1 都道府県歯科保健条例の記載事項の比較 都道府県歯科保健条例の記載事項の比較 竹内研時, 相田潤, 岩城倫弘, 田口千恵子 田浦勝彦 3), 小林清吾, 小坂健 Comparison of Prefectural Ordinances for Dental and Oral Health Promotion in Japan Kenji Takeuchi,Jun Aida,Michihiro Iwasiro,Chieko Taguchi Katsuhiko Taura 3),Seigo Kobayashi,Ken Osaka 東北大学大学院歯学研究科国際歯科保健学分野, 日本大学松戸歯学部公衆予防歯科学講座, 3) 東北大学病院予防歯科 キーワード : 歯と口腔の健康づくり 歯科保健条例 フッ化物 喫煙 健康格差 要旨日本では歯と口腔の健康増進を目的として 2011 年 12 月現在 全国 23の道県で歯科保健条例が制定されている 本研究では 各条例の記載事項の特徴を比較することを目的として調査を行った その結果 多くの条例が道県歯科保健計画 全身の健康や市町村との連携 支援 障がい者 要介護者への支援を記載していた その一方で 科学的根拠に基づいた対策であるフッ化物応用や喫煙対策 公衆衛生的な意義が特に大きいフッ化物洗口 国と日本歯科医師会が主体となって進める8020 運動の記載が存在しない条例が少なからず存在した さらに 口腔の健康格差是正にまで踏み込んで記載している条例は3 県のみであった 本研究は 今後条例制定を目指す自治体関係者にとっての基礎資料となるであろう 今後条例を制定する地域においては 大多数の地域で記載されている項目に加え 健康格差やフッ化物応用 洗口 喫煙対策といった項目の記載が期待される 緒言日本では歯と口腔の健康づくりを目指し 2011 年 12 月までに全国 23の道県で歯科保健条例が制 著者連絡先 宮城県仙台市青葉区星陵町 4 番 1 号東北大学大学院歯学研究科口腔保健発育学講座国際歯科保健学分野竹内研時 TEL: FAX: t.kenji64@gmail.com k-takeuchi64@umin.ac.jp 定されている この動きは2008 年 7 月の新潟県での条例制定に端を発し 2009 年には3つの条例 2010 年には11の条例 2011 年には8つの条例と広がりをみせており 今後も制定される地域が増えていくことが予想される しかしながら 条例の内容には差異もみられることから これまでの条例を客観的に比較し 今後条例制定を目指す自治体にとっての基礎資料を作成することの意義は大きい そこで 本研究は 2011 年 12 月の時点で公布されている23の条例について 各地域の特徴を調査し 比較することを目的とした 72
2 ヘルスサイエンス ヘルスケア Volume 11,No.2(201 方法 2011 年 12 月までに制定されている 23 道県の歯科保健条例 ( 表 を用いて 歯科保健計画 全身の健康 口腔の健康格差 市町村との連携 支援 市町村の役割 8020 運動 障がい者 要介護者への支援 妊産婦への支援 小児 学童や高齢者への取り組み 歯周疾患 フッ化物 喫煙についての記載の有無を調査した また フッ化物についての記載の有無に関連する要因の検討を行った これには フッ化物についての記載と 都道府県におけるフッ化物洗口の実施率の関連を検討した 条例の内容により都道府県を 記載なしの群 フッ化物応用 ( 利用 ) と記載する群 およびフッ化物洗口と記載する群の 3 群に分類した その上で 都道府県別のフッ化物洗口を実施している市 ( 特別 ) 区町村の割合との関連を検討するため 一元配置分散分析を行い 相関比を調べた 統計学的有意水準は 5% とし 解析には SPSS Statistics 17.0 for Windowsを用いた 都道府県別のフッ化物洗口を実施している市 ( 特別 ) 区町村の割合は NPO 法人日本むし歯予防 フッ素推進会議 WHO 口腔保健協力センター 財団法人 8020 推進財団共同調査の2010 年 3 月時点のデータを使用した 結果 1. 全身や口腔の健康 市町村との連携に関する記載の比較まず 各条例における 全身の健康や生活習慣病に関する記載および 市町村との連携に関する項目について比較をした ( 表 全身の健康に関しては 健康の保持 増進に関する記載が23 条例すべて (100.0%) 生活習慣病の記載が 12 条例 (52.2%) で確認された 口腔の健康格差に関する記載は3 条例 (13.0%) で確認された 市町村連携に関する記載は 23 条例すべて (100.0%) で確認された 市町村の役割についての記載は 7 条例 (30.4%) で確認された 2. 口腔保健に関する記載の比較次に 口腔保健に関する項目について比較を行った ( 表 3) 歯科保健計画の記載は 23 条例すべて (100.0%) で確認された 8020 運動の記載は 表 1 都道府県歯科保健条例一覧 73

4 ヘルスサイエンス ヘルスケア Volume 11,No.2(201 載が23 条例すべて (100.0%) で 小児 学童期の取り組みの記載が 14 条例 (60.9%) 妊産婦に関する記載が 5 条例 (21.7%) 高齢者に関する記載が 11 条例 (47.8%) で確認された 喫煙に関する記載は6 条例 (26.1%) で確認された 3. フッ化物についての記載と都道府県別のフッ化物洗口を実施している市 ( 特別 ) 区町村の割合の関連一元配置分散分析の結果 都道府県別のフッ化物洗口を実施している市 ( 特別 ) 区町村の割合は フッ化物の記載なしの群とフッ化物応用 ( 利用 ) と記載する群とフッ化物洗口と記載する群の 3 群間で その平均値に有意な差は認められなかった (p=0.39) また フッ化物についての記載と都道府県別のフッ化物洗口を実施している市 ( 特別 ) 区町村の割合の相関比は 0.089で 相関関係はみられなかった 考察 2011 年 12 月までに制定されている全国 23 道県の歯科保健条例について その特徴を調査したところ 記載項目に各地域で違いがみられた すべての自治体が健康の保持 増進について記載しているものの 口腔の健康格差是正にまで踏み込んで記載している自治体はごくわずかであった しかし 近年では日本でも歯科疾患に地域格差が存在することが報告されている Aidaらの研究では地域によって3 歳児のう蝕罹患経験に 2 倍 3) 以上の格差が存在することが報告されており また幼児期から思春期までの児童 生徒 成人 4,5,6,7) 高齢者においても口腔の健康格差が報告されており 健康格差の縮小は 社会環境の改善を含めた地域の課題となっている 実際 健康格差については 21 世紀における国民健康づくり運動 ( 健康日本 2 でも触れられており また国際歯科学会で健康格差が研究テーマに挙げられる 8) など 国内外で取り組みが進められつつあることから 条例に盛り込まれるべきではないかと考えられる 市町村との連携 支援については多くの自治体 が行っていると考えられるが 条例において市町村の果たす役割について規定できているのは7 条例で全体の約 3 割に留まっており サービスの主体となる市町村が歯科保健事業を円滑に行っていくためにも これらを結び付ける実効的な支援規定が今後求められる すべての自治体が歯科保健計画について記載しており さらに国と日本歯科医師会が主体となって進める8020 運動の記載も半分以上の条例で記載されていた 8020 運動の目標にあるように 高齢になっても20 本以上の自分の歯を保つような豊かな高齢社会を達成していくには 歯の主な喪失原因であるう蝕と歯周疾患の対策が欠かせない う 9) 蝕は 歯の喪失原因の 43.3% をしめる最大の要因であり 幼少期からの科学的根拠に基づくう蝕予防対策は必要不可欠である 特に 小学校などでのフッ化物洗口は 厚生労働省から フッ化物洗口ガイドライン も出されており 公衆衛生的特性による健康格差縮小の観点からも望ましいものである さらに すべての住民の歯と口腔の健康度を向上させるには地域単位での取り組みが必要不可欠であり その方策として水道水フロリデーションが存在する この方策は 第 60 回日本口腔衛生学会で 健康社会とフロリデーション として メインテーマにあがっており 10) 科学的根拠に基づく推奨度の高い安全安価な方法である さらに 性別や年齢 収入や学歴に関わりなく有益な最良の公衆衛生手段であり 健康格差の縮小にも有効である 1 フッ化物については 23 条例中 17 条例で記載されていた しかし フッ化物洗口の記載は 6 条例に留まり 水道水フロリデーションの記載は存在しなかった 公衆衛生的意義を考慮して 単にフッ化物応用と記載するだけでなく フッ化物洗口や水道水フロリデーションの記載も今後求められる また 歯周疾患は 歯の喪失の 41.8% を占め 9) う蝕に次いで大きな原因となるが 14 条例で記載が確認された一方で 歯周疾患との関連がよく知られている喫煙については 6 条例の記載と少なかった 喫煙は 歯周疾患以外にも様々な病気の 75
5 都道府県歯科保健条例の記載事項の比較 リスク因子であり 公衆衛生上大きな課題である 近年では 多くの疾患に共通するリスク因子に対して 多部門で共同して対策にあたる共通危険因子手法 (Common risk approach) 1 が進められており この観点からも喫煙対策の記載は欠かせないであろう 科学的根拠が存在して重要と考えられるフッ化物応用 洗口 喫煙対策については 効果的な歯科保健計画立案のためにも 条例への記載が必要であろう 田浦らの研究では 集団フッ化物洗口の実施率には都道府県ごとに有意なばらつきがあることが 13) 報告されており 地域差の存在も明らかになっている 本研究では 23 道県の歯科保健条例におけるフッ化物についての記載と都道府県別のフッ化物洗口を実施している市 ( 特別 ) 区町村の割合の関連を検討したところ 有意な関連はみられなかった これは すでにフッ化物洗口を実施している市 ( 特別 ) 区町村の割合が高いためにフッ化物について条例で改めて明記をしていない自治体や 逆にフッ化物洗口を実施している市 ( 特別 ) 区町村の割合が低いためにフッ化物洗口普及を目的として条例にフッ化物洗口を明記している自治体が混在したためと考えられる 今後は 条例におけるフッ化物についての記載の違いが フッ化物洗口を実施している市 ( 特別 ) 区町村の割合の向上にどのような影響を与えていくかの検討が必要である すべての自治体が障がい者 要介護者の口腔の健康づくりへの支援を記載していた一方で 同様に特に配慮が必要となる妊産婦についての記載は 5 条例に留まった これに関しては 歯科医師と他の保健医療福祉関係者との連携を図って対策を講じる必要性もあり 条例によるさらなる周知が必要であろう 歯科疾患の予防などを推進し 口腔の健康保持を総合的に行うための法律として 歯科口腔保健法が 2011 年 8 月 2 日の衆院本会議で 全会一致で可決 成立した この法律と併せ 各地域の条例により 日本歯科医師会や地域の歯科医師会 地方自治体などが中心となり 口腔保健に関する個 別施策への取り組みがさらに前進することが期待される 14) 今後条例を制定する地域においては ほとんどの条例で記載されている項目に加え 健康格差やフッ化物応用 洗口 喫煙対策といった項目を漏らさずに記載することが必要であろう 文献 都道府県歯科保健条例制定マップ 8020 推進財団 : 年 12 月 10 日検索 ) NPO 法人日本むし歯予防フッ素推進会議 WHO 口腔保健協力センター 財団法人 8020 推進財団 : わが国における集団フッ化物洗口実態調査結果 NPO 法人日本むし歯予防フッ素推進会議 ) Aida J, Ando Y, Aoyama H, et al. In addition, the association between the description about fluoride in the Ordinances and implementation of fluoride mouth rinse in each prefecture was examined. In our results, only three prefectures declared about reducing oral health inequalities. Small number of the Ordinances described about fluoride application for caries prevention and smoking control for periodontal disease prevention which were scientifically well established, and 80/20 Movement. On the other hand, most of the Ordinances described about prefectural dental health planning, cooperation with municipalities, support for disabled and elderly people. The prefecture planning to establish Prefectural Ordinances for Dental and Oral Health Promotion should include the all areas mentioned above in the Ordinances.
