(Patients and methods) Twenty-six patients with muscle invasive bladder cancer (T2-T4N0MO) were treated with concurrent chemoradiotherapy after transurethral resection of the tumor as much as possible beyond muscle layers. Chemotherapy was consisted of systemic administration of methoterexete (30mg/m2 dayl and day 22) and intraarterial infusion of cisplatin (70 mg/m2, day2 and day 23). The response was evaluated by TUR, urine cytology, CT and/or MRI 4 to 6 weeks after the treatment. (Results) Among 24 evaluable cases, pathological complete response was achieved in 13 cases (50%) and residual tumors were noted in 11 cases (ptl in 9 and pt2 in 2). During follow-up period up to 69. 8 months, Invasive recurrence was observed in 2 cases, superficial recurrence was noted in 5 patients and distant metastasis without evidence of local recurrence was noted in 4 cases. Overall bladder preservation rate was 92%. (Conclusions) The bladder preservation by radical TUR-Bt and chmeradiotherapy is a safe and effective treatment option for muscle invasive bladder cancer. 754 図1 膀胱 筋 層 の 生検 鉗 子 に よる切 除 生検 鉗 子 で 一層 つ つ切 除 を繰 り返 す こ とで, 穿 孔 を起 こ さず に, 脂 肪 層 を確 認 す る こ とが 出来 る. 図2 膀胱 周 囲 脂肪 層 の 切 除 筋 層 よ り外 に は っ き りと脂肪 層 を確 認 で き る. この よ うな所 見 が 得 られ る まで, 止 血 の ため の 電気 凝 固 は最 低 限 の使 用 と し, 標 本 の熱 変 性 を防止 す る. 過 観 察 期 間24カ 月(3.9 69.8), 中 央 値2.9カ 月で あ った. 認 め な い 症 例 で あ っ た. こ の4例 症 例 で, 治 療 前 のpTstageは, 経 過観 察 中, 表在 性 の再発 を繰 り返 す症 例 を5例 認 はす べ てGrade3の pt2が2例, 例 で あ り, 治 療 後 の 腫 瘍 はpT0が3例, pt3が2 ptlbが1例 で め た. 経 過 中に筋 層浸潤 癌 と して再発 した症例 が2例 あ っ た. 4例 中2例 で, うち1例 は筋 層浸潤 癌 の残存 してい た症例 で あっ 遠 隔 転 移 な しの 割 合 と, 治 療 開 始 か らの 疾 患 特 異 的 生 た. この2例 は確 認 され た時点 で膀胱 全摘 除術 を行 っ 存 率 を 図3, た. この た め最 終 的 な膀 胱温 存率 は26人 中24人, 92% 異 的2年 で あ った. 治 療後 の評価 で筋層浸 潤癌 残存 を認め た も が 癌 死, 2例 が 癌 あ り生 存 中 で あ る. 4に 示 す. Kaplan-Meier法 生 存 率 は, 91%で 有 害 事 象 はNCI に よ る疾 患 特 あ っ た(表2). toxicity criteria version 2.0に 従 っ う1例 は, 現 在 まで 明 らかな再発 を認 めてい ない. 遠 て 評 価 し た6). 致 命 的 な 有 害 事 象 は, 認 め な か っ た. 血 隔転 移 を認 めた の は4例(15%)で, 液 毒 性 はgrade2の 全 例局所 再発 は 白 血 球 減 少, 貧 血 が 各 々3例,
