嚢 た,(3)石 灰 化 歯 原 性 嚢 胞 は 良性 腫 瘍(石 灰 化 嚢 胞 性 歯 原性 ら れ て い る しか し,こ の 病 変 はWHOCIassification 腫 瘍,象 牙 質 形 成 性 幻 影 細 胞 腫 瘍)と 一 部悪 性 腫 瘍(歯 Tumoursの 原 性 幻 影 細 胞 癌)に 分 け られ た,(4)歯 原性 明細 胞 腫 は事 実 上 of 鼻 腔 お よ び副 鼻 腔 の 章 で 詳 し く述 べ られ て い る の で,歯 原性 腫 瘍 に分 類 さ れ る こ との妥 当性 につ い

These tumors were reclassified according to the new WHO classification revised in 2005 and the clinicopathological data were analyzed. The patients were 2582 (49.4%) males and 2649 (50.6%) females (male : female ratio, 1 : 1.03). Histologically, the tumors contained 1460 ameloblastomas, 1258 keratocystic odontogenic tumors and 1079 odontomas. These tumors constituted 74% of all the cases.
