To investigate the effect of rosuvastatin in different doses on urinary protein excretion and renal function parameters in patients treated for primary hyperlipidemia. Patients-Methods. In this study, 90 patients with primary hyperlipidemia were randomly assigned to rosuvastatin 10 (RSV10 group) or 20 mg/day (RSV20 group). Urinary creatinine and protein (total, albumin, immunoglobulin G, and a1-microglobulin) levels were measured at baseline and after 12 weeks of treatment in all patients. The protein-to-creatinine ratios were used to assess urinary protein excretions. Serum creatinine and estimated glomerular filtration rate (GFR) were used as markers of renal function. Results. A dose-dependent increase in the excretion of a1-microglobulin (17.6% in the RSV10 vs 34.9% in the RSV20 group) was observed. There was no effect on total protein, albumin and immunoglobulin G urinary excretions as well as on the fractional excretions of electrolytes and uric acid in both groups. Rosuvastatin 10mg/day did not affect renal function parameters, whereas a trend towards an increase in GFR (79.3 タ11.5 at baseline vs 82.9 タ13.9 ml/min/ 1.73m 2 post-treatment, p=0.08) was noted in the RSV20 group. Conclusions. Rosuvastatin produces a dose-dependent increase in a1-microglobulin urinary excretion implying a functional disturbance of the proximal tubules without adversely affecting renal function.
