We also discuss the characteristic spinal alignment of LFS patients. [Subjects and Methods] This study examined 18 LFS patients and 56 healthy adult persons who were measured in the standing posture, forward-bending posture (thoracic kyphosis, lumbar lordosis, sacral inclination and spine column inclination) and range of forward-bending. [Results] In LFS, the range of motion for forward-bending was decreased. In addition, thoracic kyphosis had a correlation with lumbar lordosis, and lumbar lordosis had a correlation with sacral inclination. In healthy subjects, sacral inclination had a correlation with thoracic kyphosis, and lumbar lordosis had a correlation with sacral inclination. In subjects with previous history of low back pain, lumbar lordosis only correlated with sacral inclinated. [Conclusion] We conclude that sacral inclination is more important for alignment of LFS. 2 66 理学療法科学第 24 巻 1 号 I. はじめに腰痛と脊柱アライメントの関係について, 腰椎椎間関節症 ( 以下,LFS) では, 腰椎伸展位や同一姿勢の保持により疼痛が生じ, 腰椎椎間板ヘルニア ( 以下,LDH) では前屈姿勢で疼痛が生じるという特徴がある また脊柱管狭窄症では体幹前屈位で症状が軽減するなど, 姿勢と腰痛は密接な関係にある また症状が進行すると, 疾患によっては腰椎の前彎が減少する, あるいは過前彎になるなど腰椎のアライメントに変化が生ずると報告されている このような疾患特有のアライメントの発生に関しては, 疼痛や筋力低下, 髄節性の可動域制限などの結果, 疼痛からの逃避姿勢や筋緊張のアンバランス, 他髄節での代償などにより発生すると考えられている 3) Jackson は, 腰部 X 線像から腰痛患者と健常者の腰椎前彎角, 胸椎後彎角,C7 Plumb line, 仙骨傾斜角を比較し, 腰痛患者では腰椎前彎角が有意に減少し, 仙骨はより垂直位であったと報告した 4) 一方, Norton は腰痛患者と健常者の腰椎前彎角を比較した結果, 有意差はみられないが腰痛の種類と腰椎前彎角に関係を認めたと報告している 5) このように胸椎や腰椎のアライメントと腰痛の関係についての報告は散見され 6-9), 腰痛を脊柱のアライメントから捉えることの重要性がうかがえる 腰痛をきたす疾患には様々あり, 痛みの原因も疾患によって異なっている そのため, 疾患特有の脊柱アライメントを捉えることは, 疼痛や症状の改善あるいは予防としての初期治療に役立つものと考える そこで今回は, 疾患の特性をみるために, 健常者と医学的な治療をしたLFS 患者の脊柱アライメントを比較した また健常者と患者の移行状況であろう腰痛既往者とも比較し,LFS の特徴となる脊柱のアライメントについて検討した II. 対象および方法 1. 対象本研究の趣旨を理解し同意を得た健常成人 56 名 ( 男性 24 名, 女性 32 名, 平均年齢 24.3 ±4.5 歳,19 35 歳 ) と LFS 患者 18 名 ( 男性 5 名, 女性 13 名, 平均年齢 36.8 ± 14.5 歳,19 56 歳 : 以下,LFS 群 ) の74 名を対象とした 健常成人とは, 測定日以前 3ヶ月間に腰痛の経験がない者とした このうち, 過去に腰痛経験が全くない者は 35 名 ( 以下, 健常群 ), 腰痛の既往のある者は 21 名であった 腰痛既往者 21 名の内訳は, 腰椎の伸展時痛 11 名 ( 以下, 伸展時痛群 ), 屈曲時痛 9 名 ( 以下, 屈曲時痛群 ), 安静時痛 1 名であった LFS 群は, 神経根症状を認めず, 腰椎伸展時に疼痛があり,X 線や MRI の画像所見から LFS と診断を受けた外来患者とした なお脊柱アライメントに影響を与えると考えられる疾患や手術の既往がある者は除外した 2. 方法矢状面での立位姿勢, および立位体幹前屈姿勢 ( 以下, 前屈姿勢 ) の胸椎後彎角 ( 以下 TKA), 腰椎前彎角 ( 以下,LLA), 仙骨傾斜角 ( 以下,SIA), ( 以下,SCI) を測定した 立位姿勢の条件は, 足を肩幅に開き視線を目の高さに合わせること以外は指定せず, 被験者個々が普段行っている安楽立位姿勢とした 前屈姿勢は, 膝関節伸展位での体幹の最大前屈位とした その際, 頭部と上肢は重力に抗さず下垂位とした なおLFS 群の測定は, 初診時の治療前に行った 脊柱アライメントの測定には,Index 社製スパイナルマウスを使用した 測定方法はスパイナルマウスの測定条件に従い,TKA はTh1 からTh12,LLA はTh12 から S1 までの隣接する椎体が成す角の合計値とし, 後彎角を正, 前彎角を負で表記した SIA は,S1 から S3 までの仙骨よって描かれる背部表面の輪郭線と垂線の成す角とし, 前傾を正, 後傾を負で表記した またSCI はC7 とS3 を結ぶ線と垂線の成す角とし, 前傾を正, 後傾を負で表記した 測定により得られた立位姿勢と前屈姿勢の各部位の角度から立位体幹前屈可動域 ( 以下, 可動域 ) を算出した 統計学的分析には, 各群間の比較には一元配置分散分析および多重比較検定 (Scheffe 法 ) を用いた 群内での胸椎, 腰椎, 仙骨の関係についてはピアソンの相関係数を使用した 有意水準はともに5% 未満とした III. 結果立位姿勢では SIA のみに有意差が認められ, 多重比較検定の結果, 健常群の14.2 ± 6.1 度に対して,LFS 群は8.2 ±4.5 度と有意に垂直位を呈していた (p <0.0 SCI については, 健常群と屈曲時痛群は前傾, 伸展時痛群とLFS 群では後傾している傾向にあった 前屈姿勢でも SIA のみ有意差が認められ, 多重比較検定の結果,LFS 群は44.2 ±20.5 度と, 健常群の58.7 ± 11.2 度よりも有意に小さく (p <0.0, 伸展時痛群の62 ±16.9 度よりも有意に低値を示した (p <0.0 可動域は腰椎部と仙骨に有意差を認め, 多重比較検

4 68 理学療法科学第 24 巻 1 号 表 4 屈曲時痛群の胸椎, 腰椎, 仙骨の関係 ( 相関係数 ) 立位姿勢 前屈姿勢 可動域 TKA-LLA # 胸椎 腰椎 TKA-SIA 0.666* 0.784* 胸椎 仙骨 TKA-SCI 胸椎 脊柱傾斜 LLA-SIA # 腰椎 仙骨 LLA-SCI 腰椎 脊柱傾斜 # SIA-SCI # 仙骨 脊柱傾斜 表 5 LFS 群の胸椎, 腰椎, 仙骨の関係 ( 相関係数 ) 立位姿勢 前屈姿勢 可動域 TKA-LLA # 胸椎 腰椎 TKA-SIA 胸椎 仙骨 TKA-SCI * 胸椎 脊柱傾斜 LLA-SIA 0.585* 腰椎 仙骨 LLA-SCI # 腰椎 脊柱傾斜 SIA-SCI # 仙骨 脊柱傾斜 # p<0.05) を認めた LLA とSCI との間には正の相関 (r = 0.611,p<0.0 を認めた つまり,TKA が大きい者は LLA も大きく, 仙骨はより前傾位となることが示された また LLA が大きい者ほど脊柱は後傾位となる傾向にあった ( 表 5) IV. 考 金村らは, 日本人の成人男女 160 名の脊柱矢状面 X 線像から日本人の適正な脊柱矢状面彎曲角度を定義している 10) しかし, これは平均的日本人の脊柱彎曲であり, この範囲から逸脱した者が腰痛を生じるとは限らない アライメントは年齢や性別, 体型によっても異なり, 外観は腰痛患者と同様のアライメントをしていても腰痛のない者や, 腰痛が消失したにもかかわらずアライメントに変化が見られない者も存在する このようなことからアライメントと腰痛について検討する場合, 正常姿勢や良姿勢を論じて比較するよりも, 疾患に特徴的なアライメントを捉えることの方が重要であると考え, 我々は LFS 患者を対象に, 健常者や既往者のアライメントと比較し, 疼痛の軽減や再発予防につながるアライメントについて検討した 察 屈曲時痛群の立位姿勢においては, 健常群と比較し, TKA,LLA,SIA は全て小さい傾向にあった 屈曲時に腰痛を生ずる代表的な疾患としては LDH があげられ, 屈曲時痛群のアライメントはLDH 患者と類似したアライメントであった この群は医学的な診療を受けていないため, 疾患を特定することはできないが, 屈曲時痛群内にはLDHの既往者が含まれていたことが予測される これに対し, 体幹の伸展により腰痛が誘発される代表的な疾患としてはLFS が挙げられる 1, そこで, まずは腰痛の誘因が LFS 群と同様であろう伸展時痛群を既往者とし, 健常群, 伸展時痛群,LFS 群の脊柱アライメントを比較した Adams らは椎間板腔が減少した腰椎を前彎位で荷重した場合, 荷重の 16% を椎間関節で分担したのに対して, 後彎位では0% であったと報告した 1 また Yang らは, 腰椎伸展により下関節突起は下位の椎弓に接触し, さらに関節包上部に大きな張力が加わると報告した 1 このように腰椎前彎の増強は椎間関節へ負荷を与え, 椎間関節性の疼痛を誘発すると考えられるが, 今回の結果では各群間でLLA に差が認められなかった しかし,LFS 群の仙骨は健常群と比較して有意に垂直位を呈するのに対し,LLA は健常群と同様であることから, 体幹は健常群よりも全体的に後傾することになる その結果, 重心線は腰椎の後方寄りを通ることとなり腰椎椎間関節への負荷は増強すると考えられる 各群内における胸椎部, 腰椎部, 仙骨の関係性をみると, 健常群はSIA とTKA,SIA とLLA にそれぞれ有意な相関関係を示したのに対して,LFS 群はSIA と LLA, LLA と TKA にのみ相関関係を示し,SIA と TKA には相関関係は認められなかった このことから健常群では胸椎や腰椎のアライメント変化を仙骨の傾斜角でコントロールしているのに対し,LFS 群では胸椎のアライメント変化を腰椎が, 腰椎のアライメント変化を仙骨が代償していると考えられた Mayer ら 13) は, 椎間関節症では腰椎各髄節の屈曲方向への可動性が減少していると報告している 本研究でも明らかな統計学的有意差はないものの,LFS 群は他の2 群と比べて各部位の可動性が少ない傾向にあり, 特に仙骨傾斜可動域は伸展時痛群より有意に減少していた つまり, このような少ない可動域の中で安定した立位姿勢をとるためには, 隣接部位が互いに代償しあう必要があり, また各部位の角度変化で代償できない範囲は体幹を後傾させることによって立位を保持していると考えられる 一方伸展時痛群では SIA と LLA のみに相関関係が認められ, LFS 群に認められたようなTKA とLLA との間の相関関
