2 J 日本医真菌学会雑誌第 巻第 号平成 年 Table.Numbers of patients with dermatomycoses in each clinic 疫学調査協力施設 新患総数 白癬 カンジダ マラセチア 深在性 笠井皮膚科笠井達也 ( 宮城県多賀城 ) Kasai Dermatology Clinic(KaDC), 8 () まるやま皮膚科クリニック丸山隆児 ( 東京 ) Maruyama Dermatology Clinic(MRDC), () 帝京大学溝口病院皮膚科清佳浩 ( 川崎 ) Teikyo University Mizonokuchi Hospital(TUMH), 8 () 新潟県済生会三条病院奥山長三郎 ( 新潟 ) Niigata Saiseikai Sanjo Hospital(NSSH), () 揖斐総合病院皮膚科藤広満智子 ( 岐阜 ) Ibi Kosei General Hospital(IKGH), () 金沢医科大学皮膚科望月隆 ( 金沢 ) Kanazawa Medical University Hospital(KMUH), () 大阪市立大学小林裕美 ( 大阪 ) Osaka City University Hospital(OCUH), 88 () 鳥取大学医学部皮膚病態学山元修 ( 鳥取 ) Tottori University Hospital(TUH), 8 スポロ 例 山口大学武藤正彦 ( 山口 ) Yamaguchi University Hospital(YUH), () 産業医科大学皮膚科小林美和 ( 北九州 ) University of Occupational and Environmental Health Japan(UOEHJ), () 楠原皮膚科楠原正洋 ( 福岡 ) Kusuhara Dermatology Clinic(KuDC), () 琉球大学皮膚科上里博 ( 沖縄 ) Hospital, University of the Ryukyus(HUR),8 総計,,8 8 成績, 菌種別頻度を調査した. また, 爪白癬については培養陽性率が低いことから, 今回は可能な限り検体を帝京大学に送ってもらい, 帝京大学医真菌研究センターで RTPCR 法を用いて同定した. なお, 施設の中には培養が実施できない施設や, 全例での培養がなされなかった施設も含まれるが, 地域による疾患の頻度の違いを調査することに主眼を置いたためである. 結果は, 可能な限りexcel の表にしてもらい集計の簡略化を図った. 結果を見ていただくと分かるが, 集計数が合致しない例は少数含まれている. 筆者の入力ミスがその一因となっていることを御了承願いたい. 結果疾患別では白癬が最も多く,8 例, ついでカンジダ症の 8 例, マラセチア症 例, その他としてスポロトリコーシス 例の報告があった. マラセチア感染症は少数だがほぼ全施設で認められた (Table ). 白癬の症例の頻度が高かった施設は KMUH, ついで MRDC であった. 一番頻度の低かったのは UOEHJ の.% でした. その他の施設は 8% から 8% に分布し, 地域差, 施設差より, 来院する患者層によるのではないかと思われた. 皮膚糸状菌症の病型別では, 多い順に足白癬, 例 ( 男 8, 女 ), 爪白癬 8 例 ( 男, 女 ), 体部白癬 例 ( 男, 女 ), 股部白癬 例 ( 男 8, 女 ), 手白癬 例 ( 男, 女 8), 頭部白癬 ケルスス禿瘡 例 ( 男, 女 ) の順であった (Table ). 足白癬の季節変動についてはほぼ男女同様なグラフが得られた. すなわち 月と 月にピークがみられること,,,, 月はきわめて少なくなることである (Fig. ). 足白癬の年齢別分布については, やはり男女ともほぼ同様のグラフとなった. すなわち 代から 歳代に急速に増加して, 歳代にピークを有するパターンであった (Fig. ). 爪白癬患者は今回 8 例であった. 前回の調査では,8 例であったので, やはり大幅に減少している. この原因も患者の受診への動機つけが減少したものと考えた. 爪白癬の季節変動に関しては足白癬ほどのピークではなく, さらに男女でわずかな違いもみられた (Fig. ). 爪白癬の年齢別分布については女性のピークは 歳代, 男性のピークは 歳代とやや違いがみられた (Fig. ). 爪白癬の菌種の同定については, 帝京大学医真菌センターで一括して realtime PCR 法を用いて行った. 検体を送ってくれた施設ならびに結果は Table に示した. 施設から 件の検体がセンターに送られた. これら

Table.Number of patients with dermatophytoses in each clinic classified by clinical type and gender 新患 Dermatophytes T. pedis T. manuum T. corporis T. cruris T. unguium T. capitis, Kerion 総数 KaDC, 88 MRDC, 8 TUMH, 8 NSSH, IKGH, 8 KMUH, OCUH, TUH, 8 YUH, 8 UOEHJ, KuDC, HUR,8 総計,,, 検体から DNA を抽出して realtime PCR 法を用いて行った. その結果, 約.% と高い確率で菌種の同定ができた. 菌種に関しては, ほとんどが白癬菌で T. rubrum.%,t. mentagrophytes.% そして Candida が.% であった. 今回の疫学調査結果のうち爪白癬以外の起因菌種については各施設から報告がされている.T. rubrum は最多で約 8%,T. mentagrophytes % であった. それ以外の菌についてだが, 当初ほとんどの菌が海外から格闘家によって持ち込まれ, 当初は格闘家に限定していた新型水虫とも呼称される T. tonsurans は頭部から 株, 体から 8 株同定された. 現在では格闘家と接点がない症例や小児, 幼児からの報告も認められる. 本菌は非常に感染力が強く, かつ頭部ではほとんど皮診の認められない症例すなわち無症候キャリアーが存在するため, 分からないうちに感染が拡大する結果となった. 今後なお一層の T. tonsurans 感染症に対する啓蒙活動を続けていかなければならないと考えている. 犬や猫から感染することが多い M. canis は 株で以前より減少したが, 野良猫の保菌率は高いとのことで今 後も注意が必要と思われた. 土から感染する M. gypseum は 株と, やはり土と接触しなくなってきたため減少したと考えた. 股部白癬から E. floccosum が 株培養されたがこの菌は徐々に減少していくものと思う. カンジダの全症例数は 8 例で男女の比率は全症例の平均で :. であった. これはこれまでの報告とほぼ同頻度である. カンジダについては病型の推移について話すが, 男女差が顕著な病型は以前からの報告と同様に爪囲爪炎, 指間びらん症であった. 間擦疹は最も症例数が多い病型である. 男女差はあまりない. おむつ部に生じる乳児寄生菌性紅斑は紙おむつの改良によってずいぶん減少した. マラセチア症マラセチア症は 例の報告があった. 癜風は比較的年間を通じて認められた. 年齢は 代から 代までに渡って認められた. マラセチア毛包炎は 例の報告があったが, 夏季に比較的多く認められ, 年齢は 代から 代が中心と癜風にくらべて若年者に多く認められた. マラセチア毛包炎の男女差は女性で若い例が多い傾向があった.

Fig.Distribution of tinea pedis by age group. 考察日本医真菌学会疫学調査委員会は, これまで 年から 年ごとに 回, 皮膚真菌症の疫学調査を行った. その参加施設は Table のごとくで, 大学病院が減少し, 総合病院も減少した. このことは鏡検, 培養, 同定などを行うことで, 皮膚真菌症をしっかり診断治療する施設が減少していることを意味する. 今回の調査に協力してくれた開業医は, それまで大学病院や総合病院に勤務していた先生である. これらのいわゆる真菌症のスペシャリストが大学や総合病院に継続的に育たなければ, 真菌症をしっかり診断, 治療できる専門医が減少していくことになる. また, 新しく入局してくる皮膚科医に皮膚真 菌症を教育する人材も激減しており, その対策が必要と思われる. 年の疫学調査までは初診患者に対する白癬患者の割合は % から 8% 程度の開きであったが, 今回の調査結果では頻度が施設によって.% から % と大きなばらつきがあった. 施設のうち大学病院に絞って検討してみると, 施設により大きな違いがあることが分かる. 白癬の患者数は 年までは 施設あたりで年間 例以上と多数認められたのに対し, 今回は少ない施設では 例, 多くの大学病院で 例に達しなかった. 産業医大のように初診が全例紹介患者に限定されると, 真菌症の患者は大学にはほとんど紹介されないため症例数が減少する. 著者の病院では患者の訴えがあれば足を診察するが, そうでなければ, マンパワーの問題から訴

Fig.Distribution of tinea unguium by age group. えがない部位の診察までできないため, 足白癬の症例は 8 例に過ぎなかった. 一方, 金沢医大のように来院した初診患者のほぼすべての足を診察すると, 例と高頻度に白癬が認められる. 足白癬の頻度は人口比 数 % といわれており, 本来であれば外来患者の 数 % にみられると予測される. 足白癬の総数は 年, 名, 年, 名, 年,8 名, 年, 名と徐々に減少したが, 今回は, 名と激減した. 減少した一番の理由は, 多数の報告のあった施設から違う施設への変更と考えた. すなわち, 白癬の総患者数はそれほど変わらないと思われるのに病院を受診する症例が減少したのは, 患者の病院を受診しようという動機の減弱が一因であろうと考えた. UOEHJ は, 高度医療を提供する特定機能病院として厚生労働省から承認されているため, 受診には原則とし て, 医院または病院からの紹介状 ( 診療情報提供書 ) が必要となる. つまり皮膚科外来を受診する全例が紹介患者となると, 足白癬や爪白癬では大学にはほとんど紹介されないため, 皮膚真菌症患者が来院しないことになる. 近年, 大病院や大学病院は紹介状がないと受診しにくくなっており, 日常高頻度にみられる皮膚真菌症以外のニキビや水いぼ, かぶれ, 虫刺されなどいわゆる軽症の疾患がそれらの施設に来院しないという結果になる. つまり, 紹介されない軽症の皮膚病の診療を学べなくなってきているので, 真菌症に限らず今後の皮膚科領域の卒後教育でこのような軽症の皮膚疾患についてどのようにしていくべきなのかを考えなければならない. 爪白癬の菌種の同定については帝京大学医真菌センターで一括して real-time PCR 法を用いて行った. その結果,.% と高い確率で菌種の同定ができた. 従

Epidemiological Survey of Dermatomycoses in Japan Epidemiological Investigation Committee for Human Mycoses in the Japanese Society for Medical Mycology (Chairman and Reporter Yoshihiro Sei) Teikyo University School of Medicine Mizonokuchi Hospital Head of Department of Dermatology An epidemiological survey of dermatomycoses and their causative fungus flora in Japan for was conducted in accordance with methods and criteria of the past four surveys. The survey covered a total number of, outpatients who visited dermatological clinics throughout Japan. The results were as follows. )Dermatophytosis was the most prevalent cutaneous fungal infection(,8 cases)seen in these clinics, followed by candidiasis(8 cases)and then Malassezia infections( cases). )Among dermatophytoses, tinea pedis was the most frequent(, cases: male, 8; female, ), then in decreasing order, tinea unguium(8 cases: male, ; female, ), tinea corporis( cases: male, ; female, ), tinea cruris( cases: male, 8; female, ), tinea manuum( cases: male, ; female, 8), and tinea capitis including kerion( cases: male, ; female, ). )Tinea pedis and tinea unguium were seen to increase in the summer season and occur mostly among the aged population. Compared to the last survey, by clinical form, there was a marked decrease in dermatophytosis patients. )As the causative dermatophyte species, Trichophyton rubrum was the most frequently isolated at about 8% among all dermatophyte infections excluding tinea capitis. T. mentagrophytes was about %. Microsporum canis was isolated in five cases. M. gypseum was isolated in three cases, and Epidermophyton floccosum was isolated in only one case. T. tonsurans was isolated in cases. )Cutaneous candidiasis was seen in 8 cases(, male;, female). Intertrigo(8 cases)was the most frequent clinical form, followed by diaporcandidiasis( cases), erosion interdigitalis( cases), genital candidiasis( cases). )Tinea versicolor was seen in cases. Malassezia folliculitis was isolated in cases.

はじめに 皮膚糸状菌はケラチンを栄養源とする真菌の一群で, 皮膚糸状菌による感染症を皮膚糸状菌症という. 日本では, 皮膚糸状菌症を白癬, 黄癬, 渦状癬に分類し, 黄癬, 渦状癬を白癬と独立した疾患としているが, 欧米では黄癬, 渦状癬は白癬に含まれている. 白癬は, 皮膚糸状菌の寄生が角層, 毛, 爪にとどまる浅在性白癬と, 皮膚の深部 ( 真皮 皮下脂肪織 ), 内臓に寄生する深在性白癬に分類されている.
